Olfactory groove meningioma and multiple aneurysms. Case report.

We report a case of a patient with olfactory groove meningioma associated with multiple aneurysms. The association between intracranial aneurysms and meningiomas is infrequent and perhaps it is due to chance alone; many theories have been formulated to explain it. In the cases with both lesions, meningiomas are more frequently located at level of the convexity while aneurysms are more often located at level of the anterior cerebral-anterior communicating artery complex or at level of the middle cerebral artery.

Megadolichobasilar artery and acute cerebrovascular pathology.

Sixteen patients with a megadolichobasilar artery and acute cerebrovascular symptoms are reported. All were explored by computerized tomography and vertebral angiography. Nine had transient ischaemic attacks or definitive ischaemic lesions, whereas 7 had subarachnoid or intracerebral haemorrhage. Among the patients with ischaemic troubles, vertebro-basilar insufficiency and pontine infarction were the most common clinico-radiological findings. Among the patients with intracranial haemorrhage, 4 had associated aneurysms or arteriovenous malformations, while 3 had subarachnoid or intracerebral haemorrhage with no vascular malformations on the angiograms. The possible relationships between the megadolichobasilar anomaly and the cerebral ischaemic or haemorrhagic pathologies are discussed also from a review of the pertinent literature.

Giant intracranial varix associated with venous angioma and intracerebral hemorrhage.

A rare case of giant cerebral varix with venous angioma and intracerebral hemorrhage, surgically treated with good outcome, is reported. Varices and venous angiomas are infrequent in the clinical practice and are usually asymptomatic. Giant venous varices are less rarely associated with vein of Galen aneurysms or high flow arteriovenous shunts, whereas isolated giant varices are exceptional. Association of varix and venous angioma has been reported only in three instances. The radiological diagnosis of the cerebral venous malformation is briefly discussed.

Cerebellar metastasis from prostatic carcinoma simulating, on CT-scan, a cerebellopontine angle tumor. Case report.

The authors report a rare case of cerebellar metastasis from prostatic carcinoma simulating on computerized tomography a cerebellopontine angle tumor. Solitary intracerebral metastases from prostatic cancer are infrequent, and their location in the posterior fossa with extension in the cerebellopontine angle is quite unusual. The differential CT findings from more common benign cerebellopontine angle tumors are briefly outlined.

Fibromuscular dysplasia of the carotid arteries. Clinical and radiological considerations.

Seven patients with fibromuscular dysplasia of the carotid arteries are described. Three were asymptomatic and four had ischemic troubles. Angiographically, 3 had a fibromuscular dysplasia of type 1 and 4 of type 2; one patient also had an associated aneurysm of the cervical internal carotid artery. The neurological troubles and their pathogenesis, the angiographic findings and the differential diagnosis of this angiopathy are briefly discussed; the surgical procedures and their risks are also analyzed.

Megadolichobasilar anomaly. Clinical and diagnostic considerations on 30 cases.

30 patients with megadolichobasilar artery, explored by angiography, are reported; 23 among them were first studied by computerized tomography and 4 by magnetic resonance. From the clinical point of view, 9 had transient ischemic attacks or definitive ischemia, 7 had subarachnoid or intracerebral hemorrhage, and 7 had deficits of the cranial nerves; in the other 7 cases the condition was incidentally discovered as isolated asymptomatic anomaly or with other unrelated lesions. Among the patients with ischemic symptoms, vertebrobasilar insufficiency and pontine infarction were the most common clinico-radiological findings. Among the patients with intracranial hemorrhage, four had associated aneurysm or arteriovenous malformations. CT and MR allow a correct diagnosis of this anomaly. MR seems to be superior to CT in delineating the relationship of the anomalous artery with the nervous structures, and the coexisting pathology.

The role of real time B-mode ultrasonography in the follow-up of patients submitted to carotid endarterectomy.

Sixty-eight patients submitted to carotid endarterectomy for carotid stenosis have been studied periodically by B-mode real time ultrasonography. This non-invasive investigation allows to detect not only a recurrent stenosis, but also minor changes following endarterectomy, including suture, lesions from clamps, myointimal proliferation, and intimal flaps. We think that all patients submitted to carotid endarterectomy must be explored periodically by duplex scanning, in order to follow-up the normal and pathological changes of the arterial wall.

Epidermal growth factor receptor and tissue tumor markers in human intracranial neoplasms.

The content of the epidermal growth factor receptor (EGFr) and its relation to tissue CEA and NSE were examined in 22 intracranial neoplasms (18 primary intracranial tumors and 4 metastasis from lung cancer). The EGFr was measured by a radioligand assay on crude plasma membrane preparations. The tissue tumor markers (CEA and NSE) were measured on the high speed supernatant of tissue homogenates. In the primary intracranial tumors the EGFr was positive in 68%, CEA in 43% and NSE in 47% of all cases examined. As it concerns the metastasis from lung cancer the positivity was 75% for EGFr while for CEA and NSE was 50% and 32% respectively. These results suggest that the routine measurement of EGFr, associated with the determinations of tissue CEA and NSE could provide useful information on the type of the intracranial neoplasms.

Sturge-Weber disease without facial nevus.

A rare case of Sturge-Weber syndrome without facial nevus and epileptic seizures is reported. The other cases of incomplete form of the disease reported in the literature showed occipital calcification and epileptic seizures without facial nevus, while in the present case also the convulsions were absent. The possible pathogenic mechanism is discussed. The CT findings of these incomplete forms include unilateral or often bilateral occipital calcifications with no evidence of contrast enhancement. The Authors conclude that the radiologic finding of bilateral gyriform calcifications in the occipital region must suggest the diagnosis of Sturge-Weber disease even in the absence of facial nevus and epileptic seizures.

Cerebral hemorrhagic complication in polyarteritis nodosa. Case report and review of the literature.

Polyarteritis nodosa (PN) is frequently cause of both central and peripheral neurological disorders. However, involvement of the central nervous system is rare at the beginning of the disease. In this paper we report a case of a 38-year-old woman hospitalized in our Neurosurgical Service because of left hemiparesis in presence of cutaneous arteritis (PN). At computed tomography (CT) an intracerebral hemorrhage was found. No radiographic evidences of vasculitis of the visible arterial branches, at angiography, were seen. Cerebral arteritis should be suspected as a cause for intracranial hemorrhage in patients without hypertension or other risk factors.

Cerebellar medulloblastoma in adults: report of 15 cases and review of the literature.

From 1974 to 1988, 15 adult patients (aged over 16 years) with cerebellar medulloblastomas were observed in our Neurosurgical Department. All were treated by total (8 patients) or subtotal (7 patients) resection of the tumor, followed by radiation therapy to the posterior cranial fossa, spine and whole brain. A 5-year survival was achieved in 7 patients (46.6%). Local tumor recurrence occurred in 4 patients. The best treatment and the factors that influence the prognosis and survival of adult medulloblastomas are discussed also from the analysis of the pertinent literature.
