[Treatment of chronic recurrent vulvovaginal candidiasis with fluconazole (fungolon--Actavis)].

The object of the study was to evaluate the efficacy of peroral administration of Fluconazole (Fungolon caps. 50 mg - "Actavis") in dose 150 mg (3 caps. x 50 mg) once weekly for a period of 6 months. 50 female patients of reproductive age with chronic vulvovaginal candidiasis caused by C. albicans with three or more recurrences per year were enrolled in an open trial prospective study and 42 women were evaluated before and after treatment. Clinical improvement was observed in 81% of the patients after treatment Microbiological cure was observed in 86% of the patients (36/42). Positive cultures for C. albicans after treatment had 6 patients. Four of these 6 patients had duration of the chronic candidiasis for more than 3 years. Side effects during the treatment were not significant and might not be directly correlated with the administration of fluconazole. There was no cessation of therapy due to side effects. 53.3% of the patients accepted positively the long duration of therapy while 30% found the long duration of treatment a major inconvenience. Fluconazole is easily administrated well tolerated and is suitable for the long treatment of chronic vulvovaginal candididasis.

[Contraception in women with diabetes mellitus].

Contraception in women suffering of diabetes mellitus is an important question mainly due to it has been proven that pregnancy outcome both for the fetus and for the mother depends on glycemic control before conception right to delivery. That's made planning of future pregnancy mandatory for patients with diabetes mellitus. Patients are advised for contraception until optimization of metabolic control has been achieved or until complete and contemporary treatment of the diabetes complications has been fulfilled. Incorrect contraceptive method could extend metabolic disorders and to reinforce vascular complications of the diabetes. Choice of contraception depends on the aim: short lasting contraception aiming on future pregnancy planning or long lasting contraception aiming on family planning. In women with diabetes mellitus it is extremely important to take into consideration such factors as type of the diabetes, its lasting, degree of metabolic compensation, presence of diabetic complications, body-mass index of the patient, presence of risk factors for cardiovascular diseases and future pregnancy planning. In cases when pregnancy is planning it has been preferred local contraception, such as condoms, diaphragms. In cases of nullipara or in women with plenty of partners, condoms are method of choice. In women who gave birth intrauterine device is such a method. In women with diabetes mellitus type I (insulin dependent diabetes mellitus) it is possible to use hormonal contraception depending on patient's request or on medical indications only in cases when diabetes has less than 15 years duration and microangiopathic complications and other vascular risk factors lack. Combined hormonal contraceptive preparations have to contain less than 30 microg ethinylestradiol and gestagen of "third" or "fourth" generation. This contraception has to be prescribed together with insulin dosage correction and demands on strict metabolic control of the diabetes and body weight. When the combined preparations are contraindicated it could be suggested progestagenic oral hormonal contraception if gynecologic contraindications lack and if this not leads to menstrual disturbances. In women with diabetes mellitus type II (non insulin dependent diabetes mellitus) combined hormonal contraceptives has not to be used, because they could provoke clinical manifestation and deteriorate progress of the diabetes mellitus.

[Emergency contraception with levonorgestrel for teenagers--efficacy, tolerability, and level of information awareness].

OBJECTIVE: Assessment of efficacy and side effects of emergency contraception for teenagers with levonorgestrel (LNG) and the level of users' informedness about possibilities and practical application. METHODS: The subjects are healthy girls (n = 49) with regular menstrual cycles at the age between 15 and 19, having had one unprotected or faultily protected sexual intercourse. All of them have administered 0.75 mg LNG within the 72nd hour, repeated after 12 hours. The data have been processed by variational analysis. RESULTS: One pregnancy was registered of a girl with firstintake at the 67th hour - pregnancy rate - 2,0%. The most frequent side effect was nausea - 26,5%, folowed by breast tenderness - 22,4% and fatigue - 20,4%. An up to 7th day delay in menstrual cycle is non significantly more frequent - 14,3%, followed by a delay of more than 7 days breakthrough bleeding - 8,2%. No significant changes were established in the lenght of the menstrual cycle. Emergency contraception is sought for after unprotected sexual intercourse in 69,4%, and condom failure problems in 30,6%. Only 18,4% have sufficient information about the possibilities and practical use of emergency contraception. CONCLUSION: LNG provides effective, highly tolerable contraception with a small number of side effects. Need is felt for serious popularization of the application of emergency contraception with teenagers.

