A defective purine nucleotide synthesis pathway in psoriatic patients.

Purine nucleotide concentrations in skin- and blood-cells of psoriatic patients are abnormal: The increase in the steady state level of cGMP and the decrease in the cAMP concentrations are associated with an enhanced rate of cellular proliferation. Concomitantly we found in the present study decreased ADP and ATP concentrations in blood cells (p less than 0.0001). The change in nucleotide concentrations suggests a defective purine nucleotide synthesis pathway. Stimulation of the Krebs cycle with fumaric acid raises ATP (p less than 0.0001) and most probably cAMP levels and at the same time slows down the purine nucleotide synthesis through end-product inhibition. Both effects can inhibit DNA and protein synthesis activity, which results in inhibition of cellular proliferation. Fumaric acid seems therefore a useful treatment for psoriatic lesions if liver and kidney functions (purine nucleotide and urea cycle) are controlled during treatment.

Increased plasma norepinephrine in psoriasis.

Free plasma catecholamines were measured by means of a standardized HPLC method in 50 adult patients with psoriasis and in 18 healthy volunteers. The concentrations of circulating norepinephrine were significantly higher in the psoriasis group (p less than 0.005); by contrast only slight differences were found in the epinephrine and dopamine concentrations. The possible mechanisms leading to these changes are discussed.

Carcinoembryonic antigen and bladder carcinoma.

The 24-hour urinary carcinoembryonic antigen determinations were performed on 61 patients with different stages of bladder carcinoma. Elevated titers were found in 81 per cent of the patients with active tumors and falsely positive studies were found in 7 per cent. High stage lesions were found to have high carcinoembryonic antigen levels. Plasma carcinoembryonic antigen determinations were elevated in only 45 per cent of the patients with active tumors but further study may be warranted in advanced bladder cancer cases. The 24-hour urinary carcinoembryonic antigen measurements yield the highest percentage elevations in bladder carcinoma and further investigation is required to better define its clinical application.

Anergy and tryptophan metabolism in bladder cancer.

The tryptophan loading test and cutaneous antigen test were done on 55 patients with various stages of bladder cancer. Patients with an abnormality of tryptophan metabolism showed a greater degree of unreactivity to cutaneous delayed hypersensitivity testing. Of the 23 patients with normal tryptophan metabolism 47 per cent were unreactive to 2 or 3 skin antigens. Of 13 patients with 1 abnormal tryptophan metabolite 62 per cent were unreactive to 2 or 3 skin antigens. Of 19 patients with 2 or more abnormal tryptophan metabolites 68 per cent were unreactive to 2 or 3 antigens.

A new method for determination of urinary tryptophan metabolites in bladder carcinoma.

Abnormal tryptophan metabolism in patients with bladder carcinoma has been reported to have an extremely high correlation with future tumor recurrences. The methods for determination of these urinary metabolites have not been applicable for routine clinical use in the past. A new method is described using thin layer chromatography followed by fluorescent scanning with the SD 3000 spectrodensitometer. The range of recovery for the 6 tryptophan metabolites was from 96.9 to 106.7 per cent. In our study 31 per cent of the male and 50 per cent of the female bladder cancer patients had 2 or more abnormal tryptophan metabolites.

Serological diagnosis in human Yersinia infections.

Serological examinations revealed Y. enterocolitica 0:3 specific antibodies in 34% of 80 children affected by a yersiniosis epidemic at titres of 1/40-1/640 and in all sporadic Yersinia enteritis cases. Similarly, agglutinins were identified for Y. enterocolitica 0:3 and 0:9 in 3.4% of 430 appendectomized children hospitalized with painful syndrome in the right iliac fossa. In 10% of 101 patients hospitalized with arthritis and Reiter's syndrome antibodies to Y. enterocolitica 0:3 (8 cases) and Y. pseudotuberculosis (3 cases) were detected at titres of 1/80-1/640. Our results stress the importance of serological examinations in diagnosing human infections with Yersinia.

[Physiognomic reconstruction of the dental arches in emergency treatment].

The gnathoprosthetic treatment in emergency is now a compulsory step in solving the esthetic problems raised by the affections of stomatognathic system (coronary lesions, edentation) localised in visible areas of dental arches. Nowadays, the state of stress induced by these affections can be relieved by an immediate and highly physiognomic treatment, a method at hand being the Scutan mask. Excellent technic results can be obtained by using conformators obtained by thermoplasticizing and vacuumating, in current use in our clinic.

Calcium blockers in ischemic stroke.

The study investigated the evolution of 72-nifedipine treated cases with ischemic stroke. Dipyridamole was administered to 72 controls. Subjects showed clinical improvement, thus calcium blockers can constitute a therapy alternative. A good influence of nifedipine was remarked in blood pressure.

Presence of isorenin in human pineal gland.

A renin-like enzyme was identified in adult human pineal gland preserved post mortem in acetone. The mean value of the isorenin content was equivalent to 470 +/- 75.4 ng. ang II/g Pt/h.

Experimental preparation of deantigenized vascular heterografts and study of tolerance after transplantation.

A new technique in preparing a heterograft is presented. Deantigenized calf carotid arteries are obtained by papain digestion and formol tanning. The biochemical determinations show a pronounced reduction in total protein content after papain digestion. The formol tanning gives a good rigidity and elasticity in adapting to different blood volumes without aneurysmal dilatation. Eight dogs were grafted : two died after surgery, two were killed after 35 days and two others 60 days after transplantation, one died after 8 months and one is alive 30 months after transplantation. The good in vivo tolerability has been appreciated. There have been no haemodynamic troubles and no cell changes of rejection. An insignificant quantity of immunoglobulins G has been found in the sera of the animals with heterografts. These data draw attention on the possibility of the use, in clinical practice, of the deantigenized calf arteries.
