Prolonged manometric study of the gastroduodenal junction in man.

A study of the motor activity of the gastroduodenal (GD) junction has been carried out on 8 subjects by using an original probe provided with a suction cup, that allows not only a duodenogastric (DG) pull-though, but also a prolonged recording of the pyloric, antral and duodenal motor activity, in basal condition and after intraduodenal instillation of HCl 0.1 N and intravenous infusion of cerulein. The position of the probe was controlled with fluoroscopy and transmural potential difference. The DG pull-through did not show a zone of high pressure at the GD junction, but the prolonged study showed, during HC1 and cerulein administrations, a significant increase of the pyloric tonus.

Exocrine pancreatic function after total gastrectomy.

The secretion of bicarbonate, lipase, and chymotrypsin into the duodenum in response to exogenous stimulation with secretin, 1 CU/kg-h, plus caerulein, 100 ng/kg-h, was investigated in 12 patients, on an average, 20.7 months after total gastrectomy and in 14 control subjects. The secretion of bicarbonate and lipase was significantly lower in patients than in controls. The reduction in outputs compared with the control values was 47.9%, 38.7%, and 24.2% respectively for bicarbonate, lipase, and chymotrypsin. Eight of the 12 patients (67%) had steatorrhoea. No significant correlation was found between this parameter and lipase output. It is concluded that the exocrine pancreatic function is impaired in the majority of patients subjected to total gastrectomy. The impairment, which particularly affects bicarbonate and lipase, is generally mild to moderate.

Endoscopic retrograde cholangiography (ERC) through artificial endoscopic choledocho-duodenal fistula.

A new endoscopic method for retrograde cholangiography in cases of unsuccessful cannulation of the papilla is described. In 1 patient with obstructive jaundice, severe diabetes and a previous history of biliary pain traditional ERC failed. An endoscopic artificial choledocho-duodenal fistula by means of a diathermic cutter (needle type) was performed at the lower end of the intramural portion of the common bile duct for retrograde cholangiography. X-ray showed an obstruction of the common bile duct due to a carcinoma of the head of the pancreas. Complications did not occur during the procedure and in the post-operative period. It is concluded that the described method may be helpful when traditional selective ERC fails, moreover it may permit an endoscopic choledocho-duodenostomy for choledocholithiasis in cases of unsuccessful endoscopic papillo-sphincterotomy.

A comparison between injection and infusion of pancreatic stimulants in the diagnosis of exocrine pancreatic insufficiency.

Exocrine pancreatic function was studied by rapid intravenous injection and by prolonged intravenous infusion of a combination of secretin and caerulein in a group of patients with proven pancreatic disease and in controls. The aim was to compare the value of the two procedures of stimulation in the diagnosis of exocrine pancreatic insufficiency. Results showed that the pancreatic response to infusion of the stimulants discriminates normal from abnormal pancreatic function better than responses to rapid injection.

[Histological and ultramicroscopic aspects of the small intestine in aregenerative enteropathies after treatment with 2-mercapto-propionyl-glycine (Thiola)].

The results of using alpha-mercapto-propionyl-glycine treatment (500 mg i.v. pro die for a time varying from one to four months) in a group of 25 patients suffering from aregenerative enteropathy with differing aetiopathogenesis are reported and compared with 29 control patients, i.e. patients suffering from the same situation and not treated with the drug. Morphological (by optical and electronic microscope) and functional (absorption of oleic acid, xylose and B12) study of the small intestine showed up the effectiveness of alpha-mercapto-propionyl-glycine on the trophism of small intestine mucosa, by way of normalization of intestinal cytokinetics, in aregenerative enteropathies due to special physiopathological situations (gastroresection) or to drugs (cytostatics, etc.) inhibiting enterocytic regeneration.

Effect of adrenocorticotropic hormone on pure exocrine pancreatic secretion in man.

The effect of adrenocorticotropic hormone (ACTH) on pure exocrine pancreatic secretion was studied in 7 subjects with external transduodenal drainage of the main pancreatic duct performed after biliary tract surgery. Intravenous injection of 0.25 mg of ACTH during a prolonged intravenous infusion of secretin (0.5 clinical units per kg-hr) plus cholecystokinin (0.5 Ivy dog units per kg-hr) significantly reduced protein and lipase (both concentration and output) without affecting volume and bicarbonate secretion. The reduction appeared soon after ACTH injection (peak inhibition in the first 15-min period) and lasted about 75 min. The adrenocortical response to ACTH reached its peak at the 60th min. The mechanism by which the pituitary hormone selectively inhibits pancreatic enzyme secretion without affecting volume and bicarbonate remains to be clarified.

Investigation of exocrine pancreatic function by continuous infusion of caerulein and secretin in normal subjects and in chronic pancreatitis.

Exocrine pancreatic function has been evaluated in 24 controls and 29 patients with confirmed chronic pancreatitis by continuous infusion (90 min) of synthetic caerulein, 100 ng/kg/h, plus GIH secretin, 1 CU/kg/h. Mean secretory values of the controls were comparable to those obtained by others using maximal doses of secretin and CCK. Unlike the controls, patients suffering from chronic pancreatitis demonstrated a progressive reduction in secretion during infusion. Thus, the comparison of responses for the final 30-min period gave much clearer and more complete discrimination between normal and abnormal pancreatic function than the preceding 30-min period.

[Therapeutic action of S-adenosylmethionine in some chronic hepatopathies].

Albuminopoyesis, prothrombin activity, BSF clearance, bioptic and sometimes also laparoscopic pictures have been examined in order to test the hepatic activity of SAMe. This study has been carried out in patients suffering from liver cirrhosis and other chronic hepatites. The above-mentioned parameters proved to be significantly improved in almost all the 25 patients studied and checked after 30 and 60 days' treatment with 30-45 mg SAMe administered by slow intravenous route.

[Therapy of cholesterol lithiasis with chenodeoxycholic acid (Chenoxyl)].

Chenodesoxycholic acid (500-750 mg/day Chenoxyl) was employed over a period of 9 months in the treatment of 40 patients with radiotransparent calculi in a functioning gallbladder and 8 with choledochal lithiasis. Radiological examination and complete hepatological exploration were carried out before and after the treatment in all cases. Complete dissolution was obtained in 6 and 2 cases respectively, with reduction of size in 13 and 2, and no change in 21 and 4. No significant variations were notes in the blood lipid picture or in liver function. There were no cases of diarrhoea.
