Neurite branching pattern formation: modeling and computer simulation.

A model for nerve cell pattern formation is proposed in this paper. The model is based on some experimental results and an assumption that there is a kind of inhibitive interaction between growing neurites on the same nerve cell. In this paper, this interaction is termed lateral inhibition. A group of ordinary differential equations are used to describe the elongation of the terminal neurite segments of individual nerve cells. Computer simulation and comparison of it with in vitro studies are also made in this paper.

[Surgical treatment of primary liver cancer complicated with cirrhosis].

From 1964 to 1990, 318 cases of primary liver cancer were treated by surgery. Of them, 227 (71.3%) were complicated with cirrhosis to various degrees: mild, moderate and severe. The lesions in the latter group was greater than 10 cm in 3 patients, 5-10 cm in 7 and less than 5 cm in 17. All were confirmed by histopathology to be hepatocellular carcinoma. The method of operation was partial hepatectomy in 23 patients and intra-tumoral absolate alcohol injection supplemented by microwave coagulation in 4. The amount of absolate alcohol ranged from 18 to 30 ml with an average of 24 ml. The chief complications were jaundice and ascites (13 patients--48%) and gastrointestinal hemorrhage (3 patients--11%). Operative mortality was 15% (4/27). The 1-, 3- and 5-year survival rates were 82%, 39% and 10%.

A comparison of injury death rates in China and the United States, 1986.

BACKGROUND: The importance of injury as a public health problem is not well recognized in many developing countries. Data have recently become available on injury mortality in China. METHOD: We compared Chinese injury data based on a 100 million population segment for 1986 with data for the United States. RESULTS: The age-adjusted death rate from all injuries for China exceeds the rate for the US (69.0 vs 61.3 per 100,000). The US has higher death rates from motor vehicle crashes, fires, and homicide; China has greater mortality from drowning, poisoning, falls, and suicide. Especially noteworthy in China are the high drowning rates among young children and the elderly and the high suicide rates in rural areas among young adults and the elderly. CONCLUSION: Injury is an important public health problem in China, exceeding in many respects the problem in the United States. It is urgent for China to place high priority on injury research and prevention.

[Clinico-epidemiological investigation of schistosome-induced hepatosplenomegaly: a community-based study in Jishan, Xinjian County, Jiangxi].

Since the primary objective of mass chemotherapy in schistosomiasis control is reduction of schistosome-induced morbidity, it would be reasonable to assess the impact of a control program on the morbidity in a given population by investigating hepato-splenomegaly associated with schistosomiasis in a schistosomiasis endemic area. In this paper, the authors described the relationship between the prevalence, intensity and morbidity of Schistosoma japonicum infection in terms of stool egg count and ultrasonographically detectable hepatosplenomegaly in a community-based study. It was found that the epidemiological pattern of the infection in this study community was quite different from our usual understanding, that is, the prevalence remained relatively high (39.4%) when the intensity became lower. This unusual pattern might be resulted from intermittent and sporadic chemotherapy associated with frequent exposure of people to the infection in an area of high transmission. It was surprised to note that despite the praziquantel treatment carried out over the past years, the prevalence of hepatosplenomegaly induced by schistosomiasis in this community was still very high, suggesting that intermittent and sporadic chemotherapy might render little impact on schistosomiasis-induced morbidity. The investigation also showed that ultrasonography was a sensitive tool for assessing morbidity associated with schistosome infection.

Localization of small liver tumors.

Thirty-six patients with small liver tumor were diagnosed by alpha-fetoprotein (AFP); sonography, and computed tomography (CT), and underwent hepatectomy. The pathological types included 23 hepatocelluler carcinoma (HCC), 11 hepatic cavernous hemangioma, and 2 secondary liver cancer. In 22 patients, the tumor nodules were located in the right lobe and 14 cases in the left lobe. The diagnostic accuracy rate of CT was 100% for HCC and secondary liver cancer, but for hepatic cavernous hemangioma it was only 72.2%. However, the accuracy rate of sonography was as high as 81.8% for hepatic cavernous hemangioma and only 60.4% for liver malignancies. The positive rate of AFP for the HCC patients of this series was only 66.6%. The method of intraoperative detection of small liver tumor is introduced, if the tumor was invisible grossly or nonpalpable during exploratory laparotomy. In the series, 7 cases in whom the right lobe lesion was too small to be located by routine manual examination during exploratory laparotomy were detected by this method, and all small liver tumors were resected successfully.

