Fifty years of surgical management of carcinoma of the lung has been associated with much enthusiasm in a search for improvement in survival rate. Preoperative evaluation (including node sampling), subsequent staging, responsible resection, and avoidance of serious postoperative complications have all led to an increasingly objective system of management. Unfortunately, this has not been associated with a marked improvement in overall rate of survival following pulmonary resection.

Twenty surgical and two autopsy cases of pheochromocytoma are reviewed. Preoperative diagnoses were reliably determined by urinary catecholamine excretion studies in most cases. Arteriography and computed axial tomography were the most consistently useful studies in localization of tumor. Preoperative alpha blockagde with phenoxybenzamine, although incomplete in most cases as judged by intraoperative hypertension or postexcision hypotension, appear to have been beneficial as there was not operative mortality. Continued hypertension after tumor removal can be expected in 25 percent of patients despite normalization of catecholamine excretion studies. The cause of this is unknown, but vigilance for recurrent tumor should be maintained.
1) Paraplegia or death occurred uniformly in dogs whose descending aorta was cross-clamped for 1 hour. 2) Protection was complete in the experimental model in which a passive shunt was used, provided the flow was greater than 8 ml/kg/min.
Ten cases of giant lung abscess were treated by one-stage closed tube drainage after initial medical therapy. Sepsis or hemorrhage was relieved in all patients. There were no operative deaths, serious complications, or late recurrences. Three patients have subsequently died from bronchogenic cancer.
Results in 23 patients operated upon for the repair of aneurysms of the descending thoracic aorta with a TDMAC-heparin-bonded shunt are reported. The results with particular reference to paraplegia are compared with those in 365 patients operated upon by members of The Samson Thoracic Surgical Society utilizing this and other techniques. No technique completely prevented paraplegia.
A vena caval umbrella was implanted in twenty-eight patients for the prevention of recurrent pulmonary embolism. Eighteen of these patients were catastrophically ill at the time of reimplantation. Thirteen patients died after implantation, one from a recurrent pulmonary embolus originating in the vena cava. There have been two additional recurrent nonfatal emboli. Three of the thirteen long-term survivors had significant lower extremity edema. Our experience and that of others indicates that the inferior vena caval umbrella is comparable to other methods of incomplete interruption in terms of effectiveness, morbidity, and mortality. Its unique technical advantage is the simplicity of insertion in severely ill patients in whom abdominal surgery is undesirable.
A total of 104 cases of bronchoscopically biopsy proven bronchogenic carcinoma are reviewed. Twenty-seven of 39 patients with operable lesions underwent pulmonary resection. There are seven patients with squamous cell tumors who are long-term, cancer-free survivors. The operative mortality was low at 2.3 per cent. A positive bronchoscopic biopsy should not be considered a contraindication for exploration, and an aggressive approach for squamous cell lesions seems justified.
