A weekly schedule of docetaxel for metastatic hormone-refractory prostate cancer.

RATIONALE: Docetaxel has proven its efficacy in the management of hormone-refractory prostate cancer (HRPC). Schedules of docetaxel administration differ. This prospective phase II study was designed to reevaluate the activity and toxicity of docetaxel administered weekly at an optimal dose to a large cohort of HRPC patients. PATIENTS AND METHODS: Sixty-four patients were treated with docetaxel 40 mg/m(2) i.v., administered weekly for 6 consecutive weeks followed by a 2-week recovery period. Three treatment cycles were planned in the absence of progression or toxicity. The principal end point was the biochemical response based on the prostate-specific antigen (PSA) level (a decline of more than 50% for at least 4 weeks). Secondary end points were objective response to measurable disease, survival and toxicity. RESULTS: Toxicity was assessed in 64 patients. Toxicity was acceptable, with no toxicity-related deaths. Twenty-one percent of the patients developed grade 3-4 hematological toxicity. Sixty-four patients were evaluable for the PSA response. Forty-one patients (64%) achieved a decrease in PSA of >50%, 13 of whom had a PSA <4 ng/ml. Two out of 12 patients with measurable disease exhibited an objective response. With respect to PSA, the median progression-free survival was 29 weeks (95% confidence interval: 18-46 weeks). The global 1-year survival rate was 58%. CONCLUSION: Weekly docetaxel at a dosage of 40 mg/m(2) is a well-tolerated treatment, which has very promising activity on the reduction of PSA in metastatic HRPC. A large phase III study is underway.

[Host-tumor relationship as a function of the orosomucoid-prealbumin ratio].

Of the various prognostic factors for cancer, biological parameters of the Host/Tumour relationship allow objective evaluation of the conflict between the organism and the disease. Alpha 1-acid glycoprotein (Orosomucoïd), which belongs to the Acute Phase Reactant's Proteins, and prealbumin, a highly sensitive nutritional protein, are two compounds whose blood levels vary in inverse proportion during perceptible-stage cancers: alpha 1-acid glycoprotein concentrations increase while prealbumin values drop. The ratio of the blood levels of these two parameters provides a more sensitive index than either parameter taken alone. Study of the orosomucoïd/prealbumin ratio (ROP) in a population of 132 patients and 63 controls revealed a good association between the ratio values and the different stages of the disease. The highest values corresponded to patients with active disease; normal levels were obtained for patients in partial or complete remission. During the course of disease, the orosomucoïd/prealbumin ratio is more reliable than the sedimentation rate. Furthermore, when measured at the onset of the disease, this ratio is an undeniable prognostic element, independent of the stage of disease extension.

[Primary chemotherapy combining vincristine, bleomycin, methotrexate and cisplatin systemically or intra-arterially in cancers of the upper respiratory and digestive tracts].

85 patients with tumors of the upper respiratory/digestive tract were treated during an initial stage by polychemotherapy associating Vincristine, Bleomycin, Methotrexate and Cis-platinum. Administration was either by a systemic route (two cycles at a ten-day interval) or by a combination intra-arterial and intra-venous route (18-day protocol). Complete regression or regression greater than 50% of tumor volume was observed with the combination route in 76.5% of cases versus only 56.5% with use of the systemic route. In contrast, the percentage of lymph node regressions was less than 30%. No serious toxicity was observed and the classical treatment by radiotherapy for curative purposes (51 cases) or by surgery alone or associated with postoperative irradiation (30 cases) was then able to be completed without any complications linked to this initial treatment. The probability of survival, evaluated by the Log Rank Test, was significantly longer for patients who showed a positive response to initial chemotherapy.

Polyamines as biological markers in malignant lymphomas.

The urinary polyamines putrescine (PU) and spermidine (SPD) were measured for 67 patients with lymphomas: 48 non-Hodgkin (NHL) and 18 Hodgkin's disease (HD). Monthly repeat measurements were obtained over an average follow-up period of 12.5 months. For NHL, a good association was observed between polyamine levels and the severity of the the stage; the distribution of values between nodular and diffuse forms was also significantly different. No such correlations were seen for HD. The evolution of PU values in time has been shown to reflect disease activity for NHL. Non-specific fluctuating Pu values were found in the case of HD. In the case of 20 patients with NHL undergoing chemotherapy, comparison of polyamine levels before and after chemotherapy allowed differentiation between the population of complete responders to treatment and those with only partial or no response.

[Lymphangiosarcoma on a chronic edema of the lower limb. Radiologic study apropos of a case].

Lymphangiosarcoma is a rare tumor of the soft tissues, in particular outside of cases where the etiology is represented by chronic edema following mastectomy for breast cancer (Stewart-Treves syndrome). In the case reported, the patient experienced chronic edema of the lower limbs, with diffuse involvement of the right leg. The sonographic appearance revealing thickening of the dermis, with localized infiltration of the aponeurosis, and especially the angiographic findings showing hypervascularization with early venous return are discussed.

[Polyamines and malignant diseases (author's transl)].

The aliphatic polyamines (putrescine, spermidine, spermine) are associated with the phenomena of normal and neoplastic cellular growth. Two third of cancers generally show elevated urinary excretion of polyamines. The urinary levels of polyamines are significantly higher in cancer in evolution than in cancer in remission. The variable association of polyamines with different localisations is reviewed. The potential interest of polyamines analysis in the diagnosis and prognosis of cancer is discussed. The more exciting application of this test remains in the possibility of a short term appreciation of the oncostatic treatment efficiency.

[Exploration and surveillance of the retroperitoneal space by lymphography and ultrasonography in patients with non-Hodgkin's lymphomas (author's transl)].

The authors propose to conduct, in a systematic manner, lymphography and ultrasonography examinations before treatment of 75 cases of non-Hodgkin's lymphoma. The latter investigation enables examination of the upper part of the retroperitoneal space, and the detection of coeliomesenteric adenopathies, or possible lesions in the liver, spleen, and kidneys. The surveillance of most patients during treatment is possible with ultrasonography, if marked lipiodol resorption has occurred. Though this procedure is easy to conduct in all radiological departments, it does not have the same sensitivity as lymphography associated with computed tomography.

[Leiomyosarcoma of the renal capsule: ultrasonographic and angiographic appearances: a case report (author's transl)].

A 70-year-old patient presented with general ill-health, and a mass in the right hypochondrium which was found to be a leiomyosarcoma of the renal capsule. Ultrasonography demonstrated the retroperitoneal origin of the tumor and its lack of relation with the right kidney. Arteriography showed the right renal capsule origin of the mass, with partial invasion of the inferior vena cava. Operation confirmed the angiographic findings, particularly the independent nature of the mass i relation to the kidney and the excretory pathways.

Polyamines in lymphomas: their value in disease staging and monitoring of therapy efficiency.

The urinary polyamines putrescine and spermindine were evaluated in a population of 49 patients with malignant lymphomas (40 with non-Hodgkin's lymphoma and 9 with Hodgkin's disease). Analyses were performed on a regular basis during the course of treatment. Polyamine levels were found to increase significantly with the disease stage. Results show that: (a) polyamines closely reflect disease evolution (clinical deterioration/clinical improvement); (b) comparison of polyamine measurements effected 24 h prior to and 48 h after initiation of a chemotherapy course allows evaluation of the response to treatment.
