[Functional and aesthetic refinements of free flap coverage at the dorsum of the hand and distal forearm].

INTRODUCTION: Free coverage of exposed functional structures of the hand and distal forearm can be achieved using a variety of free flaps. However, there is a lack of data in the literature which tissue components are best used for defect coverage of this specific area regarding aspects such as tendon gliding and tissue elasticity. The purpose of this retrospective study was to compare and to evaluate the functional and aesthetical results using free cutaneous, fascial and muscle flaps. PATIENTS AND METHODS: Between 1994 and 2002, a total of 56 patients underwent free flap coverage at the dorsal side of the hand and forearm at our institution. Of these patients, 20 with 21 free flaps met the study criteria and were available for follow-up examination. Depending on the tissue component used patients were allocated into three different groups. In group 1 eight patients received a fascial flap, in group 2 eight a cutaneous flap and in group 3 five a muscle flap. The mean follow-up was 50 months (range: 4-103 months). At follow-up examination the patients answered the Client Satisfaction Questionnaire (CSQ 8) and the DASH questionnaire. RESULTS: Regarding range of motion of the wrist and fingers, fascial flaps clearly showed the best results. Concerning grip strength and pinch grip, fascial and cutaneous flaps demonstrated the same results. Patients with cutaneous flaps showed the best DASH score. Regarding the overall aesthetical outcome, fascial and cutaneous flaps were far better than muscle flaps. Donor site morbidity was lowest for fascial flaps followed by muscle flaps and was highest for cutaneous flaps. CONCLUSION: Cutaneous and fascial flaps had the best functional and aesthetical results. Fascial flaps achieved the best aesthetical outcome of the donor site. We recommend free fascial flaps and cutaneous flaps as the first choice due to their advantages in the specific area of the dorsal site of the hand and distal forearm which requires pliable and thin tissue coverage.

[Relations between general pathology and clinical medicine].

After two preliminary remarks concerning the kind of approach in modern scientific research and the relations which exist between structure and life the author at first deals with the present state of the general and special pathology as well as the modern clinical medicine. Following this the change is discussed, which is outlined in the correlations between clinical medicine and pathology, and possibilities for a repeated closer cooperation are investigated.

[The relation of general to special pathology].

As most important fundaments of the general pathology the theory of the mechanisms of pathogenesis, the close relations to the general biology and the theory of science are described more in detail. Moreover, every pathologist must deal with the conditions of general-pathological evidence concerning the theory of cognition. Finally it is extraordinarily important to show the significance of the general pathology and of the constituting it theoretical thinking for the pathologist, the physician and the student. Since medicine is more and more studied under theoretical scientific viewpoints, for every physician a scientific basis knowledge is necessary.

[Some theoretical principles of aging research and its use in the aging of arteries. B. The aging of the arterial wall].

Processes of ageing on blood vessels can be made understandable only when special characteristics of structure and function of the artery are known and when a methodology is used which corresponds to these features. In these cases not only a reduction of structural elements are the results, but also qualitative alterations in the composition of fibres and of the basic substance as well as a cell proliferation of the area of the intima. The process of ageing in the arterial wall proceeds discontinuously and with local accentuation. Particularly weighty is the changing of the collagen and glycosaminoglycan synthesis which is dealt with in detail. In cooperation of cells, fibres and basic substance in the arterial wall exists a "micromilieu", the condition of which intensely influences the process of ageing. Influences of ageing on the side of the entire organism are, of course, mentioned in the present paper, but not treated in detail.

[Various theoretic principles of geriatrics and their application to aging of arteries. A. Aging on different organizational levels of living organisms].

At all times "theories of ageing" have played a leading part in gerontology. The experimental theoretic gerontology has elaborated interesting findings on monocellular organisms and insects as well as on small laboratory animals. Recently, models of cells and ageing at cellular and molecular level play an increasing part. Also the physico-chemical alteration of the condition of macro-molecules has contributed to the clarification of ageing processes. It will be a task of future to clarify the ageing of higher organisational forms of life (organs, systems of organs, organisms) better than up to now. The laws of systems active in these cases are complex and cannot be explained by simple addition of cellular or subcellular ageing mechanisms.

