We present the results obtained on 110 menstrual cycles of 87 patients; echography study of ovulation, immunologic LH determination, basal body register card and serial cervical mucus score, since -6 to 0 day. Echography study determined follicular diameter, endometrial bulk and the presence of liquid in the cul-de-sac. We considered only those patients in whom the disappearance of the follicle was observed 24 hours before; being this fact, the indicator of ovulation, related to all other variables. These patients presented spontaneous (47.3%), epimestrol (28.2%) or clomiphene induced (24.5%) cycles, and they turned out to be pregnant or not in that cycle, (34.6%, 25.8% and 37.0% respectively). It was 36 (32.7%) pregnant women in all the The three follicular diameter curves for pregnant cycles, were similar being the preovulatory follicular range 21.5 mm for spontaneous cycles, 18.9 mm for epimestrol induced cycles and 20.7 mm for clomiphene induced cycles. Conceptional range was from 15 to 22 mms, 15 to 22 mm, and 14 to 27 mms, respectively. We didn't observe free fluid in cul-de-sac in 5 pregnant cycles (13.9%). Most of positive LH, are about -2 and -1 (21% and 50%, respectively). There were one pregnant patient and 2 no pregnant patient with repetitive, negative LH. In 50% of patients who became pregnant, the Nadir was on -1 day. In spontaneous conceptional group, bad cervical score was not observed. The good cervical score period is longer on conceptional group than in those with epimestrol. On conceptional group, dissociation between best score day and the day of ovulations was not observed, fact that we observed in non-conceptional group.
Suppression of the secretion of gonadal steroids by chronic administration of a superactive agonist of luteinizing hormone-releasing hormone (LH-RH) was used for treatment of leiomyomata uteri. Ten menstruating women, presenting with a total of 20 uterine leiomyomas, were treated for 3 months with daily subcutaneous injections of D-Trp6-LH-RH. Serum estradiol (E2) levels were suppressed rapidly in five patients and were decreased in other patients. At the end of therapy, leiomyomas regressed completely in three patients, while five patients showed a decrease of more than 40% in the volume of leiomyomas. The reduction in tumor size was correlated with the rapidity of the fall in serum E2 levels. In one patient, the leiomyomata increased in size during treatment, and one woman had a poor clinical response. The agonist was well tolerated and few side effects were observed. Therapy with LH-RH agonists offers an alternative in the management of some uterine leiomyomas.
The study of a stillborn infant and the placenta, showing extensive infiltration of fetal organs and chorionic villi by leukemic cells, is reported. The nature of the malignant cells was not apparent by light microscopic examination. Ultrastructural examination showed that these were immature myeloblasts, stressing the usefulness of electron microscopic study in establishing the diagnosis. The current case study seems to be the first in the literature in which both placenta and fetus were studied in extenso.
The retrograde percutaneous nephrostomy, a new technique for access to the urinary tract for stone removal, involves cystoscopic placement of a 9 French catheter into a selected minor calix, through which a sheathed 20- or 21-gauge needle is then advanced to exit at the skin. The tract is then dilated over this needle. This technique has been used successfully in 38 patients with only three complications. The technique is more reliable and less time-consuming than the standard antegrade percutaneous approach, particularly in the nondilated pelvicaliceal system. However, to avoid damage to the liver, spleen, or pleural cavity, care must be taken to ensure that the needle does not exit in a cranial direction.

We evaluated generic sodium stibogluconate (SSG) (International Dispensary Association, Amsterdam) versus Pentostam (sodium stibogluconate, GlaxoWellcome, London) under field conditions in Ethiopian patients with visceral leishmaniasis (VL; kala-azar). The 199 patients were randomly assigned to Pentostam (n = 104) or SSG (n = 95) in 1998/99; both drugs were given at 20 mg/kg intra-muscularly for 30 days. A clinical cure after 30-days treatment was achieved in 70.2% (Pentostam) and 81.1% (SSG). There were no significant differences between the 2 drugs for the following parameters: frequency of intercurrent events (vomiting, diarrhoea, bleeding or pneumonia) or main outcome (death during treatment and death after 6-month follow-up; relapse or post kala-azar dermal leishmaniasis at 6-months follow-up). Twenty-seven patients had confirmed co-infection with HIV. On admission, HIV co-infected VL patients were clinically indistinguishable from HIV-negative VL patients. The HIV co-infected VL patients had a higher mortality during treatment (33.3% vs 3.6%). At 6-month follow-up, HIV-positive patients had a higher relapse rate (16.7% vs 1.2%), a higher death rate during the follow-up period (14.3% vs 2.4%), and more frequent moderate or severe post kala-azar dermal leishmaniasis (27.3% vs 13.3%). Only 43.5% of the HIV-positive patients were considered cured at 6-months follow-up vs 92.1% of the HIV-negative patients. HIV-positive patients relapsing with VL could become a reservoir of antimonial-resistant Leishmania donovani.
Pancreatic metaplasia in Barrett's esophagus was originally described by Krishnamurthy et al. They found that these focal clusters of cells resemble pancreatic acinar cells by immunohistochemistry and electron microscopy. Wang et al one year later, described these same cell clusters in normal and inflamed gastroesophageal junction. We studied 318 cases diagnosed as Barrett's esophagus (199 cases) and chronic esophagitis (119 cases) in the ABC Medical Center seen in 1996 and the first four months of 1997, to look for pancreatic acinar metaplasia. We found 14 cases of Barrett's esophagus and 11 cases of chronic esophagitis with pancreatic acinar metaplasia. By immunohistochemistry and electron microscopy that these cell clusters are actually acinar pancreatic cells. Our results are in keeping with those found by Krishnamurthy and Wang that the clusters represent pancreatic acinar cells and may be found in Barrett's esophagus and in chronic esophagitis. The significance of these findings remain to be elucidated.
