A new theory of micturition and urinary continence based on histomorphological studies. 1. The musculus detrusor vesicae: occlusive function or support of micturition?

Micturition and urinary continence theories have been under discussion since the last century. Up to now all these theories have been unsatisfactory. There is an obvious discrepance between the anatomical presentation and the clinical perceptions and physiological phenomena. This was the reason for a renewed and global examination of the whole distal urinary tract. The results are published in five successive papers. Our own examinations are based on 30,000 serial sections of the bladder neck taken from 65 male and female cadavers of all age groups. The muscle system of the urinary bladder consists of a network of smooth muscle cells forming three layers. Caudally the longitudinal muscle layers form two special recently described structures: the collare vesicae and nodus vesicae. None of the muscle systems of the urinary bladder leaves the spatial dimension of the organ. Simply two anatomical structures fix the urinary bladder in the pelvis. Dorsally it is the musculus vesicoprostaticus and the musculus vesicovaginalis, respectively; ventrally the existence of the musculi pubovesicales is introduced. There is no involvement of the lamellas of the bladder muscles in the formation of the urinary sphincter. Therefore the morphological substrate for a hitherto generally acknowledge contribution of the detrusor vesicae to the active continence function does not exist.

Changes in the distribution of proteinpolysaccharide complexes within the articular cartilage in experimental osteoarthrosis.

The quantity and type of proteinpolysaccharide complexes in the matrix determine up to a great extent the mechanical properties of articular cartilage. It is the purpose of this study to evaluate the changes in the mentioned matrix components against the background of experimentally induced osteoarthrosis. As shown by electron microscopic and morphometric studies, the changes in the superficial layer are promptly occurring and clearcut, whereas those in the deep layers are recorded in late observation terms only. A reduction of proteoglycan quantity is noted with a simultaneous differentiation of their fine structure in the various stages of osteoarthrosis development. Initially the alteration in the cell organization of chondroblasts is associated with occurrence of differences in proteoglycan content, and subsequently--in the collagen structures of the matrix too.

[Establishment of the necessity for a review of the vascular endothelium: an introduction].

The vascular endothelium despite of its relative small total mass is distributed all over the organism and is obligatory for nearly all vital processes. The following review will show, how many structural and functional properties of the endothelium are accessible to modern morphological research and will summarize results obtained by the authors of this review in context with the modern literature. The intentions of the authors are to contribute to a better understanding of morphological results and to find new access to scientific and clinical problems of endothelium.

[The aging alterations of the arytenoid cartilage].

The aging processes of the arytaenoid cartilage were described. We inspected 58 arytaenoid cartilage at the age of 0 to 91 years. The arytaenoid cartilage consists mainly of hyaline cartilage, whereas the apex, colliculus and the vocal processus consist of elastic cartilage. The cartilage of larynx exhibits changes concerning the cells and the intercellular substance. Chondrocytes show fatty degeneration, the cell density decreased. During the age intercellular substance shows the following changes: albuminoid degeneration, partly loss of intercellular substance with exhibition of well visible collagen fibers, calcification and ossification. Intercellular substance shows basophilic reaction up to the 4th decennium, later the reaction becomes more and more eosinophilic.

[Age dependence of functional systems, exemplified by the vocal cord and cricothyroid articulation].

The age dependency of functional systems is treated at the vocal cord and the cricothyroid articulation. At the time of birth there is a different developmental stage of the vocal cord. The anterior parts are essentially further differentiated than the posterior ones. The cause is to be found in the insertion of the vocal muscle at the anterior part. By intrauterine muscle contraction the polypeptide chains of the collagenous fibres are directed. The oblique cricothyroid ligament which limits the tension of the vocal cord in the cricothyroid articulation is already well developed and protects the vocal ligament from overstretching. The differences in the development balance in the first decennium. At the same time a sex-different structure of the vocal cord develops, whereby the vocal cord of the woman above all consists of elastic, the vocal cord of the man of collagenous fibres. At old age the two kinds of fibres degenerate. The cricothyroid ligament, however, does not show any involutional processes and as in early childhood fulfils its protective function.

[Measures against air pollution].

The Swiss Environmental Protection Law provides the legal basis for an efficient confinement of the air pollution. The concept of the Swiss air pollution control policy is based on two steps. In a first step, all emissions are limited providently. In a second step, the emissions have to be limited more severely, if it is certain or if it can be expected that the air pollution is harmful or annoying to human health or to the environment. The new ordinance on air pollution control is of great importance in abating air pollution. This ordinance will enter into force on March 1st, 1986. Is contains regulations for industrial sources and for heating systems, for instance emission limits for a number of pollutants, regulations for construction and operation of stationary installations, limits for the maximum content of pollutants in fuels. Moreover, stringent exhaust regulations for new passenger cars will enter into force on October 1st, 1987. Although important measures to reduce air pollution have already been taken in Switzerland, further efforts are necessary to guarantee a sufficient protection of human health and environment.

[Speed of and sex differences in the aging process in human tissue].

By means of the point counting method we determined the decrease of musculature and the increase of the connective tissue in the ageing ciliary muscle of the human eye and in the tunica muscularis of the small intestine as well as the increase of fatty tissue in human parathyroid glands during life. The paper describes the changes found by adaptation of exponential functions to the series of marks and reveals the implications resulting concerning the speed of the histological biomorphosis in the individual part of life as well as on their theoretical final stage. The speed of ageing permanently decreases from birth to death and aims at zero. There are only small differences between the observed parts of muscles and connective tissue, respectively, of very old people and the calculated values for "immensely" old people. Human parathyroid glands age markedly different to sex. The sexual divergence of the biomorphosis may be among others be morphometrically demonstrated on the basis of the increase of the interstitial fatty tissue.

[Morphological age changes of the heart and blood vessels--contribution to the asynchronous aging of human organs and tissues].

The weight of the heart increases in men and women very rapidly up to the 20th year and after this time grows slowly up to the 90th year. Only after the 90th year there is a decrease of the heart weight. Thickness and length of the heart muscle cells are changing analogous. The capillaries likewise multiply during the first and second decade very quickly and do not diminish in advanced age. We do not find a "withered condition of the top" of the capillary tree and a thickening of the basement membrane. Lipofuscin already appears in heart muscle cells of 7-year-old children; its amount raises continuously. The interstitial connective tissue increases only during the first decade of life. In advanced age a part of the collagenous fibres degenerates. The fibrous framework of the cardiac valves is developing to a solid and stable organ during the first and second decade. After the 50th year the collagenous and elastic fibres show symptoms of degeneration. The different ageing of the blood vessels is demonstrated by the arteries of the iris.

Morphometrical and mathematical analysis of the ageing changes of the muscle-connective-tissue-relation in smooth muscles.

The rate of the process of ageing studied in this work as well as of other ageing processes observed by us constantly decreases with increasing age. By an adaptation of exponential functions to series of measurements it is possible to estimate the theoretically possible end stage of histological ageing processes for extremely long lifetimes. The histological biomorphosis is much slower in female persons during their generative phase than in males. Results of planimetric measurements made on sections suggest that, so far as the shift in muscle-connective tissue relation in favour of the connective tissue is concerned, primary significance should be attached to muscular atrophy, while the connective-tissue proliferation may be considered to the secondary in this respect.
