[Prevention of endogenous infectious complications in patients after severe trauma and orthopedic surgery].

The clinical efficacy of a complex procedure for the prophylaxis of purulent septic complications in patients requiring pulmonary artificial ventilation after severe traumata and orthopedic operations, as well as in patients with the endoprosthetic appliance of the hip joint and the resection of bone tumors was studied. The procedure includes a selective decontamination of the gastrointestinal tract and oral cavity, the oral administration of lactic acid bifidum-bacterin (eubiotic) during the entire period of the pulmonary artificial ventilation and a short-term systemic administration of a 3rd generation cephalosporin. The autoflora and microflora of the pathologic foci in the patients were tested. There were observed regularities in the colonization of the mucous membranes and wounds with aerobic opportunistic organisms and the endogenic pattern of the infectious complications in the patients subjected to the pulmonary artificial ventilation. The use of the complex procedure for the prophylaxis resulted in a significant decrease in the number of the patients with purulent septic complications.

[The range of antigenic specificity of Bifidobacterium peptidoglycan].

The antigenic relationships of Bifidobacterium bifidum 1 peptidoglycans with different strains of this species (LVA-3, 791, GO-4), bifidobacteria of other species (B. adolescentis GO-13, B. breve 79-38, B. lactentis 79-41, B. longum GO-3) and bacteria of remote taxonomic groups (Streptococcus faecalis 6-3. Staphylococcus aureus COM 885, S. epidermidis COM 2124. Lactobacillus plantarum 1, Escherichia coli M-17) were studied on the basis of a highly sensitive test system permitting the registration of normal human antibodies to peptidoglycans. The level of cross reactions with staphylococci and streptococci correspond to intraspecific and antigenic affinity to L. plantarum and E. coli was considerably less pronounced. Copying a number of epitopes of bifidobacteria, S. aureus peptidoglycan seems to possess additional antigenic determinants which participate in the formation of immunological responsiveness in man.

[The efficacy of bifidum-containing preparations in enterogenous reactive arthritis].

Analysis is made of the intestinal microflora in 43 patients suffering from reactive arthritides (ReA) that developed after intestinal infection. The overwhelming majority of the patients manifested a decrease of the level of the bifidoflora. The disorders of the intestinal microflora were related to the disease activity and standing. Bifidumbacterine was found to produce a significant beneficial effect on the course of ReA and to cause no serious side effects. According to the preliminary data, the efficacy of bifidumbacterin exceeded that of the known basic drug salazopyridazine, thus making it possible to apply the bifidum-containing drugs as basic agents in the treatment of ReA.

[Microbiocenosis and the status of the intestinal mucosa in alkylosing spondyloarthritis].

Intestinal microbiocenosis was examined in 53 patients with ankylosing spondyloarthritis (AS). Of these, 23 had the central and 30 presented with the peripheral form. 34 patients underwent endoscopic examinations (rectoromanoscopy, ileocolonoscopy). As a result, in 74% of cases with the central form of AS and in 80% of cases with the peripheral form of AS, alterations in the intestinal microflora were revealed, which can be regarded as intestinal dysbacteriosis (ID), with dysbacteriosis being more pronounced in patients with the peripheral form. In such patients it manifested by a considerable decrease of the amount of bifido- and lactoflora, which promoted the increment of the amount of opportunistic microorganisms, particularly Klebsiella. Macroscopic signs of nonspecific inflammation of the small and large intestines were identified in 63% of patients with the central and in 67% of those with the peripheral form of AS. Mean-while histomorphological study of biopsy specimens of the intestinal mucosa showed that all the examinees had chronic inflammation of varying degree.

[Isolation of plasmid DNA from Bifidobacterium].

Conditions for accumulation of the biomass and a procedure for isolation of plasmid DNA from bifidobacteria in microquantities were developed. It was shown that all the strains tested had 1 to 3 plasmids of different molecular weights electrophoretically detected. Relation between the detected plasmid DNA and bifidobacteria resistance to tetracycline and fusidin as well as utilization of some carbohydrates is discussed.

[Microbial ecology of the intestines in health and in pathology].

Bifidoflora constitute 85--95 per cent of the large intestine microbiocenosis in children under 1 year of age. Bacteroides are not specific of children under 6 months of age. Lactobacilli, lactic acid streptococci, colon bacilli, enterococci and staphylococci (saprophytic, epidermal) constitute not more than 15 per cent. The predominating group peculiar of eubiosis in adults includes bifidobacteria and bacteroides. Changes in human autoflora are often induced by such factors as impaired ecology, stress, uncontrolled use of antibacterial drugs, radio- and chemotherapy, etc. Dysbacteriosis (first of all lower quantitative contents of bifidoflora) has an unfavourable effect on the intestine secretory function, absorption and certain indices of protein, lipid and mineral metabolism, vitamin synthesizing and fermentative functions and leads to disfunction of the gastrointestinal tract. It is a cause of persisting and relapsing infections in children and adults. High and optimal levels of bifidoflora in the intestine usually prevent the pathogenic action of pathogens. Lysozyme of digestive secretion and secretory immunity with immunoglobulin A prevalence also play an important role in the complicated mechanism of the protective barrier. The use of bacterial preparations such as bifidumbacterin, lactobacterin or bifilact for stabilization and recovery of high bifidoflora levels is indicated in all the cases with impaired microbiocenosis of the intestine.

[Qualitative and quantitative composition of acids formed by bifidobacteria].

