Granular cell tumor in differential diagnosis of tumors of the breast. The role of fine needle aspiration cytology.

The granular cell tumor (GCT) represents a rare but important lesion in the differential diagnosis of breast tumors. Since this generally benign tumor may be misdiagnosed as malignant in clinical investigation, mammography, sonography, and even in frozen sections, a preoperative diagnosis is of utmost importance. Two cases illustrate that fine needle aspiration cytology (FNAC) may be the method of choice in achieving a correct preoperative diagnosis. The histogenesis of GCT is examined by means of immunohistochemical stainings. The results confirm that this tumor arises from the peripheral nerve tissue. Most likely the Schwann cells and not the nerve cells constitute the origin of GCT.

[Cyto-diagnosis of cystic ovarian tumours].

The value of aspiration cytology of cystic ovarian tumours was studied, as an aid to the differential diagnosis between functional ovarian cysts and cystic ovarian tumours. In 158 women age 14 age 77 the cystic ovarian tumour was aspirated by laparoscopy or at laparotomy and the contents of the cyst were centrifuged and the sediment investigated by papanicolaou cytology. The findings of the smears showed direct or indirect cytological criteria for a cystic ovarian tumour. In 47 cystic neoplasms verified by microscopic examination the cytological diagnosis showed conclusive evidence of a neoplasm in 27 cases. In 7 cases the cytology was suspicious of a neoplasm. In 36 cases the differential diagnosis between cystic tumour and functional cyst was supplemented by radio-immuno-assay for estradiol in the aspirated contents of the cysts. Values over 500 picomol/litre are suggestive of a follicle are corpus luteum cyst.

[Acquired black disc and melanotic retina after resection of an orbital meningioma (author's transl)].

Resection of an orbital meningioma in a 53-year-old patient caused an occlusion of the central retinal artery and total ophthalmoplegia. Eight weeks postoperatively rapid development of a black disc was observed with migration of pigment into the juxtapapillary retina. One year later the eye was removed during a radical reoperation. Histological and ultrastructural examination showed that the melanosis of the disc and the retina was apparently due to reactive proliferation of the juxtapapillary pigment epithelium which had migrated into the atrophic tissues of the retina and the disc. The histological differentiation from hamartomatous proliferations of the juxtapapillary pigment epithelium is discussed. The proliferated pigment epithelium cells in this patient resemble those described in epiretinal membranes apart from abundant RER and actin filaments. In the absence of mechanical or inflammatory factors it must be assumed that hypoxia was the stimulus responsible for the proliferation. The presence of fenestrated retinal capillaries in the proliferation zone is described and the significance of this finding is discussed.

[Needle biopsies of sarcomas of the breast: cytomorphology and DNA cytophotometry (author's transl)].

When needle biopsies are done on all palpable changes in the breasts, some sarcomas of the breast can be found. The cytomorphology of this mesenchymal tumor is described and the differential cytology is discussed. In some of our cases the relative content of DNS of the cell nuclei in the smear was determined by quantitative cytophotometry in a Feulgen Stain. It was found that the DNS values of lymphosarcoma and spindle cell stromal sarcoma were within the norm of class 2C to 4C. The DNS content of angiosarcoma and polymorphous stromal sarcomas showed wide variations up to class 8 C. The result is discussed in view of automated cytological diagnosis.

[The importance of fine needle biopsy in modern breast diagnosis (author's transl)].

860 mammary tumours were clinically, mammographically and cytologically examined. Only thereafter, the histological diagnosis was established following surgical biopsy. There were found 356 malignant and 504 benign processes. The limited reliability of the individual methods is pointed to. As is proved, the diagnostics can be improved by combining the application of all three methods. On the one hand, a larger number of malignoma or malignoma-suspect processes are discovered, on the other hand, the preoperative diagnostics becomes more reliable. If the 3 methods yield similar findings, the diagnostic probability of error is below 1 per cent. Before, the range of indication was clearly outlined, the technique of punction, obtaining and processing of material was discussed, and the complications and limits of the methods were pointed to.

[Results of needle aspiration cytology in proliferative fibrocystic disease of the breast (author's transl)].

The accuracy of needle aspiration cytology in fibrocystic disease of the breast was evaluated in 101 cases with microscopically proved proliferative fibrocystic disease. The morphology of the obtained cytosmears was described. In about 50% of the cases a cytological diagnosis of proliferating fibrocystic disease is possible with accuracy. The needle aspiration is therefore a new method for intensive follow-up of patients with a high risk of develop carcinoma of the breast. The accuracy is increased in cases with well developed proliferation of the glandular cells. In cases with intraductal and intracystic proliferations, the results were better than in cases with adenosis.

[Investigations on the accumulation of radio-bleomycin in gynaecological tumors (author's transl)].

The indication for treatment with Bleomycin in gynaecological tumors is as yet not well defined. At present Bleomycin is primarily used for advanced gynaecological malignancies. Results of our investigations show that the individual affinity of gynaecological tumors to Bleomycin is very variable. In subsequent investigations it should therefore be determined whether the accumulation of radio-Bleomycin and the sensitivity of a gynaecological malignant tumor to Bleomycin therapy show a positive correlation. The probable chances of success of a course of chemotherapy with Bleomycin could then be predicted.

Characterization and comparison of receptors for 17 beta-estradiol and progesterone in human proliferative endometrium and endometrial carcinoma.

Sedimentation coefficients of cytoplasmic estradiol and progesterone receptors of human proliferative endometrium and endometrial carcinoma were determined by sucrose gradient centrifugation. In the absence of KCl, receptors from proliferative endometrium sedimented as single bands in the 8 S region and in the presence of 0.3M KCl in the 4 S region of the gradients. Receptors from endometrial carcinoma sedimented in several bands (between 3 and 9 S). When chromatographed on agarose gel comumns, the receptors (from both normal and neoplastic tissue) showed different molecular weights in the presence and absence of KCl (approximately 40,000 and 120,000, respectively). Elution profiles from agarose gel and ion exchange columns, as well as electrophoretic patterns from isoelectric focusing, revealed a similarity between biochemical properties of the receptors from endometrial carcinoma and proliferative endometrium. While the concentration of binding sites for estradiol and progesterone in normal endometrium depended on the day of the cycle, in endometrial carcinoma it depended on the degree of differentiation of the tumor. The binding of estradiol was highest at the beginning of the proliferative phase and declined continuously towards the 14th day of the cycle. In contrast, the concentration of progesterone binding sites was relatively low throughout the proliferative phase. In endometrial carcinoma low binding of estradiol was obtained in well differentiated tumors and high binding (as high as in proliferative endometrium) in undifferentiated tumors. For progesterone the contrary was the case. There was no difference in pH sensitivity between cytoplasmic receptors from normal and neoplastic tissue, optimal binding occurring at pH 7. Dissociation constants (Kd) for estradiol and progesterone depended on the degree of tumor differentiation. Kd values increased for E2 and decreased for P with increasing differentiation of the tumor. Competition studies with various unlabeled steroids revealed no significant difference between the specificity of the receptors from proliverative and neoplastic endometrium.
