Use of electrocochleography to monitor antigenic challenge in Menière's disease.

Allergy has been reported as a cause of Menière's disease. King et al. have established the validity of the provocative food test (PFT) for the diagnosis of food allergy. When the PFT is used to test patients with Menière's disease, the test is considered positive if the patient develops aural fullness, hearing loss, increased tinnitus, or dizziness during challenge with the offending food and relief of these symptoms during neutralization. Ferraro et al. have shown that electrocochleography (ECoG) provides an objective indication of subjective symptoms in Menière's disease by demonstrating an increased SP/AP amplitude ratio when the symptoms of aural fullness and hearing loss are present. We present several patients with Menière's disease in whom measurement of the SP/AP amplitude ratio was compared with symptom production during antigenic challenge and neutralization.

Homografts in chronic ear surgery.

There has been a considerable expansion in the use of tympano-ossicular homografts in the past decade. The use of homograft tympanic membranes and homograft ossicles as well as combined tympano-ossicular homografts has been shown to be effective in reconstructive surgery for chronic otitis media. While homograft tympanic membranes are effective in the repair of perforations, temporalis fascia is at least as effective and more readily available. Therefore, the unique benefit of the tympano-malleal homograft (TMH) is that it is an effective way of replacing the malleus, the "building block" for reconstruction of the ossicular chain. The indications and techniques for the use of the TMH are presented and illustrated through representative cases. The inclusion of the TMH in the otologist's armamentarium offers another avenue for reconstruction of the chronically diseased ear.

Allergic secretory otitis media: an approach to management.

An allergic etiology should always be suspected in those children having recurrent secretory otitis media. These children often have a seasonal recurrence of middle ear effusion, a history of allergic disease in infancy or early childhood, and a family history of allergy. An approach to allergic management is presented. Inhalants are tested by the serial dilution titration technique and treated at optimum dose levels. Several food tests are used to detect offending foods. They are used in progressive fashion which utilized each technique to best advantage. Fifty allergic children with recurrent secretory otitis media, despite conventional surgical therapy, were treated for inhalant and food allergy for one year. There was a significant reduction in the recurrence of secretory otitis media and need for myringotomy.