The dynamics of acid production in 18 strains of bifidobacteria, belonging to 5 different species, has been studied Bifidobacteria have been found to produce 3 acids, lactic, acetic and formic, in the process of their metabolism. The lactic acid/acetic acid quantitative ratio varies, depending on the culture medium on the average, 1:2 in Blaurock medium, 1:5 in milk hydrolysate medium. Various strains have also been found to differ in the dynamics of acid production with respect to the amounts of lactic and acetic acids. The study has shown that, despite the active production of acids for a period of up to 72 hours, a decrease in the pH of the medium is observed for not more than 48 hours. The existence of a specific mechanism permitting bifidobacteria to regulate the acidity of their environment is supposed.

[Human Bifidobacterium flora, its normalizing and protective functions].

The paper is concerned with investigation of the normalizing and protective role of the Bifidobacterium flora in humans and its quantitative predomination in intestinal microbiocenosis of children and adults. The data on antagonistic properties of bifidobacteria with respect to pathogenic and facultative pathogenic bacteria, the Bifidobacterium flora influence on the host metabolism and efficiency of bifidobacterin in recovery of the Bifidobacterium flora optimal level are presented.

[Joint use of antibiotics and antibiotic-resistant bifidobacteria in treating acute intestinal infections].

Acute dysentery and other acute intestinal diseases with obscure etiology are accompanied by pronounced intestinal dysbacteriosis which is manifested, on one hand, by the increased levels of Escherichia, enterobacteria, staphylococci and by the appearance of Klebsiella and Proteus in large amounts and, on the other hand, by an essential decrease in the number of lactic acid streptococci, lactobacteria, bifidobacteria. The treatment of dysentery and other intestinal diseases with obscure etiology with antibiotics, while eliminating the infective agent, leads to the aggravation of intestinal dysbacteriosis. The combined use of antibiotics and antibiotic-resistant bifidobacteria in intestinal infections with obscure etiology contributes to the effective restoration of normal intestinal microflora already in the process of antibiotic therapy.

[Use of antibiotic-resistant Bifidobacterium in treating ulcerative-necrotic enterocolitis in young children].

Ulceronecrotic colitis in premature children is accompanied by the development of pronounced intestinal dysbacteriosis characterized by a sharp increase in the number of Escherichia, enterococci, staphylococci and the appearance of opportunistic enterobacteria (Klebsiella, Serratia, Citrobacter, Proteus) in large amounts. Clinical convalescence was observed in 2 weeks in children receiving antibiotic-resistant bifidobacteria with antibiotics and in 3-4 weeks in children receiving commercial bifidobacteria with antibiotics, while children treated only with antibiotics showed no signs of clinical convalescence during the whole course of treatment. After the course of treatment the most effective recovery of the intestinal microflora was observed in the group of patients who had received antibiotics in combination with antibiotic-resistant bifidobacteria. It was manifested by a decrease in the number of Escherichia, Klebsiella, Proteus, Serratia, staphylococci and enterococci simultaneously with an increase in the number of endogenous lactobacilli and bifidobacteria. In those children who had received commercial bifidobacteria in combination with antibiotics or had been treated only with antibiotics the process of the recovery of the intestinal microflora was considerably less pronounced than in the patients of the first group; a decrease in the number of Escherichia, Klebsiella, Proteus and Serratia occurred to a lesser extent, and at the same time an essential increase in the number of enterococci and staphylococci was observed. The level of endogenous lactobacilli and bifidobacteria was considerably reduced. Antibiotic-resistant bifidobacteria actively took in the intestine of the patients. On day 5 after the course of treatment was over their level was (4.3 +/- 0.4) X 10(4) cells/g feces.

[Bifidobacterium flora status of cosmonauts before and after completing space flights].

The state of bifidoflora was examined in eight cosmonauts before and after space flights of varying duration. Prelaunch quantity of bifidobacteria decreased drastically. In space flights of 30 days and longer the level of acid formation by bifidobacteria was reduced. The species composition of bifidoflora remained stable. The most typical intestinal species of bifidobacteria were B. Longum and B. adolescentis.

[Effect of lactobacillus and bifidobacteria preparations on the intestinal microflora of mice irradiated with gamma quanta].

Lactobacilli (strain B12G) and bifidobacteria (strain 75-41), administered orally in a dose of 5 x 10 cells to CBA mice on days 1, 3, 5 and 10 after irradiation with gamma quanta in a dose of 700 rad, restored the "eubiosis" of the intestinal tract, specifically suppressing opportunistic bacteria and facilitating the normalization of the quantitative and qualitative correlation between microbial associations constituting the obligatory intestinal flora. For one thing, the preparations of lactobacilli and bifidobacteria, used for treatment, restored the amount of lactobacilli in the intestinal tract of the irradiated animals to the level, characteristic of the intact animals; for another, these preparations prevented the dissemination of Escherichia, Proteus, Enterococcus in the small intestine and considerably decreased the amount of these microorganisms, as well as Clostridium, in the large intestine.

[Morphology of the bifidobacteria by light and electron microscopy and their biological properties].

The morphology and the acid-producing, antagonistic and enzymatic activity of the strains of bifidobacteria isolated from the intestine of adults and children were studied. Bifidobacteria showed a considerable polymorphism, all the strains were antagonistically active. The strains isolated from adults were found to have greater acid-producing activity; the predominant species were B. longum and B. adolescentis, and in children B. bifidum were also isolated. The characteristic feature of B. longum strains was the presence of a slime-like layer and formations resembling bubbles around bacterial cells. The stains with greater physiological activity were found to have an extensive mesosomal complex, as well as a great number of volutin granules.