[Three years experience of using mycosyst].

The aim of this study was to estimate therapeutical potentialities of the Mycosyst (Fluconazole) preparation, of the Gedeon-Richter company (Budapest, Hungary), in women with data for mycotic colpitis. 157 patients for three years period (April 2001-2004) were included in this study. Two groups of patients were formed. In 104 patients mycotic colpitis was diagnosed during curettage for feticide or missed abortion. Another 53 women were treated for mycotic colpitis unrelated to pregnancy. Complains of the patients disappear by the 48-th hour and in almost all women there were not complaints by the 4-th day. The most quickly were influenced itching, discomfort, erythema and dyspareunia and slowly and hardly discharge had been influenced. Mycosyst is a broad spectrum antimycotic agent of bistriazol type. It has good absorption and tissue saturation. 1-2 hours after oral administration maximal plasma concentration is achieved. Plasma half life is about 30 hours. This gave us opportunity for single or two doses Mycosyst for vaginal candidosis treatment. Use of two doses of 150 mg. was enough for acute forms treatment in almost 95% of cases. Mycosyst is sure, easy for use broad spectrum antimycotic from the azole group. This preparation is comparatively cheep and in the average Bulgarian income possibilities.

[Utrogestan and high risk pregnancy].

Utrogestan is a modern progesterone, which shows maximal effectiveness with minimal side effects. It is a natural progesterone in micronized form, which makes it suitable for oral administration and vaginal application with same effectiveness. The aim of our retrospective study was to evaluate the therapeutical effects of Utrogestan in women with threatened abortion in the first trimester. Our experience dated from about one year and a half. Sixty eight women were treated for threatened abortion with a daily dose of 400 mg Utrogestan. The treatment continued at least 14 days/average 21 days/. Utrogestan was administered orally twice daily. The main indications were first or consecutive threatened spontaneous abortion in first trimester. For the period of time no side effects and subjective complaints were established except particular cases of slight headache and dizziness after morning application. Sixty one of the sixty eight women were dehospitalised with healthy pregnancy with no side effects. In our experience we preferred to use the drug in women with lutein insufficiency before pregnancy. We conclude that Utrogestan can be widely applied in Obstetrics with the proper indications, such as threatened abortion in the first trimester. The prophylactic treatment in women with slighter complaints like dull pain and weight Utrogestan is applied one tablet twice daily. In graver cases the starting dosage is two tablets two or three times daily.

[Some opportunities for the application of the drug mycosyst (preliminary communication)].

The colpitis is one of the most frequent gynaecological disease. The different methods of treatment of colpitis are a matter of discussion in many specialized studies. The aim of our retrospective study was to evaluate the therapeutic effects of Mycosyst (Fluconazol), produced by the pharmaceutical company Gedeon Richter (Budapest, Hungary), on women with acute fungal colpitis. The experience dated from about eight months (April--December 2002) with 27 patient with anamnestic, clinical, microscopic and microbiological data of fungal colpitis. Women were divided into two groups: 16 patients with diagnosed fungal colpitis at the time of interruption of pregnancy and 11 patients with this complaints not related to pregnancy. The therapeutic scheme is 150 mg Mycosyst as a single dose. The subjective complaints began to disappear 48 hours after the administration and were completely eliminated in almost all cases four days later. The medicine has a very fast effect on pruritus, erythema and urinary complaints, and a little bit slower influences the vaginal fluor. Some good pharmacodynamic and pharmacokinetic parameters of Mycosyst are: fast and complete absorption in gastrointestinal tract, long plasma life, elimination by the kidneys without being metabolized, doesn't affect the plasma concentration of steroids and testosterone, high and long terming concentration in each stratum of skin, nails, vaginal secretion and saliva. Our conclusion is that Mycosyst is reliable, easily applied broad-spectrum azol antifungal. The single dose of 150 mg is sufficient for treatment of acute fungal colpitis in almost 90%. The medicine is comparatively cheap and accessible.

[Comparative diagnostic characteristics of carbohydrate metabolism disorders in women with polycystic ovary syndrome (PCOS)].

