Dementia's impact on pain sensation: a serious clinical dilemma for dental geriatric care givers.

Dementia's impact on pain sensation is not well understood. Very little research has been done in this area, our clinical knowledge is poor, and there is no published information available to practitioners. This article reviews the current literature and discusses the need for further research. The number of patients with dementia is growing. This patient group will require dental diagnosis and treatment, and cannot be ignored by dentists. A humane approach to treatment must be adopted.

Hypodontia, impacted permanent teeth, spinal defects, and cardiomegaly in a previously diagnosed case of the Yunis-Varon syndrome.

The Yunis-Varon syndrome is a rare hereditary disorder with ectodermal and skeletal anomalies that include agenesis or hypoplasia of the clavicles, craniofacial disproportion with severe micrognathia, digital anomalies, prenatal and postnatal growth deficiency, and neonatal death. Only 12 cases have been reported in the literature. Although neonatal death is a significant feature of this syndrome, two case reports describe children, both males, who were 30 months and 3 years of age at the time of investigation. The 30-month-old child was reexamined at 11 years of age, and had further clinical and radiologic features that included hypodontia, impacted permanent teeth, spinal defects, cardiomegaly, bilateral hearing loss, and metatarsus adductus. Children who survive the neonatal period and continue to thrive with many of the features of the Yunis-Varon syndrome as well as the new features described in this article may not have a distinct yet related syndrome.

The dentist as a member of the palliative care team.

Palliative care is the knowledgeable and compassionate care of the dying patient. Although based on an holistic multidisciplinary philosophy, it does not usually include dentistry in its team approach. Many terminally ill patients have oral problems resulting from anti-cancer therapy and/or poor oral care during lengthy illnesses that require therapeutic intervention. By incorporating dentistry into the palliative care team, the dental needs of the dying patient will be managed more effectively.

Congenitally missing maxillary first permanent molars: a case report.

Congenitally absent maxillary first permanent molars is a rare clinical finding. It usually occurs with severe oligodontia in association with other abnormalities of the integumentary system. This article presents the case of bilateral absent maxillary permanent molars with severe oligodontia and no other abnormalities.

Psychiatric referral for craniomandibular pain patients.

The underlying process for the development and perpetuation of a chronic pain state is still unclear. This creates a perplexing problem for dentists dealing with chronic craniomandibular pain problems. This article outlines the diagnostic criteria used in our clinic to identify craniomandibular patients requiring psychiatric consultation and describes the management of the psychiatric referral.

Acupuncture and myofascial pain: treatment failure after administration of tricyclic antidepressants.

A 63-year old depressed woman with a 2-year history of chronic pain on the right side of her face in the area of the zygomatic arch and temporomandibular joint received acupuncture before and after the medical treatment of her depression. During her unmedicated state electroacupuncture provided pain relief, while after prolonged intake of doxepin the treatment no longer produced analgesia. The literature fails to provide a mechanism explaining the differential responses to acupuncture with regard to antidepressant medication. Suggestions on further endorphin related research are made with particular emphasis on gonadotropin activity.
