[Stress breaker framework. 2. clinical results].

All prosthetic proposals can be judged only after a lapse of time. After twelve years of clinical practice using the concept of the stress breaker framework the results have been established. The clinical results revealed concern prostheses made according to identical criteria as regards the design (stress breaker framework), kind of prosthetic teeth used, mounting of teeth, imprint technique. The clinical protocol was that used for the comparison and verification, in time, of determined criteria. The criteria used were: the presence of pillar teeth, the absence of re-lowering indication, maintenance of V-O-D. The method involved: 170 cases, 418 pillar teeth, clinical follow-up from 3 to 8 years. The results revealed: preservation of teeth 96% absence of lowering indication 95% maintenance of V-O-D. 95% The analysis of the results shows that the preservation of the tooth pillars and the limitation of resorption below the stress breakers seem to be positive elements which are obtained easily and constantly due to the use of the stress breaker framework used in a movable partial prosthesis.

[Stress breaking saddle framework or stress breaking framework. 1. Principles].

The treatment success of a removable partial denture is conditioned by the respect of the base structures, tooth abutments and hard and soft tissues. In order to achieve this goal, the prosthesis should be in accordance with the following fundamentals: tissue duality (STEIGER diagram); visco-elastic behaviour of the mucous membrane (TURK, KYDD, DALY, PICTON WILLS); JORES, LERICHE and POLICARD principles. The proposed prosthetic solution, the stress breaker framework allows the disconnexion between the saddle and the framework as well as between the different saddles. The originality of the concept is due to the particular design of the framework, which permits the disconnexion of the saddle. Due to its principle, the displacement of the saddle takes place only when mastication forces are applied on the prosthetic teeth. These movements stimulate the bony tissues. The clinical construction of a stress breaker framework can be adapted to all types of edentation, as well as to sub-total dentures. The clinical results shown that, after twelve years of experience, the stress breaker framework allows the preservation of the abutments as well as the conservation of osseo-mucous tissues (no need of rebase). Several experimental research investigations have been carried out. Work on trial banks have revealed that, compared with the semi-rigid framework, the stress breaker framework lays less stress on the abutment and provides a better base surface beneath the saddle. Other studies have dealt notably with the displacement of the saddle as well as with fatigue tests of the prosthetic system. These studies have led to the development of specific preformed systems.

Stelligraph, a software program for designing the removable partial denture offers many possibilities in computer assisted technology. The design can be realized on any particular case, based on three different concepts. This software program can also provide on individual, manual and personalized design this offening complete freedom in the conception. This software program is an expert system in removable partial denture by providing the general practitioner with a rational design as well as clinical guidance for oral rehabilitation.

[Stress-breaking framework in removable partial dentures].

The "TOURTET stress breaking framework" is an evolution in the prosthetic therapy for partial edentations. All clinical cases could be treated, however, it will be necessary to respect a precise protocol to disconnect the saddle from the rigid framework. The advantages are: the preservation of the abutment teeth and the maxillary bone without the need of rebning. The implicity of this technique is depending however on the amortic thinking.
