[Techniques of continuous nerve block at the level of the wrist].

Functional treatment without pain is a condition for good results after operations at gliding structures (tendons) or periarticular structures. Good analgesia of the arm and hand is obtained with continuous axillary nerve blocks. Often there is concomitant paralysis and active motion is not possible. We therefore use continuous nerve blocks at the wrist. The technique of these blocks is described in the paper. We use a prefabricated catheter set to introduce the polyethylene catheter and to place it near the nerve as in single shot wrist block. This technique for continuous analgesia of the hand can be used for functional treatment after tenolyses, arthrolyses and stable osteosyntheses of fingers. Median, radial and ulnar nerves can be blocked all at the same time or alone.

[Ileostomy--stabilization by stapler technique].

In order to prevent nipple-sliding of the everted small bowel, conventional Brooke ileostomies in 26 patients were stabilized on both sides of the mesentery by longitudinal rows of staples. There were no early complications. With respect to historical controls with unstapled ileostomies, late complications (retraction, prolapse) could be reduced from about 16 to 0% in a median observation time of 2 years. The ileostomies were stable in size and form and therefore appliances were facilitated.

[Complete reconstruction of the natural anus in complicated rectal prolapse].

In a 44-year old woman a severe case of prolapse of the rectum was complicated by complete incontinence and rectal stenosis. Continence was restored by colo-anal anastomosis between an oral smooth-muscle-plasty and the anal skin introduced in a newly created anal canal by a distal voluntary muscle-plasty. The successfully applied one-stage technique in reestablishing non-voluntary and voluntary muscular continence is described in detail.

[Testing of the renal capsule technic in normal immunocompetent mice].

We have examined the possibility of transplanting syngeneic and xenogeneic tumor material below the renal capsule of normal, immunocompetent mice as a chemotherapeutic test model. Whereas the Lewis lung carcinoma in the syngeneic system was appropriated for the 6-day-test, only 56% of spontaneous C3H mammary carcinomas showed an increase in the size of the transplanted material. An individual sensitivity to the cytostatic agents as an expression of biological variability could be detected for primary C3H mammary carcinomas. Both, xenogeneic lines of human origin, established in nude mice, and fresh resection material from the clinic showed on the normal, immunocompetent mouse no time-dependent, reproducible growth of the transplanted material in control animals. However, growth is considered as a precondition for estimating the effect of therapy. The normal, immunocompetent mouse was in our experiments not suitable for application of the renal capsule assay in the 6-day-test as a recipient for xenogeneic transplantation.

[Animal experimental examination of a phase III study on the effect of the combination therapy of bruneomycin, cytostasan and prednisone].

The effectivity of a combined application of Bruneomycin, Cytostasane and Prednisone has been tested in five qualitatively different experimental animal test models. The selected dose levels and the treatment schedule were based on an instruction given in a phase III-study. No effectivity was demonstrable in any of the tumor systems selected neither with the single components, nor with their combination, though the toxicity clearly revealed a dose dependence.

[Therapeutic effectiveness of carminomycin (CRM) in combination with dibromodulcitol (DBD) compared with the combination vincristine, cyclophosphamide, methotrexate, 5-fluorouracil (VCMF) on some tumor systems (author's transl)].

The antineoplastic effectivity of VCMF was compared with the combined therapy CRM plus DBD in the mouse tumor systems L1210, 1142 A and Lewis lung carcinoma. Therapy of tumor-bearing mice was carried out by administering doses on the basis of LD 10 or with doses directly calculated from the clinical study CMEA 0102. In the most of all experiments VCMF was superior to the combination CRM plus DBD.
