Monocyte-mediated immunosuppression in chronic multiple sclerosis: implications for therapy.

The goal of immunotherapy in multiple sclerosis (MS) is to halt disease progression by correcting an immunologic abnormality. The experiments described here sought an abnormality of immunoregulation. Specifically, the immunosuppressive activity of adherent monocytes in an in vitro assay of immunoglobulin-secreting cells was tested. Although the peak response was slightly lower, the induction, development and shutdown of the response by MS cells reproduced that of control cells. Most importantly, adherent monocytes from patients with MS exerted the same maximal suppression as did monocytes from controls. Further experiments are required to determine if the adherent monocyte is an appropriate target for immunotherapy in MS.

Evaluation and primary management of spasticity.

Spasticity is often a lifelong manifestation of central nervous system damage in adults with upper motor neuron lesions. Practitioners who initiate the management of spasticity when damage occurs are more likely to offer the patient a life without permanent joint deformity. Spasticity may be helpful or harmful to the patient, but in either event warrants a careful assessment and management plan based upon current health care concepts. Two general intervention principles and three specific intervention measures, including the use of medication, are discussed.

Impact of a nurse managed clinic in multiple sclerosis.

This article indicates the importance of evaluation in the practice of professional nursing. In the chronically ill neurological population both process (what the nurse actually does) and outcome measures need to be considered when formulating the objectives and scope of service to be provided by the clinical nurse specialist. For this population, morbidity was a natural evaluative measure for the nurse to employ as an index of intervention effectiveness. This does not mean to imply that nurses are the sole persons who can manage this type of outpatient clinic, but rather that nursing can play a pivotal part in the promotion of health for this particular group of neurologically disabled individuals. For this population, a consistent caregiver is important in implementing a rehabilitative approach to manage the interdisciplinary effort needed to help these individuals and their families maintain or improve health.

Clinical practice and the graduate nursing faculty.

When graduate faculty consider their responsibility to practice nursing, there needs to be an ensured framework within the school and university that will provide necessary support that includes opportunities and reward for demonstrated practice excellence. Setting realistic goals within an adequate time frame is essential. It does take a carefully formulated plan to balance practice, teaching, and scholarly work, and not make practice the sacrificial lamb when academic pressures increase. It is unfortunate that while in some nursing programs clinical excellence is expoused loudly or even raved about, its actual value in measures that make sense to faculty - promotion and tenure issues - may be limited. It is difficult to meet the triple commitment, carrying out each at the same time. Yet, the more individual faculty make effort in seeking to demonstrate models of practice, the more nursing excellence will be recognized. One way of improving the image of the nurse is for nursing's most prepared persons - faculty - to practice.

A great nursing challenge--recognition and effective management of the patient with herpes simplex encephalitis.

There are many difficulties associated with management of persons suspected or diagnosed with HSE. Excellent coordinated care is critical and the health care team must devote particular attention to these patients' complex clinical problems, including nutrition. While Vira-A therapy as the treatment for diagnosed HSE is significant, it may be associated with difficulties in fluid overload. Other problems are likely to evolve around a serious illness that progresses rapidly and is associated with fever, seizures, and increased intracranial pressure.