[Experimental study of warming and recuperating kidney yang by you-gui-yin].

Yang deficiency animal models were induced by hydrocortisone 1 mg/day for 7-day to male mice. Then these model mice of experimental group were perfused by gastrogavage with 0.5 ml You-gui-yin (YGY) solution, the control group 0.5 ml normal saline 10-day. The plasma circular nucleotide and cortisol were radioimmunoassay by r-immune counter. The content of cAMP of experimental group was 144.24 +/- 33.35 pmol/ml, the control group 109.11 +/- 31.98 pmol/ml (P less than 0.05). The contents of cGMP were 28.39 +/- 10.22 and 45.39 +/- 15.33 pmol/ml respectively (P less than 0.05). The contents of cortisol were 12.42 +/- 2.21 and 8.96 +/- 1.19 micrograms/dl respectively (P less than 0.05). On the other hand YGY to the model mice could raised the living ability at low temperature. The results suggested that YGY had the effect of adjusting system circular nucleotide and promoting the secretion of adrenal cortex.

[Clinical analysis of 107 cases with chemical burns].

From 1978 to 1987 107 cases of chemical burns were treated. The patients were mainly young and middle age people (83.08%), with mean burned areas 17.8%. The majority of involved parts were low extremities. 57.44% of burned eyes became blind. Mean days in hospital were 43 days. Cure rate was 98.13% and death rate 1.87%. We emphasized that burned wounds must instantly be washed down with water, intoxication resulting from chemical matters must be effectively treated. Diagnosis of depth of burned wound by acids and means of prevention of chemical burns were also discussed.

Evaluation of partial hepatectomy for primary liver carcinoma.

From 1964 to 1985, 120 cases of primary liver carcinoma had undergone hepatectomy in our hospital. Seven of these cases underwent hepatic lobectomy, 28 cases had palliative hepatic resection, and 85 cases had partial hepatectomy. We introduced different surgical modes and compared the mortality, survival rates, and complications between the hepatic lobectomy and partial hepatectomy groups. The mortality rates of these two groups were 14.3 and 3.5%, respectively, and the 1, 3, and 5 year survival rates were 83.3, 33.3, and 16.7 and 68.8, 48.1, and 20%, respectively. In the palliative hepatectomy group (28 cases), ten cases received combined radiotherapy postoperation. Most of these cases died during the first year postoperation. Primary liver cancer in Asia is commonly associated with hepatic cirrhosis. We suggest that partial hepatectomy is suitable for such patients. The results of the present series showed that the outcomes of the hepatic lobectomy and partial hepatectomy groups did not differ, but in partial hepatectomy, the operative mortality and complications were reduced, bleeding minimized, and operation time also shortened.

Subclinical primary liver carcinoma.

From 1976 to 1984, 30 cases of asymptomatic primary liver cancer (subclinical primary liver cancer) have been treated in our hospital. The most important approaches used to uncover subclinical primary liver cancer were 1) the use of alpha-fetoprotein (AFP) monitoring in patients with liver disease; 2) the use of an AFP survey among high-risk area populations; and 3) X-ray examination of the diaphragm and sonography examination. The resectability rate of this series was 50%. The 3 and 5 year survival rates after hepatectomy were 62.5 and 57.1%, respectively. Hepatic arterial ligation combined with irradiation for the nonresectable cases had a better result. We discuss here the approaches and the diagnostic criteria of subclinical primary liver cancer.

Inflammatory pseudotumor of the liver.

Since the first report of inflammatory pseudotumor of the liver in 1953, 12 cases have been reported in the literature. The authors report herein five cases of inflammatory pseudotumor of the liver in our hospital. These tumors were resected and the diagnosis confirmed pathologically. Three of the patients were male and two female. Four of these patients had been misdiagnosed as having primary liver cancer by sonography and CT scan. Our experience has shown that if the patient had a long clinical course, few clinical symptoms, good general condition, and a negative alpha-fetoprotein assay, if the tumor was well encapsulated and the liver presented without cirrhosis, the patients should be suspected of having inflammatory pseudotumor of liver. In this series, all five patients underwent hepatectomy. There were no complications after operation. Surgical resection should be considered as the main therapeutic method for inflammatory pseudotumor of liver. If the patient proves unsuitable for surgical treatment, steroid therapy should be the treatment of choice. In this paper 12 cases of inflammatory pseudotumor of liver reported in the literature are reviewed and discussed.

