[New method of colorectal anastomosis in children with Hirschsprung disease].

Developed method of treatment of Hirschsprung's disease lies in removal of injured part of the intestine and establishment of oblique termino-terminal colorectal anastomosis between pull through colon and rectum resected from parasacral-perineal approach. Proposed method ensures complete removal of aganglionar site, favorable conditions for anastomosis and healing that decrease probability of postoperative complications. Operation was performed in 7 patients. There were no immediate and remote complications.

[Comparative evaluation of various methods of surgical correction in Hirschsprung's disease].

An analysis of results of treatment of 46 patients with Hirschsprung's disease has shown that complications at the postoperative period were observed in 17 of 39 children subjected to radical operations by the methods of Duhamel, Swanson and others. A number of common and specific for each traditionally used method disadvantages responsible for the results of treatment were found out. The authors have developed a method of treatment of Hirschsprung's disease including ablation of the aganglionary zone and forming the termino-terminal colorectal anastomosis between the descended colon and the rectum mobilized and resected from the parasacral-perineal access. The new method was used in operations on 7 patients without complications.

[Intestinal desinvagination in children].

168 cases of conservative and 52 cases of surgical desinvagination are reported. The push-like insuflation of oxygen (under the pressure of 130 mm Hg) in large intestine after gaining the pressure of 20-30 mm Hg was preformed in case of conservative desinvagination. The electrometric control and management of the procedure has been performed. The following 3 parameters of biological beed-back were regarded: the acoustic noise, the fall of oxygen pressure in the rectum and changes of the abdomen volume. The method was effective in 91% of cases.

[Characteristics of the surgical treatment of total duodenal atresia].

The authors analysed the results of surgical management of total duodenal atresia in 12 newborns. In 10 of them the operation was successful. Duodenojejunoanastomosis on a short jejunal loop was formed through a window in the mesocolon in all patients. The success of the treatment is determined by careful performance of all the details of the surgical intervention. Of particular importance are such elements of the operation as the formation of the spur from the afferent genu of the jejunum and fixation of the anterior duodenal wall in the window in the mesocolon. It is preferable to create the anastomosis with a single row of interrupted sutures of synthetic atraumatic material 5/0-6/0 with the knots in the intestinal lumen.

[Treatment of hygroma in children].

Treatment of 192 children for hygromas of various localization by different methods is analysed. The results of histological examination showed these structures to be of dysplastic nature. An aspiration-cystotatic method of treatment was developed, which can be applied in outpatient practice and causes neither local nor general complications.

[Unusual variants of proctoplasty in Hirschsprung disease].

The results of surgical treatment of 42 children with Hirschsprung's disease are analysed. The authors describe unusual 2-stage techniques of the perineal part of Duhamel's operation, consisting in primary downward transposition of the colon excess to the perineum and its subsequent (14 days later) removal with the formation of an intestinal intrapelvic wedge anastomosis. The method was applied in 23 children. A method of small intestinal proctoplasty in reconstructive operations in unfavourable anatomical variants of the colon is also described; it was carried out successfully in 2 patients.

[Anatomo-experimental parallels in the elaboration of methods of increasing the viability of non-free muscle flaps used for sphincteroplasty].

As demonstrate morphological investigations of transplants, obtained from thin, rectal, long adductor and semitendonous femoral muscles in 100 animals after sphincteroplasty, the methods elaborated contribute to preserve vital capacity of the muscle flaps both soon after and in a long time after the operation. Rearrangement of the blood bed in the muscle transplants and the colon sphincters formed is completely achieved by the 30th day after the operation. The blood bed becomes more powerful than the normal one.
