Agreement: the origin of ethical action.

The health care setting is an interweaving of human concerns. What is needed in this arena is an ethic--a guidance system centered on, and relevant to, these concerns. Such an ethic must be derived from, and appropriate to, the health care setting and the practice of health care professionals. This is established by the health care professional/patient agreement. This article will describe how a practice-based, symphonological ethic--an ethic based on agreement--can be brought to bear on the adversities faced by nurses and any other health care professional in any setting.

Test of an educational brochure on advance directives designed for well-elderly people.

The aim of this study was to test an advance directive brochure designed especially for well-elderly people. The brochure was developed using information from literature on preparing educational materials and from information gained from well-elderly focus groups. The information sought was in regard to what they knew and what they wanted to know about advance directives and what they found appealing in brochures on advance directives. Twenty well-elderly people participated in the study. Prior to data collection, the brochure was given to 10 nurses who were asked to evaluate it according to the Bernier Instructional Design Scale 2. Out of a possible 280 points, the brochure received 244. The participants in the study were asked to read the brochure and answer 15 questions about the content, user friendliness, and attractiveness of the brochure. The overall results were positive. The first five questions had to do with the appearance of the brochure. All 20 participants responded favorably to these questions. The next 10 questions had to do with understanding the content. A simple percentage was used to calculate the results. The participants understood 90.5% of the content. Based on these results, the brochure should be used in future intervention research and in discussions regarding the education of well-elderly people and the drafting of advance directives.

The bioethical standards: the analysis of dilemmas through the analysis of persons.

The contemporary bioethical standards: autonomy, freedom, objectivity, privacy, beneficence, and fidelity are maximally appropriate to the health care setting. It is a common but erroneous belief that they are instruments for the analysis of dilemmas. They are not. They are instruments for the analysis of the character-structure of the person whose role in the dilemma is that of beneficiary. When the bioethical standards are used in this way they produce an understanding that tends to unite nurse and patient in harmonious interactions. These interactions produce results appropriate to ethical agents in a human context. These results are not accidental.

The ethical dimensions of delivering culturally congruent nursing and health care.

This article discusses the ethical parameters of giving culturally congruent care to individual patients by health care professionals. Leininger's Cultural Care Diversity and Universality theory (Leininger, 1995) is used to demonstrate the importance of culture in a person's life and Husted's and Husted's (1995) bioethical theory is used to create a mind-set of ethical interaction and to direct the analysis of a bioethical dilemma involving cultural differences between persons of the same culture, a depressed Mexican-American woman and her husband. The differences between transculturalism and multiculturalism are explored. We defend the position that a patient's culture is only a useful tool in caring for a patient if the individual person is made the primary focus of care.

Retention is the goal: extinguish burnout with self-esteem enhancement.

This article presents dialogues and selected strategies intended to enhance self-esteem and lessen burnout. While the article focuses on the dialogues and the strategies, it is necessary to begin with a brief discussion of self-esteem and burnout to make the connection between the two clear. The existing relationship between self-esteem and burnout contributes to the current nursing shortage. Among the reasons contributing to the nursing shortage is the exodus of nurses who have experienced or are experiencing burnout. This article guides nurses through a series of dialogues and strategies that, when practiced, will enhance self-esteem and decrease burnout by increasing job satisfaction for nurses.
