[Models of inter-group difference of the homeostasis system in patients with artificial heart valves and the set of laboratory parameters].

Patients with artificial heart valves (AHV) belong to a unique multi-component model of the homeostasis system presenting an interaction of activated coagulation processes and pharmacological effects, on the one hand, and genetic peculiarities functioning homeostatic mechanisms, on the other hand. The described method was evaluated on the basis of multi-factor mathematic analysis by information-metry. The data obtained provided for isolating the most information-dense laboratory indices and some genetic polymorphisms describing the shaping of continuous intravascular coagulation of stage III in AHV patients. The clinicians and laboratory experts are offered a practical instrument for the diagnosis (prognostication) of the above condition. It was proven as necessary to examine (by the morphofunctional method) the intravascular activation of platelets and to determine the D-dimer in the remote period after prosthesis of heart valves.

[The influence of hereditary thrombophilic mechanisms on the degree of permanent intravascular coagulation in patients with artificial heart valves].

The genotyping of 40 patients with artificial heart valves (AHV) was performed after prosthesis of the mitral and aotic valves with bicuspid AHV (Medinzh-2 and CarboMedics). The patients took phenylin and varfarin. The patients' genotype was estimated by the thrombophylic genes: factor V Leiden (FVL), prothrombin G20210A, methylene tetrahydrofolate reductase C677T, G/A--455FGB, 4G/5G PAI-1, PI A1/A2 GPIIIa. The genes determining the thrombocytic activity or the vascular wall state substantially influence the third degree of the intensity of the permanent intravascular coagulation (PIC-3) independent of the degree of correction of hemostasis of oral anticoagulants. The addition of anti-aggregants to therapy is the only that can normalize functional activity of thrombocytes in patients with AHV having such defects. The laboratory detection of the genetic defects is of great practical importance for the determination of risk groups of formation of PIC-3 and the strategy of antithrombotic protection of patients with AHV.

[Prognostication of obstructive syndrome in patients with chronic bronchitis considering hereditary factors].

AIM: To propose method of accurate prediction of chronic obstructive pulmonary disease (COPD) at early stages of development. MATERIAL AND METHODS: 92 males with COPD and 55 males with chronic non-obstructive bronchitis aged 38 to 72 years were examined according to a technique which identifies hereditary predisposition to multifactor diseases and using an algorithm based on multivariate analysis. RESULTS: An original mathematical model is proposed which can quantitatively examine and range by influence the role of environmental and hereditary factors in development of COPD. CONCLUSION: Detection of hereditary predisposition to COPD may be one of the objective criteria to recommend giving up smoking and for occupational orientation.

[Choice of therapeutic tactics in treatment of endogenous depression by means of statusmetric expert system].

There was elaborated expert models based on computer data base including 42 formalized signs (anamnesis, state of the patients, medical measures and indices of expert estimation of the response to therapy, based on the reduction of scores of Hamilton's scale). The study was carried out in 104 in-patients whose clinical states corresponded according to ICD-10 to category "depressive episode" and "recurrent depressive disorder". The patients were divided into 2 groups: in the first one the treatment was performed by serotoninergic antidepressants (SA)-fluoxetine (20 patients), fluvoxamine (20), sertraline (30). Tricyclic antidepressant (TAD) amitryptiline was administered to 34 patients of the second group. Two data bases were formed: responders to TAD and responders to SA. Natural pair model of classification (error of the model--12.5%) including 9 informative signs, was constructed, that gave chance to define probability sensitivity to TAD and SA. Check-up of computer model revealed that 3 patients of SA group didn't submit to decisive rule, while there were found 2 such patients in TAD application. Application of computer experiment permitted to turn from group prognosis to individual one.

[The diagnosis of aspirin-induced asthma by using a functional model].

The analysis of case history, clinical, laboratory and instrumental findings, objectively characterizing the patient's condition in two alternative groups (nonaspirin and aspirin bronchial asthma) enabled the authors to create a functional diagnostic model for aspirin bronchial asthma. The model-based computer program designed by the authors can perform early diagnosis of aspirin asthma outpatiently without any risk for the patients.

[Status metric analysis of epidemiological data on congenital developmental defects caused by environmental pollution].

Status metry was used to determine the portion of congenital developmental abnormalities (CDA) of obscure etiology (probably of mutation origin) among the total number of CDA in 3 cities of the Ukraine. The contribution of CDA determined by new mutations correlates with the reported data and corresponds with the level of atmospheric air pollution in the cities under study. Status metry made it also possible to define the tendencies and magnitude of the influence of factors involved in CDA formation. The conclusion is made about adequacy of the use of status metry for indirect estimation of the rate of the mutation process and its modification under the effect of environmental pollution.

[Systematic analysis of the state of man exposed to radio wave irradiation for a long time].

A complex examination was made of 72 men--short-wave (3-30 MHz) radio equipment controllers. Two parallel control groups of 70 people all in all were examined. No regular changes in the dynamics of amplitude-frequency characteristics of electroencephalogram, associated with the length of service, were observed. The number of subjective complaints from people in contact with radiofrequencies increased with an increase in the length of service. The role of certain clinico-physiological parameters of the organism in the process of adaptation to electromagnetic radiation was quantitatively assessed with the use of status-metry technique; in particular, a decrease in the functional activity of the right hemisphere brain structures, aggravating with the length of service, was observed.

[Optimization of the immunologic selection of donor-recipient pairs in transplantation of the kidney].

Three functional models relating the integral index of transplantation outcome to a combination of immunogenetic status determinants in the members of the donor-recipient pair were constructed using statusometric techniques. These models for the first time make it possible to perform a quantitative assessment of the contribution of each of the 24 A and B locus HLA antigens and four blood groups of the recipient, of the role of coincidence and discrepancy among the determinants making up the combination, and to automate selection of the most suitable recipient for every donor kidney from waiting lists.

[Demonstration of individual predisposition to myocardial infarct based on immunogenetic status].

Statusometry, a method of automated quantitative assessment of the state of composite multiparametrical objects, is proposed for the detection of genetic predisposition to myocardial infarction as an instrument of integral quantitative assessment of systemic immunogenetic status on the basis of HLA phenotype. The prognostic error does not exceed 15%. Large-scale application of the new method appears a promising approach to individualization and better efficiency of preventive cardiologic treatment.