The aim of the study was to compare the information provided by both fasting and stimulated during a standard oral glucose tolerance test (oGTT) levels of glucose and insulin as regards glucose tolerance and insulin resistance in women with PCOS. Few criteria are used in practice to verify insulin resistance in PCOS--elevated fasting insulin, decreased basal glucose/insulin ratio (< 6 for glucose in mg/dl or < 0.333 for glucose in mmol/l), insulin increase of more than 100 lul during an oGTT, and HOMA index > 2. The study comprised of 94 women (mean age 21.8 +/- 5 = 8 ys, mean BMI-30.38 +/- 7 = 7 kg/m2), divided in 4 groups according to BMI--group with BMI < 25, n = 21; group with BMI25-30, n = 27; group with BMI = 30-35, n = 23 and group with BMI > 35, n = 23. An oGTT with parallel determination of blood glucose and insulin was performed in all patients. The ratio basal glucose/insulin and HOMA index were calculated. Glucose levels on the 2 md hour during the oGTT pointed at IGT (impaired glucose tolerance) and/or diabetes in 8.5% of the investigated patients while the fasting levels indicated disturbances in only 2.2% of them. Fasting insulin levels were in the normal range in a considerable part of PCOSE women, mainly those with normal weight, and were not predictive for the response of insulin to glucose load. Relative percent of patients with elevated fasting insulin rose with the increasing of BMI. Decreased glucose/insulin ratio, elevated HOMA index, and abnormal stimulation of insulin were seen in more than 2/3 of the PCOS women including nonobese ones. Information provided by the above three markers was similar and they could be used alternatively according to the particular case and assessment of the cost/benefit ratio.

Efficacy and safety of phospholipid liposomes in the treatment of neuropsychological disorders associated with the menopause: a double-blind, randomised, placebo-controlled study.

A double-blind, randomised, placebo-controlled clinical trial was performed to evaluate the efficacy and safety of phospholipid liposomes (Liposom Forte) administered parenterally in the treatment of anxiety and depression linked to the menopause. A total of 64 females aged 40-60 years were randomised to receive the active drug or placebo intramuscularly; 58 patients completed the study. Treatment lasted 60 consecutive days. One i.m. administration of 2 ml active drug or placebo every other day was carried out. Efficacy was evaluated by the Hamilton Anxiety Scale (HAMA) and the Climacteric Index. An intention-to-treat analysis was performed, defined as all patients administered with at least one dose of the study medications with at least one return visit. A highly significant (p < 0.001) decrease in HAMA total score in both groups was noted. However, the decline in the HAMA score was significantly greater in patients administered phospholipid liposomes after 40 days (p = 0.006), 60 days of treatment (p < 0.001) and at the last follow-up visit (p < 0.001). Also, there were statistically significant differences between treatment groups after 60 days of therapy for individual items, such as anxious mood (p = 0.006), tension (p = 0.024) and fear (p = 0.009), with significantly less patients experiencing these symptoms in the phospholipid liposomes-treated group. When the Climacteric Index was evaluated, a highly significant (p < 0.001) decrease in the total score in both groups was noted. However, the decline was significantly greater in patients administered phospholipid liposomes after 40 days of treatment (p = 0.017), 60 days of treatment (p = 0.0013) and at the last follow-up visit (p = 0.0012). Significant differences between treatment groups were recorded after 60 days in asthenia (p = 0.05), dizziness (p = 0.024) and restlessness (p = 0.019) in favour of the active treatment. Twelve patients reported at least one adverse event, nine in the phospholipid liposomes group and three in the placebo group (p = 0.062). The most commonly reported event was drowsiness with two reports in each group. These findings further demonstrate that phospholipid liposomes administered intramuscularly are active against mild anxiety and depressive symptoms in menopausal women.

[Complex effects of cavinton on climacteric symptoms].

