Workshop on perinatal exposure to dioxin-like compounds. I. Summary.

An international workshop reviewed 20 ongoing or recently completed studies of the effects of perinatal exposures to dioxins, dibenzofurans, and PCBs on the reproductive, endocrine, neurodevelopmental, and immune systems. Many of the observed effects are consistent with these compounds acting as "environmental hormones" or endocrine disrupters. This report summarizes the conclusions and future directions described at the workshop.

Natural history of scaphoid non-union, with special reference to "asymptomatic" cases.

33 patients with non-union of the carpal scaphoid were diagnosed by X-ray examination two to 37 years following the original trauma. All of the patients could be contacted and summoned for a re-examination ten to 17 years later. X-rays revealed a 100% incidence of progressive radio-carpal osteoarthritis. It is concluded that freedom of pain is not a reliable prognostic indicator, and that all patients with non-union of the carpal scaphoid are likely to benefit from surgical treatment of the pseudarthrosis. The only exception to this rule might be the patient in whom the radio-carpal joint is already deteriorated by an advanced degenerative arthritis.

Segmental metacarpal replacement with bone cement and a silicone joint prosthesis: a case report.

The replacement of the distal half of the third metacarpal and the metacarpophalangeal joint with bone cement and a silicone implant is reported. Eight years after surgery, x-ray and clinical examination revealed no evidence of instability or material failure. The active and passive range of motion in the reconstructed metacarpophalangeal joint was 35 degrees, and the hand was free of pain.

Incidence of post-traumatic arthrosis after primary healing of scaphoid fractures: a clinical and radiological study.

In a retrospective study of 229 patients with healed fractures of the waist of the scaphoid, the incidence and development of post-traumatic radiocarpal arthrosis was studied. With a minimum follow-up period of seven years, 5.2% of patients showed radiological evidence of radiocarpal arthrosis. It is concluded that an alteration of the carpal dynamics, due to deformation and shortening of the scaphoid, is the most likely cause of post-traumatic arthrosis after primary healing of scaphoid fractures.

Bupivacaine: a safe local anesthetic for wrist blocks.

Seventy-one patients having minor hand surgical procedures under wrist block anesthesia were studied, with the goal of identifying a possible neurotoxic potential of bupivacaine when used according to standard clinical practice. This drug was compared with lidocaine at equipotent analgesic concentrations (bupivacaine: 5 mg/ml; lidocaine: 20 mg/ml) by use of a double-blind randomized protocol. At postoperative follow-up, four (5.6%) patients had new neurologic symptoms. Mechanical factors, e.g., nerve manipulation during surgery, or nerve trauma during performance of the block, were identified in three patients. In the remaining patient, anesthesia was induced with lidocaine, and no cause could be identified. It was concluded that bupivacaine, when used in clinical concentrations, is not associated with an increased incidence of neural complications.

Residues of polychlorinated-dibenzodioxins, -dibenzofurans and -biphenyls in human milk samples collected in a continental town in Croatia, Yugoslavia.

The levels of several polychlorinated-dibenzodioxins (PCDDs), polychlorinated-dibenzofurans (PCDFs) and total polychlorinated-biphenyls (PCBs) were determined in pooled human milk samples. The samples were collected in a continental town of Croatia, Yugoslavia, during the years 1981/82 and 1985. The concentrations of seven PCDDs and nine PCDFs were within the range less than 0.2-116 ng/kg milk fat; the total PCBs were 0.47 and 0.32 mg/kg milk fat. The levels determined were about the same as those reported for the general population in other countries.

Dental prophylaxis for youths in their late teens. II. Knowledge about dental health and diseases and the relation to dental health behavior.

A study of the knowledge about dental health and diseases of youths in their late teens has been conducted. A 10-class questionnaire relating to dental caries and gingivitis/periodontitis was answered by three test groups (A, B, C) totalling seventy-eight 19-year-olds who, during a 3-year period, had undergone professional prophylaxis at different intervals. Nineteen young people of the same age without organized preventive measures since their last attendance at the public dental service constituted a control group (D). The test groups were found to have a significantly better knowledge of factors relating to gingivitis/periodontitis than the control group. In questions relating to caries there were no differences among the groups. Higher points were scored in knowledge of therapy and prevention than of the causes and symptoms of the dental diseases. Several regularly repeated sessions for professional prophylaxis did not lead to better knowledge than only a few. No consistent correlation was found for any of the groups between knowledge and both quantitative tooth cleaning, i.e. type and frequency of use of oral hygiene aids, and qualitative tooth cleaning, expressed as the occurrence of plaque and gingivitis. Nor were significant differences found in these respects between individuals with and without a caries increment during the observation period regardless of grouping.

Abnormal haemoglobin in Finnsheep.

An abnormal type of haemoglobin in one ewe and one of her four lambs of Finnish breed has been reported. This type of haemoglobin in Finnsheep seems to be similar to HbC reported in some Norwegian breeds and in anaemic Merino sheep.

Multivariate data evaluation of PCB and dioxin profiles in the general population in Sweden and Spain.

Thirty-three polychlorinated biphenyls and 20 dioxin-like congeners were determined by GC-MS in blood and adipose tissue samples from two general population groups, one in Spain (n = 35) and one in Sweden (n = 28). The results were evaluated by multivariate projection (Simca), after normalisation to the most abundant and stable congener in each data set, in order to reveal differences in the "national" congener profiles. The mean values of each population group did not differ significantly. The sum of PCBs in the Spanish was 1450 ng/g and in the Swedish 1310 ng/g. The sum of the dioxin-like compounds in Spain was 1180 pg/g and in Sweden 804 pg/g. The congener profiles differed between the countries. Mainly the higher chlorinated congeners for both the dioxin-like compounds and the PCBs are more dominant in the Spanish group compared to the Swedish.
