Rapid cytological diagnosis during thyroid surgery.

The authors report the results of the use of intra-operative cytological examinations of the thyroid gland. In their hands, the method has a 96.3% accuracy in 757 patients with thyroid disorder. Benign conditions and malignant tumours can be correctly diagnosed. False negative diagnosis can arise with well-differentiated follicular carcinoma and the differentiation of follicular adenoma may be difficult. Atypical adenomas may give a false positive diagnosis. The incorrect diagnosis of chronic lymphocytic thyroiditis is unlikely when sufficient attention is given to the whole clinical picture.

[Management of lung and bone metastases from thyroid cancers of differentiated structures].

During a period of thirty years (1959-1989) 374 patients underwent surgery for differentiated thyroid carcinoma at the 1st Department of Surgery of the University Medical School in Debrecen. Distant metastases were found in the lungs or bones in 36 patients (9.6%). In 20 patients the first clinical sign of the primary disease was the distant metastasis. The aspects of surgical treatment depended on whether the metastases were solitary or multiple, capable or uncapable of radioiodine uptake. Whereas the majority of pulmonary metastases respond well to 131I-treatment the therapy of bone metastases is multidisciplinary. The 5-year survival rate was 47%, the 10-year one 19%.

Late prognosis of childhood and juvenile thyroid carcinomas.

The authors report on the biological properties and late prognosis of 16 children and juvenile patients operated on during the past 24 years for thyroid tumour who underwent regular followup. The clinical and morphological characteristics of the carcinomas are described, together with the late immune response of the patients undergoing complex treatment. The study is also concerned with the characteristics of the DNA content of the tumorous cell nuclei, the pregnancy success rate of the operated patients and the thyroid function of the children born.

[Clinical and morphological properties of cold nodules formed in the thyroid gland of patients treated with I-131 for hyperthyreosis].

The authors report on the clinical and morphological features of patients who had been previously treated for toxic adenoma with different doses of 131I isotope, and were then operated on for cold nodule. Progressive pseudoatypic and Hürthle-cell transformations, which are difficult to distinguish from different-type thyroid carcinomas, were revealed in one and a solid anaplastic carcinoma in another of the five patients during histological examination of operative specimens in serial sections. On the basis of these findings the authors support the function-protective operative treatment of single toxic adenomas, although the causal relationship between isotope treatment and consecutive carcinoma can not be verified unanimously.

Adenoma

[Simultaneous occurrence of hyperthyreosis and thyroid carcinoma].

The authors report on the morphological and clinical features of occult thyroid carcinomas confirmed by detailed histological examinations following the surgical treatment of hyperthyreotic goiters recurrent after prolonged thyreostatic treatment, as well as on the follow-up data of the patients. Joint occurrence of hyperthyroidism and thyroid tumors was observed in five patients, two of whom showed progressive endocrine ophthalmopathy. In the surgical preparations of three patients, chronic lymphocytic thyroiditis was also diagnosed. On the grounds of postoperative care, the authors draw conclusions of practical clinical significance. They take a stand on the questions of surgical and adjuvant treatment favored by them in the case of the joint occurrence of the two diseases.

Adenocarcinoma

A case of whipple's disease with repeated operations for ileus and complete cure.

The authors make a case report of a male patient, 58 of age, whose complaints started 37 years ago; inspite of an operation, repeated medical check-ups and treatment, they had no other choice but to perform a further laparotomy when the patient presented himself with progressive cachexia and signs of an acute abdomen. Taking the history, the operative and histological findings of the patient into consideration, the correct diagnosis could be made: Whipple's disease. The presence of this condition was also proven by postoperative X-rays and immunological examinations. The patient has been restored to health after a year's antibiotic treatment. An impaired defensive ability, mainly of the immune and macrophage system can be proven to exist in Whipple's disease. Two main tasks are essential: 1) to hold the patient under constant control and 2) to continue research with the aim of finding ways of immunocorrection.

[The unrecognized thyroid neoplasm].

During the last 10 years the authors operated upon 8 patients with illnesses beginning with different lung alterations. In all cases the basic illness was a papillary or papillo-follicular cancer of the thyroid gland which later was found indirectly after other operation and histologic examination. By evaluation of clinical symptoms, operative and morphologic findings conclusions as well as the results of reexamination the authors draw conclusions as to the biological attributes of this tumor type. They recognize that the carcinomas of the thyroid gland cause metastases of regional lymph nodes or distant lung metastases approaching the TO stage. Lung metastases appeared in different shapes and were in all cases the fist clinical sign of the basic illness. Considering the biological attributes of this tumor the authors emphasize that it is not inoperable or untreatable.