The use of nonhormonal drugs in order to ameliorate climacteric symptoms makes it possible to treat also those women in whom there are some contraindications or lack of compliance for taking hormonal replacement therapy. The drugs with complex effects on the whole body functions are more widely used recently. The aim of the present study is to assess the therapeutic effect of Cavinton (vinpocetin) on the degree of climacteric symptoms and to verify its complex beneficial influence. The study comprises of three groups of women in early menopause--control group (n = 30), treatment groups with normolipidaemia (n = 32) and with hyperlipidaemia (n = 29). All women presented with moderately expressed climacteric symptoms as assessed by Kupperman menopausal index and Hamilton-Anxiety-Skala (HAMA). The women in the 2nd and 3rd groups have been taking Cavinton in an oral dose of 5 mg three times daily for 3 months. The following parameters of lipid metabolism were determined in the beginning and at the end of the study: total, HDL- and LDL-cholesterol, triglycerides, and two indexes of lipid atherogenic risk--total/HDL-cholesterol ratio and atherogenic index (AI) = total--HDL/LDL-cholesterol. The menopausal complaints were assessed by Kupperman index and HAMA. Blood vessels reactivity was determined by pulsation index (PI). Statistically significant decrease in total cholesterol and LDL-cholesterol levels as well as amelioration of atherogenic indexes was observed in the 3rd group. Kupperman index and HAMA decreased significantly on the 45th day and the 3rd month in the women under treatment. No significant changes in PI were observed but a tendency towards a decrease was seen in the 3rd group. Our data suggest that Cavinton possesses complex beneficial effects in climacteric women significantly ameliorating climacteric symptoms as well as some parameters of lipid metabolism in women with hyperlipidaemia.

[Possibilities for an individualized approach to the treatment of climacteric symptoms with phytoestrogens].

UNLABELLED: In climacteric women with contraindications and fear or lack of compliance for taking hormonal replacement therapy as well as in those cases where risks exceed benefits, the use of phytoestrogens is appropriate having in mind their especially good effect on neurovegetative symptoms. AIM: The present randomized prospective study is aimed at estimating the possibility for ameliorating climacteric symptoms with Melbrosia and at assessment of its effect on different symptoms in order to individualize the therapeutical approach. PATIENTS AND METHODS: Two groups of women with climacteric complaints were followed-up--control group (n = 32) and treatment group (n = 34). Climateric symptoms were assessed in the beginning and at the end of the study using Kupperman menopausal index. The levels of FSH, LH, oestradiol as well as some parameters of lipid metabolism were determined. RESULTS: A significant decrease of Kupperman index was observed in Melbrosia treatment group in contrast to the control group. No significant changes in gonadotropins, oestradiol and lipid parameters were found in both groups. CONCLUSION: The preparation Melbrosia, consisting of phytoestrogens and amino acids exerts a good therapeutic effect on climacteric symptoms mainly on the nervousness, anxiety, irritability, headache, and hot flashes.

[Spontaneous abortions in Sneddon's syndrome].

Following the publications on the connection between Sneddon's syndrome and the antiphospholipid++ syndrome, the attention of researchers concentrated on the reproductive problems of women comprising the greater part of patients suffering from Sneddon's syndrome. One of the main indications of these almost coinciding syndromes are miscarriages for no obvious reason. It is supposed that the general mechanism is thrombosis. In a group of 38 patients with Sneddon's syndrome 32 (84%) are women. Twenty-eight of them have altogether given bright 43 times. Seven of them have suffered miscarriages for no obvious reason and three have habitual miscarriages. Three women have sterility problems.

[Tri-Regol and its application in gynecological practice].

One of the most common side effects of the low-dosed estrogen monophasic preparations with selective progestin are breakthrough bleedings and spotting. The aim of the study is to evaluate the contraceptive and therapeutic abilities of the preparation Tri-Regol produced by Gedeon Richter Ltd., Budapest as a suitable alternative to the monophasic preparations. The study includes 29 women at the ages between 19 and 28, non-smokers, who have not given birth. The duration of the observation and the monitoring is 4-6 months (149 cycles altogether). During the first month of administration of the drug, breakthrough bleeding was registered with 5 women (17.2%) and spotting with 2 women (6.8%). During the third month only one woman reported scarce breakthrough bleeding (3.4%). With all the 149 cycles not a single pregnancy was registered. Pearl index--0. With all the patients ovulation was restored within three months after discontinuing the administration of the preparation. The side effects were rare in appearances and tended to decrease and finally disappeared with prolonging the period of administration. The established very good control over the menstrual cycle, the high rate of contraceptive security, the quick restoration of the ovulation accompanied only by rare, mild and discontinuing side effects give us sufficient grounds to recommend the application of the preparation Tri-Regol, product of Gedeon Richter Ltd., Budapest to young women who have not given birth and to women with strongly demonstrated or persisting breakthrough bleedings when treated with low-dosed monophasic preparations.
