Clinical relevance of intraplaque hemorrhage in the internal carotid artery.

In this study the correlation between hemispherical ischemic symptoms and the presence of intraplaque hemorrhage in carotid plaques has been evaluated. 38 patients who had undergone carotid endarterectomy were examined clinically and the specimens obtained from operation were studied morphologically. The patients were divided into two groups, asymptomatic and symptomatic regarding the territory of the supplying carotid artery. The presence of intraplaque hemorrhage, shown by Ladewig's Trichrom stain, as well as evidence of iron, immunohistochemical stain of hemoglobin and native fluorescence microscopy was noted, and, according to their extension, classified into three degrees. As a result, there was neither a correlation between history of ischemic brain symptoms and the presence of intraplaque hemorrhage, nor between clinical symptoms and extension of hemorrhage. The plaques were high degree stenotic (greater than 80%) in most of the patients and showed various degenerative changes. In 97% of all plaques with hemorrhages surface defects were seen. Our results confirm that intraplaque hemorrhage is one out of a series of pathological events which occurs during advanced atherosclerosis. Blood inflow from the lumen through an already damaged plaque surface is a common event and a correlation with the onset of symptoms is unlikely.

[Acute arterial thrombosis: diagnosis and surgical therapy].

There is an increasing frequency of acute ischemic occlusion due to arterial thrombosis; the incidence has a range from 17% to 60%. In contrast to embolism we can observe a prolonged therapy less interval due to a lesser dramatic clinical course; in thrombotic occlusion only 10% can be treated within 6 h after onset of symptoms. In 11 to 93% of all cases extensive and comprehensive surgical techniques have to be performed for limb salvage. Nevertheless this aim only can be achieved in 60-84%.

[Value of intraoperative lysis therapy as an adjuvant measure after late surgical embolectomy].

The effect of intraoperative local fibrinolysis after delayed catheterembolectomy (CE) of occluded arteries was examined in a model in the dogs hind limb; the superficial femoral artery was occluded by a rapid injection of an autologeous blood clot and ligation. Embolectomy was performed 24 h after occlusion with or without additional local fibrinolysis (urocinase, r-tPA). The success of revascularisation was estimated by measurement of the total periphereal resistance. There happened a total re-occlusion in all vessels within 48 h if only CE was performed; in contrast after additional fibrinolysis a complete revascularisation could be achieved.

[Surgical treatment of asymptomatic carotid stenosis].

A total of 274 patients (mean age 67.1 +/- 8.2 years) with asymptomatic stenosis of the internal carotid artery greater than 70% was treated by desobliteration and patchplasty; 332 reconstructions were performed either uni- or bilaterally. In this group only 29% revealed a one vessel disease. Lethality within 30 days was 1.5%; the incidence of complete stroke amounted to 0.9%. In a mean follow up period of 30.8 +/- 20.2 month to lethality rate was 4.4%; 7.8% developed a completed stroke and only 3.8% were related to the operated site. The 5-year-survivalrate was 88.9% and 93.2% of this cohort was neurological free of symptoms.

Long-term investigations of serum cholesterol, serum triglyceride, and HDL cholesterol in heritable hyperlipidemic rabbits.

The purpose of this investigation was to follow the course of the serum cholesterol, serum triglyceride and HDL cholesterol concentrations of heritable hyperlipidemic (HHL) rabbits (34 animals in total) in order to obtain an early prediction of these parameters, which appear to be highly associated with the presence of arteriosclerotic lesions. Measurements were done at 6-week intervals for one year, beginning at the 10th week of age. At week 10 the serum cholesterol concentration of females was 652.80 mg x dl-1 +/- 103.82 (standard deviation) and that of males was 648.84 mg x dl-1 +/- 146.04. The 10-week serum triglyceride concentration of males was 267.90 mg x dl-1 +/- 93.03, which decreased in two marked steps after week 10 and 40, while that of females was 246.70 mg x dl-1 +/- 67.51, which reached a maximum at 7 months and then decreased. Both the serum cholesterol and the serum triglyceride concentrations decreased significantly in males and females during the investigation period. The 10-week HDL cholesterol concentrations (males: 18.08 mg x dl-1 +/- 12.81; females: 14.61 mg x dl-1 +/- 8.56) remained relatively constant in both sexes during the investigation period. We consider one or two samplings, usually at weaning and at the beginning of an investigation, to ensure reliable biochemical parameters for the state of the animals.

[Primary gastrointestinal lymphomas].

From 1980 up to 1987 seven patients with primary gastrointestinal lymphoma were treated at the surgical department of the Medical University of Lübeck; six of them were women. The mean age was 73.3 years. The tumors were located in the stomach (4), the large bowel (2) and the small intestine (1), representing disease at Ann-Arbor-stage IE (5) and IIE (2). All patients underwent primary surgery, except one due to his bad general conditions. Irradiation-therapy followed in one case, chemotherapy in two cases when relapse of disease was revealed. Three patients are still alive (0.8 to 6.8 years) without evidence of recurrent lymphoma. The success of surgery as a curative treatment is closely related to the stage of disease; both are important factors influencing prognosis. Surgery is still necessary for diagnosis, tumor-staging and prevention of regional complications as bleeding or perforation, especially when conservative therapy is performed.

Can hemofiltration increase survival time in acute endotoxemia--a porcine shock model.

In a porcine endotoxin shock model employing a continuous intravenous administration of Salmonella abortus equi endotoxin the cardiorespiratory and metabolic parameters were studied with main emphasis on the effect of hemofiltration (HF) as the only therapeutical measurement on the enhancement of survival time. Arachidonic acid (AA) metabolites Thromboxan B2 and 6-Keto-PGF 1-alpha could be lowered significantly by hemofiltration. Measuring the inadequacy of the supply and delivery systems in terms of O2-uptake, CO2 production, lung mechanics, TPR, CO, heart rate and MAP the control group seemed to be more severely compromised than the hemofiltrated groups, although the final outcome as for survival time could not be increased significantly. HF can nonselectively counteract some toxic effects of shock mediators without depriving the organism of beneficial components of a protective system being stimulated at the same time. Once the AA cascade is initiated, pharmacologic inhibition is of limited value as long as a direct specific therapeutic manipulation is still not available. Elimination of mediators by HF helps to combat the overstimulation of host defense mechanisms in ET shock which represents the ultimate threat to the host.
