Serial trough and peak amikacin levels in plasma as predictors of nephrotoxicity.

We studied 113 patients treated with intravenous amikacin to determine the value of determining serial trough and peak amikacin levels in plasma for predicting nephrotoxicity. Thirteen patients (11.5%) developed renal toxicity, with significant increases from 48 to 96 h in both peak and trough amikacin levels (6.7 +/- 4.7 [standard deviation] days before the serum creatinine rose). The nontoxicity group had no change or even showed decrements in amikacin levels in plasma. A higher nephrotoxicity risk was seen in patients with increments greater than 1 microgram/ml between 48 and 96 h, with odds ratios of 16.4 for trough, 8 for peak, and 7.2 for both levels. We suggest that an increment of at least 1 microgram/ml in amikacin levels in plasma from 48 to 96 h may predict the appearance of renal toxicity.

[Prevalence of insulin dependent diabetes mellitus in school children of the metropolitan area].

The prevalence of insulin dependent diabetes mellitus was determined in 1,078,109 students of 2,150 schools between 5 and 19 years of age. 147 diabetic females and 107 diabetic males were found, for a prevalence of 25/100,000 in females and 20/100,000 in males (p less than 0.01). Prevalence of diabetes among 3 age groups, 5-9, 10-14 and 15-19 years, was 17, 22 and 19 per 100,000 in males and 22, 32 and 28 per 100,000 in females, respectively. The higher prevalence in females was significant at all age levels. Overall prevalence according to age was 19, 27 and 24 per 100,000 in the corresponding age groups (significantly higher in the 10-14 year old age group, p less than 0.01).

[Oral lymphomas in AIDS].

We present two cases of AIDS in drug-addicts, with mouth lymphoma. The diagnosis was established by a gum biopsy which gave high-grade non-Hodgkin lymphoma lymphoblastic "B" type. It is very rare that the oral mucous is affected by lymphomas, but in AIDS and immunodeficiencies it is more common.

Periplasmic cytochrome C552 of Escherichia coli K12. Oligomeric structure and its role in nitrite reduction.

Perisplasmic cytochrome C552 was purified from a cold shock fluid. We demonstrated that this protein could reduce nitrite into ammonium. The reducing equivalents could be donated by formate or NADH through some segment of the membrane respiratory chain. FAD addition was necessary when pure cytochrome was used to reduce nitrite. The predominant form was found to have a molecular weight of about 65 to 70 K daltons, composed of three 21,000 d. subunits. Hemes may or may not be present on the subunit.

beta-thromboglobulin (beta-TG) and platelet factor 4 (PF4) in obstetrical cases.

The plasma levels of beta-Thromboglobulin (beta-TG) and Platelet Factor 4 (PF4) were measured in patients during the normal obstetrical period and in pre-eclampsia. A significant increase was observed in the two proteins with respect to the non-pregnant control group but no difference was seen between pregnancy and puerperium in either the normal or pre-eclamptic cases. The one significant variation in the concentration of the two proteins was a sharp rise during parturition. There was no correlation between beta-TG, PF4 and either the platelet count or creatinine but urea and uric acid did show a slight correlation with the two platelet proteins.

Histometry of the placental structures involved in the respiratory interchange.

Employing a stereometric technique, we determined the following parameters in six normal term placentae: respiratory area (total surface area of vasculo-syncytial membranes); total villous surface area; proportion of respiratory area (percentage fraction of villous surface that corresponds to the respiratory area). The value of each of these parameters was studied in each placental sample, as well as their distribution in the different placental zones, considered according to their distance from the placental rim. All parameters were distributed heterogeneously throughout the peripheral, intermediate and central zones of the placentae studied. The same parameters were also measured along the full thickness of the placenta, considering subchorial, middle and basal levels. All parameters were homogeneously distributed along the thickness of the placenta. We discuss the functional implications of our findings. Villous surface area values ranged from 8.6 to 13.3 m2 (mean = 10.9 m2). Respiratory areas ranged from 0.97 to 2.43 m2 (mean = 1.57 m2). Between 1:10 and 1:5 of the villous surface area corresponded to the respiratory area.

Effect of the combination of dexamethasone and sodium ipodate on serum thyroid hormones in Graves' disease.

To investigate the effect of the combination of dexamethasone (Dex) and sodium ipodate (SI) on hyperthyroidism, we studied 24 patients with typical Graves' disease, divided into four groups of six persons each. Three groups (Study I) were studied acutely (24 h) to determine the effects of Dex (5 mg every 12 h intramuscularly), SI (one oral dose of 3 g) and both drugs at the same doses, upon T4, T3, and rT3 at 0900 h before therapy was started and 24 h later. The group on Dex and that on SI had a similar T3 decrement of 25.9 +/- 4.0% and 35.8 +/- 5.0%, respectively, (P less than 0.05), whereas the effect of both drugs combined was greater (64.2 +/- 3.6%; P less than 0.01, Dex, and P less than 0.01, SI, respectively). The increment of rT3 was markedly greater in those patients on SI than in those on Dex (561.3 +/- 149.2% and 58.9 +/- 11%, respectively, P less than 0.025). A fourth group (Study II) was studied for seven days while receiving both Dex (1 mg orally three times per day) and SI (500 mg orally three times per day). Both T4 (from 18.8 +/- 1.1 to 13.1 +/- 1.1 micrograms/dl, P less than 0.02) and T3 (from 593 +/- 41 to 136.3 +/- 12.7 ng/dl, P less than 0.001) decreased at day 8. The initial brisk increment of rT3 at 24 h (808 +/- 149%, P less than 0.005) then diminished concomitantly with the fall of its precursor, T4.(ABSTRACT TRUNCATED AT 250 WORDS)
