Malignant acrospiroma.

A 76-year-old woman was referred after results of the biopsy of an ulcerated mass of the right long finger suggested poorly differentiated squamous cell carcinoma. Excision and skin grafting were done and a diagnosis of malignant acrospiroma was established. These tumors are aggressive and 5-year survival rate may be as low as 30%. A ray amputation was subsequently done and the specimen was without residual tumor. At 14 months follow-up, the patient remains free of disease.

Cardiovascular effects of topical 50% trichloroacetic acid and Baker's phenol solution.

Phenol and trichloracetic acid (TCA) are used for cosmetic face peeling. Deaths associated with phenol have been ascribed to the cardiotoxicity. Our purpose has been to study and compare the cardiovascular responses to these two agents in a rat model. Fifteen rats underwent abdominal epilation. Two to six days later a surface area comparable to the human face (16 cm2) was delineated on the abdomen. Baker's phenol or 50% TCA was applied to this area. Cardiac rate and rhythm and arterial pressure were monitored. Once stable, the same agent was applied to the entire abdomen for further study. TCA has been shown to be a safer agent than phenol in this rat model. Assuming that this model simulates the human response, monitoring would appear to be indicated during full-face Baker's phenol peeling.

A second look at chemabrasion.

Chemabrasion is reviewed as a single procedure in which a deep, full-face chemical peel is followed immediately by dermabrasion. In the past few years new information has encouraged some individualization of the chemical aspects of this procedure. A discussion of some factors to consider in selecting chemical agents and our recommendations based on over 5 years of experience are included. In addition, we present some useful refinements of the chemabrasion procedure.
