Speech perception using a two-formant 22-electrode cochlear prosthesis in quiet and in noise.

A new speech-processing strategy has been developed for the Cochlear Pty. Ltd. 22-electrode cochlear prosthesis which codes an estimate of the first formant frequency in addition to the amplitude, voice pitch and second formant frequencies. Two groups of cochlear implant patients were tested 3 months after implant surgery, one group (n = 13) having used the old (F0F2) processing strategy and the other (n = 9) having used the new (F0F1F2) strategy. All patients underwent similar postoperative training programs. Results indicated significantly improved speech recognition for the F0F1F2 group, particularly on open set tests with audition alone. Additional testing with a smaller group of patients was carried out with competing noise (speech babble). Results for a closed set spondee test showed that patient performance was significantly degraded at a signal-to-noise ratio of 10 dB when using the F0F2 strategy, but was not significantly affected with the F0F1F2 strategy.

Electrode discrimination by early-deafened cochlear implant patients.

Electrode discrimination was measured in six cochlear-implant patients who became profoundly deaf prior to the full development of auditory and speech skills. The cochlear implant manufactured by Cochlear Limited was used. Comparisons were made between two electric stimulation paradigms and two data collection procedures. The paradigms consisted of electrode trajectories and electrodes with random variation in electric current levels. The data collection methods consisted of an adaptive procedure to obtain difference limens and a method of constant stimuli that gives psychometric functions of discrimination performance. Data were collected for reference electrodes at the apical, mid, and basal positions on the array. The psychometric functions showed that discrimination performance improved with increases in the spatial separation between reference and comparison electrodes. The difference limens were similar to the corresponding spatial separation estimated from the psychometric function in most cases. The discrimination performance of most patients was consistent across the two stimulation paradigms. Difference limens were 1-4 electrodes from the reference electrode for most patients. There were differences in performance across patients and the three positions on the array.

Studies of prosody perception by cochlear implant patients.

Prosodic information is conveyed to normally-hearing listeners by variations in acoustic fundamental frequency, amplitude envelope, and duration of speech segments. This study measured cochlear implant patients' sensitivity to these parameters in electrically coded speech. The psychophysical discrimination of electric parameters used to code prosodic information, were examined, together with prosody perception using speech processing strategies which modified the contributions of these parameters. Patients were implanted with the Cochlear Limited prosthesis and used the MPEAK speech processing strategy. In the psychophysical studies, difference limens were measured for steady-state and time-varying stimuli, of different pulse rates and pulse durations, over a series of different stimulus durations. These limens were obtained using an adaptive procedure which converged on the 50 per cent correct point. In the prosody perception studies, performance was measured for the MPEAK strategy and for strategies which modified the contributions of pulse rate and pulse duration. Data were collected for five tests of prosodic contrasts. Difference limens for steady-state pulse rates were larger at higher rates (17 per cent at 400 pulses/s) than at lower rates (6 per cent at 100 pulses/s). For some patients, limens for the time-varying pulse rates were larger than those for the steady-state pulse rates while for the other patients, the limens were similar. Difference limens for pulse duration were 0.3 dB, corresponding to 4 per cent of the dynamic range, for steady-state stimuli and doubled in size for the time-varying stimuli. Prosody perception performance was generally poorer for the modified strategies than for the MPEAK strategy, suggesting that the removal of information coded by pulse rate and pulse duration reduced the perception of prosodic contrasts.

A multi-channel hearing prosthesis for profound-to-total hearing loss.

A multi-channel cochlear implant hearing prosthesis providing 22 separate channels of stimulation has been developed. The electronics for the implantable receiver-stimulator have been incorporated on a single chip, using digital circuits and employing CMOS technology. The chip is enclosed in a titanium capsule with platinum/ceramic electrode feed-throughs. A pocket-sized speech processor and directional microphone extract the following speech parameters: signal amplitude, fundamental frequency and formant frequency. The fundamental frequency is coded as electric pulse rate, and formant frequency by electrode position. The speech processor has been realized using hybrid circuits and CMOS gate arrays. The multi-channel prosthesis has undergone a clinical trial on four postlingually deaf patients with profound-total hearing losses. The speech perception results indicate that they were able to obtain open-set speech recognition scores for phonetically balanced words, CID sentences and spondees. In all cases the tests showed significant improvements when using the cochlear prosthesis combined with lipreading compared to lipreading alone.

Role of progesterone receptors in breast cancer.

It has been demonstrated that progesterone receptors (PRs) are at least as valuable as estrogen receptors (ERs) for predicting outcome in breast cancer patients. Retrospective analysis indicates that the presence of PRs may be the second most critical factor, after the number of positive nodes, in predicting disease-free survival, with a correlation between length of survival and number of tumor PRs. The presence of PRs has been shown to be of value for predicting response in both early and advanced breast cancer patients. In studies of assay consistency, major discordance rates were minimal in simultaneous assays, but extremely high in sequential assays of tumors that were PR-positive in initial assay. The responsible factor was interim endocrine therapy, and it was subsequently determined that the prognosis was worse for those patients whose tumors lost PR between assays.

