[Immunologic disorders in pathogenesis of chronic generalized parodontitis].

Systemic immunity was studied in patients with chronic generalized parodontitis. This group of patients had distinct changes in immunologic system: depression of T- and stimulation of B-cellular immunity without accumulation of plasma cells and initiation of effective humoral response. Increased peripheral blood number of lymphocytes expressing induction of apoptosis CD95 receptors and ligand for this receptor CD95L (Fas-L) can lead to intensification of lymphocyte apoptosis and may be the reason for T-cell deficit development. The results of the study confirm the important role of immune system disturbances in pathogenesis of chronic generalized parodontitis.

[Oral cavity manifestations of gastroesophageal reflux disease].

The aim of the study was to investigate changes in the oral cavity and the gastroesophageal zone of the patients receiving rabeprazol, a proton pump inhibitor (PPI). The subjects were 88 patients with gastroesophageal reflux disease (GERD), who had changes in the oral cavity typical of this disease. The patients were divided into four groups. Group I included patients with non-erosive reflux disease; the other three groups were formed according to the degree of the severity of reflux esophagitis in accordance with Los Angeles classification of GERD: A, B, and C. The patients were administered rabeprazol as a PPI. The study found that oral cavity changes are associated with the degree of the gastroesophageal reflux. The study demonstrated high efficacy of rabeprazol in GERD patients with oral cavity changes.

[Mathematical simulation and estimation of stress deformed status of a maxillodental segment after tooth depulpation].

Mathematical simulation, estimation, and evaluation of stress-deformed status of bone tissue under the roots of functionally loaded mandibular abutment teeth after their depulpation were carried out. The resultant mathematical model, realized in the SPLEN-K system, is the key stage in the development and subsequent clinical application of automated diagnosis and consultation complex for patients with dentition defects.

[Study of the characteristics of biological membranes used in osteoplasty with the aim of purposeful tissue regeneration on a model of continuous bone marrow cultures].

The biological materials Parodoncol and Hapcol (Polystom Firm) used in osteoplasty were studied in continuous mouse bone marrow cultures. Culturing in continuous bone marrow cultures showed growth and development of stromal and hemopoietic cells on Hapcol and Parodoncol. The materials caused no negative reactions and failed to inhibit bone marrow cell growth. The number of cells gradually increased, hemopoiesis was normal, and young and mature granulocytes formed. This method of culturing proved to be highly effective for evaluation of the characteristics of biological membranes and other composite materials.

[Volatile compounds in air and oral saliva in healthy people, and in periodontitis and gingivitis patients].

21 healthy persons, 7 patients with chronic generalized periodontitis and 4 patients with chronic generalized gingivitis aged between 18 and 42 years were examined. Air from the oral cavity was collected with a special device; liquid samples were collected by washing the oral cavity with sterile water. Chemical compounds of the air and the washed liquid were analyzed by chromato-mass-spectrometry, gas-adsorption and gas-liquid chromatography. The content of dimethyl sulphide, dimethyl disulphide increased in the oral air and such volatile short chain fatty acids (VSCFA) as butyrate, propionate, acetate rose, but their aldehydes (butyraldehyde, acrolein, acetaldehyde decreased in oral fluid during periodontitis. The concentration of these compounds were found to be between the levels in control persons and during periodontitis. The content of pentane decreased in oral air. That depended on the intensity of inflammation and vessel disturbances. The results suggest that VSCFA produced by Porphyromonas, Prevotella, Fusobacterium and other periodontopathic bacteria, penetrate the oral mucosa and severely harm the periodontal tissue. The increase of dimethyl sulphide contributed to oral malodor during periodontal inflammation.

[An enamel adhesion study of Esterfill adhesives with chemical and light hardening].

The purpose of this work is to define the dependence of adhesive bond strength on a way of cure of adhesive system, application of the primer and bond in two step or in one step in vitro. Chemically curing "Esterfill bond" (AB) and both light-curing primer and bond, and composite hybrid light-curing restorative material "Esterfill PHOTO" (EP) (DIAS LTD, Russia) were used. The shear bond strength was measured according to recommendation of ISO 11405:1994. The apparatus for these tests corresponds to ISO 10477, addition I. Shear bond strength was measured on enamel of 83 extracted human molars. Teeth were divided into 7 groups. Before using of adhesive system enamel was etched by 37% phosphoric acid during 60 sec. For measuring of shear bond strength the post composite restorative material "EP" (dentin) has been used. The received data show that the shear bond strength does nat depend on the type of cure, the composition of the bonds being similar. Using primer before application of the bond increases the shear bond strength approximately by 4-5 MPa. The inclusion of the primer in the content of the bond system increases the adhesion to enamel by 9 MPa.

