Flexible carbon fibre in late ligamentous reconstruction for instability of the knee.

Sixty-three knees with chronic ligamentous instability treated with flexible carbon fibre are reviewed. Assessment was by pre-operative and postoperative grading of function in work and sport, together with a subjective evaluation of the result based on stability. Fifteen knees had collateral repairs, seven had cruciate repairs and 41 had combined collateral and cruciate repairs. Overall, 71% of knees showed improvement in function and 67% had good or excellent results subjectively. Clinical examination did not correlate well with the patient's evaluation of the result. Nineteen unselected cases with intra-articular (cruciate) carbon fibre had arthroscopies. Infiltration of collagen tissues into the cruciate replacement was a slower process than at the extra-articular (collateral) site. There was no clinical or macroscopic evidence of synovitis but microscopically there was evidence of synovial irritation.

Aggressive surgical treatment of secondary spinal deformity in spina bifida children--is it worthwhile?

The results of sixty-five surgical corrections of secondary spinal deformities in spina bifida children are reported. Twenty-three patients had kyphectomies, and thirty-nine patients had anterior and posterior spinal fusions for scoliosis. There were three deaths. Patients in both groups showed functional improvement, though the effects on respiratory function were minimal. Patients with scoliosis should have their surgery before the curve reaches 100 degrees. Correction in these patients may have to be carried out very early if there is rapid curve progression. Kyphectomy ideally should be carried out between the ages of ten and fourteen years when bone size and quality are suitable for fixation.

The ball and socket ankle joint.

This paper reports fifteen cases of ball and socket articulation at the ankle followed up for an average of twelve years. All patients showed inequality of leg length. Ten patients showed coalitions of the bones of the hindfoot and nine patients had a reduction in the number of bony elements of the forefoot. Other associated anomalies are described. The abnormality seems to be part of a congenital short-limb malformation, perhaps modified by adaptive change associated with rigidity of the hindfoot. Surgical intervention was not required in any patient in the series.

Aspirin prophylaxis of venous thromboembolic disease following fracture of the upper femur.

In a prospective study of 51 patients with fractures of the femoral neck, aspirin was used as a prophylactic measure against thromboembolic disease. Thrombi were detected by cuff impedence plethysmography, Doppler ultrasonography and ascending venography. Thrombi were identified in 20 (39.2%) of the patients. There was no significant difference between the frequency with which thrombi occurred in men and in women. Blood salicylate values were the same for patients who had and who did not have thrombi. There were no instances of pulmonary embolism. The frequency of deep vein thrombosis was comparable to that in a previous series of untreated patients from the same centre. It appears from this study that in these cases prophylaxis against venous thromboembolism using aspirin in a dosage of 600 mg bid is ineffective.
