Dietary behaviour in psoriatic patients.

Two hundred and nineteen psoriatic in-patients and 747 non-psoriatic subjects were asked to report in a semiquantitative, self-administered questionnaire on the frequency of consumption of different foodstuffs and cooking and seasoning fats and the daily intake of alcohol (glasses/day) and coffee (cups/day). According to this a definite score was assigned to each of the 46 food items, which were divided into five groups: carbohydrates (CH), low-fat foods (LFF), high-fat foods (HFF), saturated fats (SF) and polyunsaturated fats (PUF) and the alcohol consumption in grams/day was calculated. The mean scores for each food group and the means of alcohol (grams/day) and coffee (cups/day) intake were compared in the two groups, after age-adjustment through ANOCOVA. Psoriatic patients showed higher scores for HFF and SF in males and for CH, HFF, SF and PUF in females and a higher intake of alcohol in both sexes. These dietary habits could explain the higher prevalence of metabolic abnormalities in psoriatic patients.

Red blood cell membrane cation transport in normotensive psoriatics.

Psoriasis might be a widespread membrane disorder. To verify whether under the influence of psoriasis the red cell membrane cation metabolism is altered, we compared 46 psoriatics with 23 normotensive controls. A significant increase was observed in intracellular K+ content, in the maximal velocity of the Na(+)-K+ ATPase, of the Na(+)-K(+)-Cl- outward co-transport, of the Na+/H+ exchange, as well as in the outward passive permeability for Na+. No difference was seen between the two groups in Na+ content, Li+/Na+ exchange or in the passive permeability to K+. In 8 psoriatics treated with etretinate (10-75 mg/day for 1-36 months), Na(+)-K+ ATPase, Na(+)-K(+)-Cl- co-transport and the passive permeability for Na+ were not significantly different from controls. These results suggest that the primary abnormality might be an increased Na+ influx, in part through the Na+/H+ exchange, which is compensated by increased activity of outward transports, and confirm that the red cells are a useful model for the study of membrane transport in this disease. Our results indicate also that these membrane transport abnormalities can be corrected by etretinate treatment.

Second surgical treatment of retroperitoneal persistent disease in epithelial ovarian cancer.

AIMS AND BACKGROUND: This report retrospectively analyzes 9 cases of epithelial ovarian cancer with persistent retroperitoneal metastasis after intraperitoneal surgery (without systematic lymphadenectomy) and chemotherapy. METHODS: All 9 patients were diagnosed as FIGO stage I to IV at the time of primary surgery. They received combined postoperative chemotherapy (8 cases with a cisplatin-based regimen and 1 with adriamycin and endoxan). They were submitted to pelvic and paraaortic lymphadenectomy at the National Cancer Institute of Milan during the period 1990-1994. RESULTS: All patients presented no evidence of disease in the abdominal cavity but retroperitoneal metastasis, which was the unique metastatic site. Chemotherapy was administered as adjuvant therapy after lymphadenectomy. Six patients were free of disease for 14 to 61 months. One patient with vaginal recurrence at the 18th month was treated with radiotherapy and chemotherapy, but died of widespread disease 25 months after lymphadenectomy. Two patients with massive positive lymph nodes dies of brain and lung metastasis 20 and 6 months later, respectively. CONCLUSIONS: We conclude that retroperitoneal metastasis may be the only site of persistent disease and that systematic lymphadenectomy technically feasible in this situation to increase the opportunity for local disease control and to obtain a good result.

Endometrioid carcinoma of the ovary: a retrospective analysis of 106 cases.

AIMS AND BACKGROUND: This report retrospectively analyzes 106 cases of endometrioid carcinoma of the ovary treated at the National Cancer Institute of Milan from 1974 through December 1993. In 12 of the 106 cases (11.3%) a synchronous carcinoma of the uterine body was observed. METHODS AND STUDY DESIGN: Only patients who had previously untreated disease were included in the study. Patients with synchronous tumors were staged according to their ovarian cancer and treated according to the stage of that disease. RESULTS: Thirty-nine patients (36.8%) had stage 1, 17 (16.0%) stage 11, 43 (40.6%) stage III, and 7 (6.6%) stage IV disease. Moderately plus poorly differentiated tumors were present in 76 of the 106 cases (71.7%). Considering the 67 patients with advanced disease, residual tumor was absent in 27 cases (40.3%), < or = 2 cm in 17 (25.4%), and > 2 cm in 23 (34.3%) cases. Systematic pelvic and para-aortic lymphadenectomy was performed in 60 patients (56.6%); selective sampling was carried out in 23 cases (21.7%). After surgery, 77 patients underwent various chemotherapy regimens. CONCLUSION: Using univariate analysis, FIGO stage, tumor grade, residual disease after surgery, lymph node status, and platinum in the chemotherapy regimen significantly influenced 5-year survival. However, when all these variables were included in a multivariate analysis only FIGO stage still had a significant impact on survival. Survival analysis also showed a trend towards longer survival in patients with synchronous tumors.
