Anesthetic considerations in hepatic porphyrias.

Porphyrias are inherited defects of heme synthesis with manifestations that can mimic surgical diseases; they can be provoked by administration of certain drugs. Manifestations such as abdominal pain, vomiting, tachycardia, hypertension, neuropathy, fever, confusion, and seizures have been described. Management of patients with porphyria is designed to avoid triggering drugs, such as barbiturates, and perhaps, benzodiazepines and ketamines. Nontriggering drugs should be used in the management of patients with porphyria. Because starvation can induce an attack, glucose infusions are important in the prevention and treatment of porphyria.

Small medical libraries can help also.

The Regional Medical Library program of the Dana Medical Library (Burlington, Vermont) is described. Practical problems of a regional program are discussed. A plea is made for providing regional service, however little or much, without waiting for surveys and large grants.

Disaster planning at a Friesen concept hospital.

Disaster planning is a task which will probably never be "completed", since to be effective, a disaster plan must constantly be reviewed and modified. The Hospital Disaster Working Committee at the Ottawa General Hospital has so far been presented with both challenge and opportunity. The challenge is to make best use of our new facility, while coping with the changes which a new site predicates. The opportunity is to take advantage of the past experience of our own people and others who are involved in disaster planning, and make a strong beginning.