[Trends in morphological research as a reflection in the Greifswald chair of pathological anatomy].

Virchow's concept of cellular pathology was chosen as the starting point for this report on the development of our branch of science since the establishment of the Greifswald Chair of Pathological Anatomy in 1856. The great enthusiasm for carrying out experiments, the dynamic power and creativity possessed by the representatives of our subject, which assisted in the combination of practical medicine with theoretical knowledge, has been described at the instance of three heads of department (Grohé, 1858-1885; Grawitz, 1886-1921; Leupold, 1926-1930). They laid the foundations for a modern morphological terminology, which takes account not only of the spacial order of the structural components but also of their modifications in the course of time (processes) according to natural laws. Eventually, the current position of our subject was analyzed and broken down into three problem areas, which are of great importance for the future development of pathological institutes.

[New aspects in the pathologic anatomy of arteriosclerosis obliterans].

The pathogenetic notion of arteriosclerosis and the clinical syndrome of the arterial obstructive diseases are not identical, but interfere in different points which are discussed in detail. Arteriosclerotic changes must be delimitated from pure age-conditioned changes of the arteries, since the two are sufficiently defined and deviate from each other in important characteristics. Only the so-called fibrous thickening of the intima is still doubtful concerning its classification. For the understanding of the severe obliterating arteriosclerosis the knowledge of the early stages is necessary. The disease begins with focal proliferations of smooth muscle cells and a disturbance of permeability of the arterial endothelium. The two cell forms are discussed more in detail and estimated in its significance for the vascular disease. Particularly the lesion of the endothelium nowadays plays an increasing role as causative factor for the development of an arteriosclerosis, since already simple increases of permeability via and edema of the intima may lead to an accumulation of lipoproteins in the vascular wall, also when the lipid level in blood is not increased. At present in clinical practice often the question of the delimitation of an obliterating arteriosclerosis from other arteriopathies leading to a vascular obstruction arises. Since a part of these diseases is to be diagnosed by biopsies of the arteries, the histological differential diagnosis of the most important inflammatory arterial diseases is discussed.

[Liver cirrhosis--classification, etiology, pathogenesis, progress, reversibility].

On the basis of our modern knowledge of structure and function of the liver at first the cause of the liver cirrhosis and the essential possibilities of classification are discussed. A further important problem is the question of progression and regression of the liver cirrhosis. Here special immunomechanisms play a part, the possible influences of which on a progressing of the disease are demonstrated in detail. Of particular importance are, apart from this, the influence of the virus hepatitis and the question, how many liver cirrhoses are of hepatitic genesis. Also, the importance of alcohol and the mechanisms which from an alcoholic liver damage form a cirrhosis are treated. Finally the author deals with the evidence of the diagnostics of biopsy, in which case, apart from the diagnostics, first of all the prognostic judgment of the development of the cirrhotic process is of importance.

[The "greifswald-therapy-model" for carcinoma of the cervix uteri for clinical testing of the cancer-multistep-therapy after M. von Ardenne (author's transl)].

The results of a preliminary study of the cancer-Multistep-Therapy (C.M.T.) (so called "The Greifswald-Modell") are reported. This Greifswald-Therapy-Model has been developed by hitherto existing experiences in the clinical test of the C.M.T. 4 patients with carcinoma uteri T2bNOMO to T2bNOMO level epithelium-carcinomas histologically verified showed after a preoperative treatment with C.M.T. and cantactradiatio (60CO) under the employment of a reduced dose of radios (1/2 to 1/3 of the pretherapy used in this clinic or 1/4 to 1/5 of the tumor dose) a complete regression of the tumor-specific changes. In the surgical specimen won after 4-7 weeks after C.M.T., carcinoma could not be detected either macroscopically or microscopically. Patients with a good therapy -- compatibility show a liberty of relapse with an increase in weight and an excellent physical and psychical health two year after C.M.T. This preliminary study shows the capability of the well - chosen therapy-model for the clinical test of the therapy-effect of the C.M.T. and it urgently demands to carry on with adequate alternate rows by including other conventional therapy schemes and other kinds of tumor to estimate definitively the effect of the C.M.T. in the tumor therapy.
