Psychodynamic aspects of psychopharmacology in functional somatic complaints.

Psychiatrists as well as general practitioners frequently prescribe psychotropic drugs (tranquilizers such as benzodiazepines and increasingly low-dose long-acting neuroleptics) to treat functional organic complaints. We are interested in finding out the psychodynamics in the doctor-patient relationship that determine whether the doctor puts more emphasis on eliminating symptoms by prescribing psychotropic drugs or on solving the underlying conflicts by psychotherapy. Furthermore we describe some interactions which are of importance for illustrating whether drugs are effective or lead to side effects.

[Encounter with Chinese psychiatry (I). Traditional medicine and psychiatry in China (author's transl)].

The opening of the People's Republic of China after the cultural revolution also offers an opportunity for German neurologists to become acquainted with Chinese public health organization and the system of psychiatric care in the People's Republic of China. Today, Chinese medicine and psychiatry are faced with the problem of combining traditional diagnostic and therapeutic methods, particularly acupuncture and herbal medicine, with western scientific and psychotherapeutic principles and methods. The example of a Worker's Sanatorium stands for the multiplicity of traditional Chinese methods of treatment and also for psychosomatic disorders and diseases.

[Encounter with Chinese psychiatry (II). Communal psychiatry in Shanghai (author's transl)].

The development of communal psychiatry in the City of Shanghai is far advanced. Two large psychiatric hospitals with a total capacity of 2000 beds, 10 regional and 10 district hospitals with 10 - 100 beds assume the hospital care of the local communities, i. e., for each 100,000 inhabitants 50 psychiatric hospital beds are available at present. The network of psychiatric and psychosocial services extends from special hospitals and psychiatric departments through cooperation with psychiatrically trained and supervised doctors of all hospitals and the barefoot doctors, through the health officers in the street communities, in the sanitary stations in the large factories up to the management of the street committees.

[Concerfing the E.E.G. changes after the Spiller-Frazer operation for the treatment of trigeminal neuralgia (author's transl)].

A follow-up of 28 patients with trigeminal neuralgia, with E.E.G. examinations was carried out at periods of 6 months to 4 years, after they had been operated on by the Spiller-Frazier method. Acturally in 14 out of the 28, discrete focal changes were observed but they were however, not so marked that one could speak of an actural focus. In comparison with the findings of other authors, the trivial nature of the changes is emphasized and it attributed to the pre-operative preparation, the position of the patients and to particular features of the operative technique.

[Brain infarctions during treatment with ovulation inhibitors (author's transl)].

392 cases of cerebrovascular disturbances in women while taking oral contraceptives are reported since 1962. There has been a remarkable change of the distribution of age among these women. Till 1970 were mostly women involved between the age of 30 and 34 years. Today this type of distribution is no more to be seen because of the growing number of reports about complications in other age-groups. Complications were till 1970 mostly reported in the period from ten days till six months after taking oral contraceptives. The publications of the last three years are showing that many women are involved, who had taken oral contraceptives more than two years. New publications and our own results are showing no changes of the clinical syndromes and of the prognosis. Our investigation shows no decrease in the number of young women suffering of thromboembolic cerebral disorders of unknown aetiology in the last years. In this group is the percentage of the women, which had taken ovulation inhibitors, no reduced. The conception, that there is a connection between oral contraceptives and cerebrovascular occlusions seems more possible as before. Especially attention in prescription of oral contraceptives should be given to the fact of a known predisposition. The rareness of these complications leads to the conclusion that one or more predisponating factors of a nature not yet known have to be existent.

[Concept determination and classification of the so-called epileptic psychoses].

Some concepts and classifications of the epileptic psychoses are discussed with special regard to their capacity of being integrated into a multiconditionel view. In epileptics, too, the term of "psychosis" should no longer be connected to the axiom of the somatic origin of the psychic syndrome. The term of "twilight state" should be reserved to those among the epileptic psychoses which go along with troubles of consciousness. The socalled episodic morbid moods of the epileptics are no more merely to be understood as psychoses of somatic origin. They may as well develop as a psychic reaction. If one conceptualizes the psychic troubles of the epileptics as "transient syndrome" (Durchgangssyndrom), as functional psychosis, or as psychosyndrome originating from local or diffuse brain damage, their classification must be limited to the undoubtedly somatogenic psychoses. The term of "epileptic psychosis", however, includes the somatogenic, endogenic, as well as the psychogenic psychoses of the patients with epilepsy. The question of the psychogenesis of schizophrenia-like, maniac or depressive psychoses in epileptics until now cannot be answered because the psychosocial patterns which might condition them have not jet been investigated upon. Anyhow, the exclusive psychogenesis of a schizophrenialike epileptic psychosis leaves to be proved.

[Psychoses in epilepsy--classification and EEG-studies (author's transl)].

It is necessary to classify the epileptic psychoses before starting a comparative study between psychopathological and electroencephalographic findings. The knowledge of the epileptic psychoses is at the moment too incomplete for a systematic order following aetiological or topic aspects. The traditionally system of epileptic psychoses distributes in chronic and episodic, phasic, especially circular, reversible and irreversible psychopathological manifestations. We differentiate psychotic states with or without disturbances of consciousness. Well-known in EEG studies of symptomatic and endogenic psychoses is, that in these psychoses the same clinical syndrom can be related to normal and to pathological electroencephalographic findings. This contradiction could often be explained when the activity of the psychoses was taken into consideration. The activity of the psychoses is to be determined by the duration, number, intensity and variability of the psychopathological symptoms. From his own investigations from 30 patients with schizophreniform epileptic psychoses, episode affective disorders and both depressive and manic psychoses, it could be shown, that normal EEGs are markedly infrequent. Most often they occur in subacute schizophreniform epileptic psychoses. The EEG of active and particularly of schizophreniform epileptic psychoses is most often characterized by simultaneous appearance of sharp waves, paroxysmal dysrhythmias and abnormal rhythm formations. Active schizophreniform psychoses correlate usually with abnormal rhythms. These abnormal rhythms ("parenrhythmien") diminish parallel with loss of activity of the psychotic process, are correspondingly seldom seen in subacute psychoses, and are not demonstrable in inactive psychoses. The experience of an alternating correspondence between epileptic psychoses and frequency of seizures could be confirmed.
