Accessing health care in a rural area: an evaluation of a voluntary medical transport scheme in the English Midlands.

In recent years notions of self-help and voluntarism have emerged as key elements in the delivery of services in rural England. This paper explores these themes by reference to 'Rural Wheels', a voluntary medical transport scheme in rural Northamptonshire, introduced to overcome the closure of branch surgeries and to provide access to a new medical centre. By focusing upon the organisation and operations of the scheme, the paper highlights the important role it plays in the welfare of rural residents, particularly elderly women. Yet, because effectively it is run by a small core group, the paper raises questions not just about the viability of this scheme but also about the increasing commitment of central government to the voluntary sector as a means of delivering health care to rural people.

The counter-urbanization process: demographic restructuring and policy response in rural England.

The authors examine the impact of counterurbanization in England. They "try and unravel how household turnover and in-migration selectivity are effecting rural change, and how local communities and policy-makers are responding to that change, in a number of villages in East Northamptonshire, a rural district in the heart of 'shire' England." The focus is on the extent to which the incoming households differ from those they replace, thus changing the socioeconomic characteristics of the rural population. The impact of such migration on the housing market is noted.

A rare cause of cholecystitis in pregnancy.

A case of typhoid cholecystitis presenting in pregnancy with acute symptoms and signs is reported. The patient failed to respond adequately to conservative treatment, and surgical intervention was required. The outcome of the pregnancy was satisfactory. The management of such a case is discussed.

Do women with menorrhagia need iron?

Haematological indices of iron deficiency and serum ferritin concentrations were compared in 42 women complaining of menorrhagia and in 34 with normal menstrual loss. No significant differences in haemoglobin concentration, mean corpuscular volume, or mean corpuscular haemoglobin concentration were found between the two groups. Serum ferritin concentrations were significantly lower (p less than 0.001) in patients with menorrhagia. Though the iron stores in these women were significantly reduced, only a few were anaemic. Thus, women complaining of heavy menstrual loss do not require prophylactic iron supplements.

A new method for treating carcinomatous biliary obstruction with intracatheter radium.

We describe a new method for relieving biliary obstruction due to malignant solid tumors. The method consists of placement of radium needles in a Ring biliary drainage catheter for three days. After removing the radium needles, the catheter is left in place to allow for repair of irradiated tissues, then it is removed. The object is to leave the patient with a patent biliary tree without a biliary drainage prosthesis. The clinical course of six patients treated by this method is described, and the autopsy findings in three cases are outlined. Three patients died with a serum total bilirubin ranging from 1--4.5 mg/100 ml, two to three months after removing the drainage catheter. Two patients died before the bile drainage catheter could be removed. One patient is alive with a bile drainage catheter in place. Possible modifications in technique and catheters are being considered.

Radiological assessement of fetal growth retardation.

Ultrasonic screening to identify fetuses suffering from intrauterine placental insufficiency will diagnose this condition, but will also produce a large number of false positives. The fetal subcutaneous fat is visible on an antenatal radiograph and is severely reduced with fetal starvation. A method of quantifying the fetal fat is described and the results of this assessment are correlated with the birthweight and maturity at birth. The technique has proved useful in the assessment of suspected fetal starvation, where the diagnosis or its severity is in doubt on clinical assessment.

Use of electromagnetic flow probes to assess myocardial performance in man.

Aortic blood flow was measured in 14 patients by means of an electromagnetic flow probe placed around the ascending aorta during cardiac surgery. High fidelity left ventricular pressure was monitored directly, 8 different indices of left ventricular function were derived from these measurements, and compared. Peak aortic blood flow correlated with the conventional indices of left ventricular function derived from left ventricular pressure, but better correlations were obtained for the maximum rate of change of power with respect to time. The clinical advantage of the derivation of this index is small and for clinical purposes peak aortic blood flow and the maximum rate of change of flow provide indices of myocardial performance which are directly available from the instrument.

A monitor for the continuous assessment of left ventricular performance after open-heart operations.

A cardiac monitoring system has been developed that incorporated phasic aortic blood flow measurement by an extractable electromagnetic flow probe and high-fidelity arterial blood pressure recordings. Cardiac output, stroke volume, and peak aortic blood flow are measured continuously; using logarithmic amplifiers, the pressure-flow derivatives of left ventricular function, such as cardiac power and stroke work, are obtained beat by beat. This monitor is used to follow the postoperative course of patients undergoing cardiac operations and to evaluate the hemodynamic effect of therapeutic agents.
