[A new anti-ischemic drug for the treatment of stable effort angina pectoris: nicorandil. Comparison with placebo and isosorbide-5-mononitrate].

The aim of this study was to investigate antianginal and antiischemic activity and tolerance of a new nitrate derivative, nicorandil (N). This research has been carried out in 18 patients, aged 47-70 years, suffering from stable effort angina with fixed ischemic threshold. The study started with 10 days of washout, during which the patients exercised twice on bicycle to verify the reproducibility of the test. Then, they took N or isosorbide-5-mononitrate (ISM) for 14 days according to a double blinded cross-over balanced study. Between the 2 periods patients took placebo (PL) for 14 days. In the first day and in the last day of each period, 2 hours after the last drug intake, patients performed a stress test on bicycle. Like ISM, N significantly increased, versus PL, 1 mm of ST depression time (ischemic threshold) in the first and in the last day, without differences between the 2 drugs and between the days. Moreover, ST depression was significantly lower at maximal common work (MCW) versus PL. The rate-pressure product was not different from PL after N and ISM at maximal common work, but is was increased at the ischemic threshold. In conclusion, like ISM, N has shown antiischemic activity in patients suffering from stable angina pectoris on effort with fixed ischemic threshold. After 14 days of treatment there was no evidence of tolerance. The activity of N seems essentially due to an increase of coronary blood flow to ischemic zones.

[Coronary artery dissection secondary to selective coronary angiography].

Coronary artery dissection during coronary arteriography is a rare complication. We describe three cases occurred in our laboratory (0.18% of all the procedures). All the invasive procedures were performed with a femoral approach with Judkins catheters by three different surgeons with limited experience; all dissections involved the right coronary artery. Two patients were men with coronary artery disease and the third was a woman who underwent catheterization for mitral valve disease. An inferior acute myocardial infarction occurred in both men and one of them was surgically treated. No ischemia was observed in the female patient with mitral disease. According to previous reports, we believe that the patients at risk for such a complication (adults with diffuse coronary artery disease or young women with valvular disease and normal coronary arteries) must be more carefully considered. Moreover, in these patients soft-tip Judkins catheters should be used during femoral procedures in the right coronary artery.

Lack of hemostatic effects of adult and suckling cat salivary gland heteropolysaccharides on blood coagulation.

Like other mammalian salivary glands, the cat submandibular and parotid glands belonging to adult and growing subjects contain heteropolysaccharides with very heterogeneous composition. Such glycoconjugates were this time tested in humans at doses used previously showing anticoagulant properties in rodents and lagomorphs. But surprisingly, no anticoagulant effects could be detected consistently. In fact, neither thromboelastography, nor hemocoagulation screening tests were affected in a statistically significant manner.

[Direct post-mastectomy reconstruction with skin expansion. Technical notes].

The demolition-reconstruction sequence in patients suffering from breast cancer may, in selected cases, be reduced to two surgical steps alone without compromising the radical nature of the surgery or the aesthetic result. The technical details of a protocol realisable in a short time that proposes on the one hand classical radical mastectomy (according to Patey or Madden) and on the other reconstruction by tissue expansion are described.

[Liposuction treatment for lipodystrophy of the pelvis and the legs].

In a series of 20 lipolysis procedures, there have been no serious major and few minor complications. Selection of patients is important and the best results can be acquired in the patients under 35 years of age. The postoperative circulation and diuresis control are very important particularly during the first 24 hours.

Heterogeneous activity of rodent and lagomorph parotid gland extracts on the hemocoagulation process.

Anticoagulant properties of parotid glands belonging to four species of mammals (rat, mouse, rabbit and hare) were investigated on the hemostatic system of human being. Heterogeneous effects of the glycoconjugates extractable from different species were demonstrated by means of thromboelastography and hemocoagulation screening tests. In fact, glycoconjugates isolated from the rodent (rat and mouse) parotid glands changed all the thromboelastographic parameters and the hemocoagulation tests (Thrombin Time, Prothrombin Time, Partial Thromboplastin Time). Glycoconjugates extracted from the rabbit parotid gland strongly affected the thromboelastogram parameters in addition to the Partial Thromboplastin Time. Finally, glycoderivatives obtained from the hare parotid gland only influenced the Partial Thromboplastin Time.

Effects of mammal submandibular gland glycoderivatives on human hemostatic system.

The aim of this study was to investigate the effects of glycoconjugate components extracted from the submandibular glands of four mammal species (rat, mouse, rabbit, hare) on human hemostatic system. The following analyses were performed: thromboelastography and hemocoagulation screening tests (Thrombin Time, Prothrombin Time, Partial Thromboplastin Time). Findings showed that all glycoconjugates induce modifications of fibrin clot formation time, modulus of elasticity and some hemocoagulation tests (TT, PTT). The anticoagulant effects were of inhibitory type.

[Level of protection against tetanus in an outpatient population].

Protection levels against tetanus toxin were estimated in 332 patients, at our Laboratory, by means of the passive haemagglutination with turkey erythrocyte test. The testing showed that the protection level decreases with age, it is higher among males in those over 30 yrs; roughly 10% of those who were not vaccinated were protected. Furthermore, it has been demonstrated that in some cases the protection level remains high even after a long period of time since the last vaccination, whereas a certain percentage of the recently vaccinated patients is not protected; consequently the effectiveness of the vaccination must be checked by measuring the antibody titers.

Addisons's disease in pregnancy.

The Authors show a rare case of Addison's disease in pregnancy, in a 27 year old patient, under cortisonic therapy. They propose the current diagnostic therapeutic orientations relating to maternal foetal risks.