[Changes in blood gas in severely burned patients with acute pulmonary insufficiency treated by high frequency ventilation (HFV)].

UNLABELLED: Since 1982, 39 cases suffering from severe burns with acute pulmonary insufficiency were treated by high frequency ventilation (HFV). Blood gas changes were observed in 15 cases with mean burn areas of 67.7% TBSA, among them, 10 cases complicated with inhalation injury. The indications for HFV treatment are: dyspnea, increase of respiratory rate (greater than 30 times/min.), and PaO2 less than 60 mmHg (8.00 KPa). RESULTS: 30 times of blood gas analysis were studied with or without HFV in 15 cases. The mean PaO2 value was 59.2 mmHg (7.89 KPa) without HFV, while the mean PaO2 value was 109.3 mmHg (14.57 KPa) with HFV. The difference was significant (P less than 0.01). Their results showed that HFV can improve ventilation and raise PaO2 value in severe burned patients with acute pulmonary insufficiency. The mean PaCO2 values were 33.6 mmHg (4.48 KPa) and 32.9 mmHg (4.39 KPa) respectively without HFV and with HFV, no significant difference was found (P greater than 0.05). There was no retention of CO2 in blood when HFV was used. The advantages of HFV were discussed in this paper. This study suggests that it is advisable to use HFV in severely burned patients with acute pulmonary insufficiency.

[Analysis of microbiological flora in the blood and wounds of burn patients].

1266 strains of bacteria and fungi from wound surface and 321 strains from blood specimens were compared from 1980 to 1987. The results showed that Gram-negative bacilli were obviously more than Gram-positive cocci. Staph. aureus and Ps. aeruginosa remained the most important agents of infective complications. E. coli and E. aerogenes increased steadily and Proteus decreased gradually in burn infection. The ratios of various species of bacteria from wound surface were roughly proportional to that from blood specimens. This paper suggests that prevention and treatment of infection caused by Staph. aureus and Ps. aeruginosa are highly important, control of infection resulted from E. coli and E. aerogenes must be emphasized, and Acinetobacter and Fungi infection must be further studied. The care of burned wounds is very important for prevention and treatment of septicemia. Factors resulting in change of germ flora are also discussed in this paper.

[Inflammatory pseudotumor of the liver].

Inflammatory pseudotumor of the liver is not common. Of the 76 patients who underwent hepatectomy in our hospital from 1986-1988, 4 had inflammatory pseudotumor of the liver. It comprised 5.2% of patients subjected to liver resection during the same period. These 4 patients were misdiagnosed as primary liver cancer and desmoid tumor of the upper abdominal wall by ultrasonography and CT scan before operation. The inflammatory pseudotumor of the liver had long clinical course, less symptoms, and clear capsule, tenacity in nature and no liver cirrhosis on operation. The patients received modified hepatectomy without any postoperative complications. Hepatic resection is the treatment of choice. If contra-indicated, steroid can be used.

[The first case of pseudotuberculosis found in China].

This paper reports the first case of pseudotuberculosis in China. From the patient's stool, a strain of Yersinia pseudotuberculosis was isolated. The isolate has all the classical characteristics of Yersinia pseudotuberculosis and belongs to serotype IV.

Surgical management of soft tissue sarcomas, with an analysis of 313 cases.

From 1964 to 1978, 313 patients, 192 males and 121 females, with soft tissue sarcoma underwent surgery in our hospital. All patients have been followed up for over 5 years. One hundred ninety patients (60.7%) had recurrence of the tumor after previous surgery. The tumor was located in the head in 55 cases, in the trunk in 136, and in the extremities in 122 cases. Regional lymph node metastases occurred in 23 patients (7.4%). Fibrosarcoma, neurofibrosarcoma, and rhabdomyosarcoma were the types most commonly encountered in this series. The 5-year survival rate, according to the extent of surgical resection for 117 patients with local resection, was 47.9%; for 75 patients with wide local resection, 62.7%; for 43 patients with amputation, 18.6%; and for 78 patients with local resection combined with irradiation, 48.7%. For the whole series, the 5- and 10-year survival rates were 47.6% and 35.8%, respectively. The 5-year survival rates of stages I, II, III, and IV (TNM classification) were 60%, 46.9%, 40%, and 22.2%, respectively.