[Patterns of mercury release from amalgam fillings into the oral cavity].

Seventy-five subjects aged 20 to 57 with 1 to 15 fillings of silver amalgam were examined. The level of mercury vapors in the oral cavity was assessed using an AGP-01 device and the method developed by the authors. Emission of mercury vapors in the oral cavity increased with the number of fillings. The concentration of mercury in the oral cavity depends largely on the number of silver amalgam fillings and less so on these fillings' length of service.

[The treatment of local gingival recession by using Kolapol].

Forty-seven patients with local recession of the gingiva of different etiology were treated by surgery with and without colapol. Clinical examinations and the findings of rheoparodontography and polarography indicate a high efficacy of the suggested treatment making use of a composite material on the base of hydroxyapatite.

[The use of leukinferon in treating measles in adult patients].

The following mucosal are characteristic of measles in adult patients: dryness in the oral cavity, dryness, desquamation, and fissures in the red edge and in the mouth corners, edema and hyperemia, of the buccal mucosa, macular enanthema on the hard and soft palate, Bel'skiĭ-Filatov-Koplik spots, whitish deposit on the gingiva, edematous and bleeding gingiva, and, as a result of general intoxication, a lingual deposit. Leukinferon, a wide-spectrum immunocorrective drug, was used to improve the efficacy of treatment of adult patients with measles; besides alpha-interferon, it contains other cytokines of the first phase of immune response. Therapy with leukinferon led to a sooner improvement of the general status and a more rapid regression of the disease symptoms, including changes in the buccal mucosa, in comparison with the control leukinferon-untreated group.

[The use of eiconol in the combined treatment of lichen ruber planus of the oral mucosa].

Clinical and laboratory examinations of patients with lichen ruber planus revealed diseases of the hepatobiliary system in 85%, pancreatic diseases in 29%, chronic gastritis with manifest secretory insufficiency in 32.5%, and fatty hepatosis in 17.5% of patients. Biochemical analysis of the peripheral blood in patients with the exudative hyperemic and erosive ulcerative forms of the disease revealed hypercholesterolemia and hypertriglyceridemia. Administration of eikonol in a dose of 6 g/day led to reduction of triglyceride level by 18% and of total cholesterol by 6%, the hypocholesterolemic and hypotriglyceridemic effect of the drug being more expressed in the patients with initially higher levels of these parameters. Eikonol therapy promoted an improvement in the clinical course of lichen planus. A positive time course of changes was observed in 69% of patients with the exudative hyperemic form of the disease (less expressed unpleasant subjective sensations, decrease of hyperemia of the buccal mucosa and of the size of foci of lesions, less pronounced papular pattern). In the group with the erosive ulcerative form of the disease only 18% of patients developed positive changes under the effect of therapy (epithelialization of erosions, attenuation of the inflammatory reaction, less expressed papular pattern). Combination of eikonol with applications of keratoplasties to the foci of lesions in patients with the erosive ulcerative condition brought about a better therapeutic effect, with the periods of epithelialization reducing by 7 to 10 days.

[Diphtheria of the oral mucosa].

High incidence of diphtheria became a pressing problem in recent years. The age structure of patients has changed. A specific feature of diphtheria in adults is a high share of toxic forms and a higher incidence of grave combined forms and rare localizations of the disease focus. The oral cavity proper is one of rare localizations of this infection. The authors examined 12 patients in whom the process was localized in various parts of the buccal mucosa: on the upper and lower lips, on the hard and soft palate, on the cheeks and tongue. Morphologic disturbances presenting as necrosis and inflammatory infiltration were detected in sites involved in the diphtheritic process. Changes whose manifestation was related to the diseases severity were found in zones adjacent to the foci. The authors come to a conclusion on a higher incidence of heretofore rare localizations of diphtheria than previously, this making difficult the diagnosis and treatment of the disease.