Expression of HER-2/neu oncoprotein in human breast cancer: a comparison of immunohistochemical and western blot techniques.

Three hundred and one primary breast cancers from patients with tumor infiltrated lymph nodes were analyzed for the presence of HER-2/neu oncoprotein by two procedures: Western blot (WB) and immunohistochemistry (IHC). Overexpression of this protein was found by WB in 16.6% of the tumors, and by IHC in 16.3%. Concordance between the two methods was found in 95% of tumors (286/301). In 7 cases we found HER-2/neu by IHC but not by WB, while the opposite was found in the remaining 8 patients. This discrepancy was found mainly in samples with HER-2/neu values just above the cut points and were therefore close to the sensitivity limits of the procedures used here. This study helps to define the parameters that should be considered to evaluate the immunostaining for HER-2/neu as positive (i.e., membrane staining, IHC score of 2 or more). The results obtained by both techniques were correlated with several currently used prognostic factors. Higher HER-2/neu protein expression was found in tumors lacking estrogen or progesterone receptors, in tumors with high S-phase fraction and in patients with more than 3 positive lymph nodes. In contrast, no relationship was found between overexpression of this protein and tumor size, ploidy, or age of the patient. Patients with elevated HER-2/neu expression showed a significantly worse overall survival by both methods, IHC (p = 0.05) and WB (p = 0.001). In conclusion, there is very high agreement between IHC and WB when measuring expression of HER-2/neu and both techniques showed prognostic significance.

Results of a preliminary clinical trial on a multiple channel cochlear prosthesis.

Speech discrimination testing was carried out under clinical trial conditions for eight profoundly postlingually deaf adults to assess the efficacy of a newly developed 22-channel cochlear prosthesis and speech processor. Three months postoperatively, these patients showed significantly better results with the cochlear prosthesis than for preoperative testing with a conventional hearing aid or vibrotactile aid (following a 6-month trial with the aid) on each of a series of tests from the Minimal Auditory Capabilities battery. Assessment of lipreading enhancement using standard speech tests, consonant recognition studies, and speech tracking showed significant improvements for each patient when using the cochlear prosthesis. Six patients showed a significant amount of open set speech discrimination without lipreading at levels which have not been reported for single electrode cochlear prostheses. The two patients who performed poorly on these tests both had restricted multiple channel systems due to their disease, one patient being restricted to virtually a single channel system and the other to only ten of the 22 electrodes. These results indicate that this multiple channel cochlear prosthesis has potential as a treatment for profound postlingual deafness over a wide range of etiologies and ages.

Sex-hormone-binding globulin and breast cancer risk.

We have measured serum concentrations of SHBG in 5000 women over the age of 35. In both premenopausal and postmenopausal women who were not suffering from cancer or other diseases or taking drugs likely to affect SHBG, the protein decreased with increasing weight and was lower in single nulliparous women than in married nulliparous women or parous women. In premenopausal women, SHBG was higher in women with late menarche. These findings suggest that diminished SHBG concentrations may be associated with the increased risk for breast cancer conferred by nulliparity and early menarche.

Clinical significance of the isomorphic pattern of the isoenzymes of serum lactate dehydrogenase.

Five hundred patients with the isomorphic pattern of the isoenzymes of serum lactate dehydrogenase (LDH) were surveyed. The isomorphic pattern of LDH isoenzymes is defined as a significant increase of total LDH with normal or low percentage of individual fractions, but with the LDH1:2 ratio less than unity. Diagnoses were, in descending order of frequency, cardiorespiratory diseases, malignancy, fracture, diseases of the central nervous system, infection/inflammation, hepatic cirrhosis and/or alcoholism, trauma without fracture, infectious mononucleosis, hypothyroidism, uremia, necrosis, pseudomononucleosis, viremia and intestinal obstruction. Incidence of increased serum activity in individuals without evidence of disease or drug explanation was 3 percent. Low PaO2 was observed in 88 percent of the 67 patients in whom it was measured.

A statistical and clinical evaluation of fingerstick and routine laboratory prothrombin time measurements.

STUDY OBJECTIVE: To compare prothrombin time measurements by fingerstick and routine laboratory methods. DESIGN: Prospective cohort study. SETTING: University-affiliated anticoagulation clinic. PATIENTS: Thirty-three patients receiving warfarin with stable anticoagulation for 3 months preceding the two studies. INTERVENTIONS: Groups 1 (17 patients) and 2 (16 patients) provided 150 and 125 paired samples, respectively, for fingerstick and routine laboratory analysis. The fact that no patient required a dosage change allowed for a clinical assessment. MEASUREMENTS AND MAIN RESULTS: Correlation and agreement between methods were good in group 1 but poor in group 2. Fingerstick results were less variable (smaller standard deviation and smaller coefficient of repeatability) in both groups. By analysis of discrepant pairs (25 in group 1, 63 in group 2), the routine laboratory results indicated dosage changes erroneously more often than did the fingerstick method. CONCLUSIONS: In these two trials, the fingerstick system was superior to the routine laboratory method in that it was more reliable (less variability and more repeatable) and less likely to indicate dosage changes erroneously.
