[Thrombocytopenia induced by low molecular weight heparin during hypotensive and diuretic treatment: coincidence or connection?].

A case is reported of a 70-year-old female who presented with early thrombocytopaenia, apparently induced by a low molecular weight heparin. The patient was admitted with anasarca. The initial treatment consisted in digoxin, furosemide, potassium canrenoate and aldactone, and 0.3 ml of Fraxiparine daily, which replaced acenocoumarol. She had never been given any heparin previously. On the fourth day, her platelet count had dropped from 210 G.1-1 to 122 G.1-1, and to 15 G.1-1 on the sixth day. Heparin administration had been stopped on the fourth day, and the platelet count had returned to 141 G.1-1 five days later. Similar cases of such an early thrombocytopaenia have been reported in patients also taking diuretics and hypotensive drugs. However, a relationship between the two facts cannot yet be ascertained.

[Course of metrorrhagia after biopsy curettage of the endometrium in women in reproductive age ].

Authors working in the department of gynaecology and obstetrics in the university hospital of Caen with non-pregnant, non-menopausal women who were followed-up for at least 2 years after curettage have been able to carry out a retrospective study of 102 curettage and biopsies carried out either for diagnosis or to lessen bleeding in cases of metrorrhagia, menorrhagia or menometrorrhagia. Histology of the endometrium showed 40 cases of hyperplasia, 17 cases of polyps in the cavity of the uterus, 19 cases of atrophy, 6 cases of endometritis, 3 cases of atypical hyperplasia and 17 cases of normal endometrium. Hysterectomy was carried out in 22 cases because of recurrence of metrorrhagia. Anatomopathological examination of the operation specimens had shown iatrogenic atrophy of the mucosa in 13 cases (59%). The authors believe that this hypoplasia can be responsible for some recurrences and suggest that the state of the endometrium should be reassessed when metrorrhagia reappears in spite of well controlled treatment with progestagens. In the case of atrophy a cycle of oestrogens and progestagens should be started as it should be immediately after the curettage. The authors hope in this way to lessen the number of hysterectomies that have to be carried out because of failure of medical treatment.

[T4 and T8 lymphocytic populations after total hip replacement].

Immunosuppression is involved in the occurrence of sepsis after surgical trauma. A postoperative lymphocytopenia is a recognised fact. In the opposite, studies on T-lymphocytes helpers (CD 4) and suppressors (CD 8) resulted in conflicting results. The aim of this study was to assess the variations in these two T-lymphocyte sub-populations using strongly standardized conditions in order to minimize the risk of non specific variations: same surgeon, same surgical technique, blood samples collected just before induction, immediately and 24 hours after surgery, automatized measures (Technicon H1). The results confirmed the lymphocytopenia, 24 hours after surgery, but no differences on CD 4 and CD 8 percentages were noted. It is concluded that during the first 24 postoperative hours surgery does not change the relative proportions of T-helpers and T-suppressors. Their measurement is not more useful than total lymphocyte count for assessment of postoperative immunosuppression.

[The induction of labour by acupunctural electrostimulation. A new technique and one that is thirty centuries old; a preliminary study (author's transl)].

Ten patients had their labours induced by acupunctural electrostimulation. This method was successful in starting labour each time. Eight women delivered normally vaginally. One had a caesarean and had uterine contractions without dilatation of the cervix. Electrostimulation has also allowed us to perform acceleration of labour.

[Should all prosthetic valve patients be treated by platelet inhibitors?].

The number, volume and adhesive and aggregation activities of platelets were studied in 31 patients with artificial valves. The differences observed in comparison with a control population are due to stimulation of thrombocytopoiesis secondary to the destruction or concumption of the platelets, it being impossible to be sure as to the dominant mechanism. Under these circumstances it would seem important in each patient to verify the therapeutic effectiveness of platelet inhibitors or of traditionnal anticoagulants by direct or indirect study of platelet life duration.

[Do enzyme determinations have any prognostic value in neurotraumatology?].

In 34 cases of skull traumas and in 14 cases of acute cerebral accidents, CK, GOT, GPT, LDH and its isoenzymes were assessed in the patient's serum and CSF. Results show that the increase of the enzymatic level in the CSF seems due to cerebral tissue damage for the main part, when in the serum the increase is influenced by combined traumas. The CSF dosages allow to define a threshold of severeness, unlike serum dosages without any pronostic value.
