Analytical biochemistry of seminal vesicle secretion: a challenge to andrological laboratories.

Chemical characterization of seminal vesicle secretion through seminal vesicle proteins would have the following advantages: (1) to judge on the secretory competence of the gland, (2) to recognize atypical secretory patterns, (3) to identify specific molecules and their epitopes for anatomical, diagnostic, therapeutic, anti-fertility and forensic purposes, and (4) to study physiologically active proteins or peptides of seminal plasma. There are different approaches for collection of the specific samples, each of which has peculiar advantages and disadvantages: ejaculate collection in the presence of protease inhibitors, use of split or multi-split ejaculates, utilization of autopsy or surgical material. Liquefied proteins are submitted to different chromatographic and electrophoretic procedures. One must keep in mind, however, that a whole series of biochemical processes can rapidly and irreversibly alter in vivo and in vitro the secretory proteins. The study of the secretion from male accessory sex glands and their interaction with spermatozoa therefore still deserves an absolute research priority.

Testosterone precursors in spermatic venous blood of normal men and varicocele patients. A study of delta 4 pathway of testosterone biosynthesis.

In the present study we determined progesterone (P), 17-OH-progesterone (17-OH-P), androstenedione (delta 4), dehydroepiandrosterone (DHEA) and testosterone (T) in spermatic venous blood of 34 varicocele patients and of 13 normal subjects. We also used the DHEA/delta 4 ratio as an index of the delta 5/delta 4 pathway ratio in testosterone biosynthesis. The mean of T and delta 4 in the spermatic blood of varicocele (V) patients appeared to be significantly lower with respect to that of normal (N) subjects (T:N = 1718.2 +/- 202.4 (SEM) nmol/l, No. 11; V = 1243.7 +/- 97 (SEM) nmol/l, No. 34; P less than 0.03. delta 4: N = 56.4 +/- 5.6 (SEM) nmol/l, No. 12; V = 38.1 +/- 4 (SEM) nmol/l, No. 27, 0.02 greater than P greater than 0.01). A negative correlation was observed between the individual age of varicocele patients and 17-OH-P (No. 34, y = -30.66x + 1300, r = -0.57, P less than 0.01) delta 4 values (No. 27, y = -1.981x + 96.52, r = -0.67, P less than 0.01). When the ratio of T precursors was evaluated, we observed a positive correlation between the P/17-OH-P ratio and age of varicocele (No. 33, y = 0.0065x-0.092, r = 0.45, P less than 0.03). The 17-OH-P/delta 4 ratio was greatly increased with respect to that of normal subjects (N = 5.12 +/- 0.93 (SEM), No. 12; V = 10.77 +/- 1.31 (SEM), No. 27; P less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical aspects of testicular biopsy in the infertile subject].

Testicular biopsy was once much used in the diagnosis of male infertility. Current clinical experience suggests that it should be confined to a prioritary method in cases of azocytospermia and serious oligozoospermia with low or normal plasma FSH, and as a routine procedure in corrective surgery (varicocelectomy, orchidopexy, etc.). An account is given of the technical variants of the operation, its complications and contraindications, and a brief reference is made to discordant opinions expressed by leading andrologists concerning its indications. It is felt that it should only be performed in suitably selected cases, and always in the event of thorough exploration of the testis and its adnexa.

High-resolution protein patterns of human expressed prostatic secretion: a new tool for the diagnosis of prostatitis.

Expressed prostatic secretions (EPS) obtained from 62 infertile and urological patients have been analyzed for their protein content by sodium dodecylsulfate electrophoresis and isoelectrofocusing. Independent cytological and bacteriological screening on the same EPS samples was carried out to detect prostatic inflammation. EPS from both urological and infertile patients without inflammation had simple and characteristic protein patterns. Important qualitative and quantitative differences in the protein composition of EPS from patients with severe dysspermia or prostatic inflammation were found. The comparison of EPS protein patterns with those of seminal plasma obtained from the same patients led to the detection of a characteristic group of low-molecular-weight bands that were not of prostatic origin.

Anaerobic and aerobic bacteria in secretions of prostate and seminal vesicles of infertile men.

A cytological and bacteriological study was made of prostatic (EPS) and vesicular (EVS) secretions from 123 infertile men who were suspected of having chronic genital tract inflammation and from 31 men with premature ejaculation (1,12,16). In the microbiological investigations samples were inoculated within 10 min on various culture media and incubated under both aerobic and anaerobic conditions. Bacterial loads of more than 10,000 colony-forming units of a single species or genus per milliliter of EPS or EVS were considered to be pathological. In the infertile subjects with proven inflammation of the seminal accessory glands, the EPS and the EVS that gave positive cultures and had bacterial loads defined as pathological contained large numbers of anaerobic or microaerophilic organisms (EPS: 51 of the 63 bacterial strains found, congruent to 81%; EVS: 19 of the 20 bacterial strains found, congruent to 95%).

Doppler examination in varicocele. A standard method of evaluation.

Doppler sonography is considered a reliable method for detecting reflux due to venous valvular incompetence, which occurs in varicocele. It is, however, a matter of debate whether the characteristics of this reflux can be correlated quantitatively in a clinical setting. In this study, a two-step method was utilized to identify reflux in basal conditions with the patient standing and breathing spontaneously and to determine the time of centrifugal secondary venous reflux of the distal spermatic cord during the squeezing and relaxing maneuver. This method is closely related to that used by the vascular surgeon to detect valvular incontinence of the saphenous vein. Since Doppler sonography is much more reproducible than Valsalva's maneuver, it is therefore much more reliable. In a 12-month period, 625 patients with pathologic findings in at least two spermiograms were studied. Thirty percent showed constant basal reflux not influenced by respiratory exhilation. The squeezing and relaxing maneuver induced secondary reflux longer than 1.6 seconds in 17%, between 0.8 and 1.6 seconds in 17% and less than 0.8 seconds in 36% of the patients. The higher sensitivity and specificity of Doppler examination compared with thermography and angiography, as well as its low cost and noninvasiveness, make this the procedure of choice in the diagnosis of venous reflux in varicocele.

Low molecular weight proteins in human seminal plasma electrophoretical evidence of their vesicular origin.

The protein composition of seminal plasma (SP), two-fractions split ejaculates and expressed prostatic secretions (EPS) from healthy and fertile donors or from infertile patients has been analyzed under proteolysis-blocking and denaturing conditions by 5-20% gradient SDS-PAGE. EPS protein patterns were found to be strikingly similar, both in protein types and concentrations. The same was true for the SP samples. Co-electrophoresis and comparison of the patterns obtained allowed to identify a medium-sized protein (MW 75-80,000 daltons) and low-MW proteins (MW 10-25,000 daltons) as of vesicular origin. Furthermore, prostatic and vesicular proteins were found to migrate into different zones of the gels, giving raise to an unexpected MW "complementarity". A similar finding was obtained by comparing the protein spectra of fluid from vesicular massage (EVS), EPS and SP from a man with moderate asthenozoospermia. However, analysis of nine other EVS samples by the same electrophoretical technique revealed that the majority of them were (sometimes heavily) contaminated by prostatic proteins. It was concluded that SDS-PAGE on gradient gels is of help to distinguish and characterize the proteins secreted by the two major male accessory glands. Such a finding has both research and clinical applications.

Evoked sacral potentials in subjects with true premature ejaculation.

Evoked sacral potentials were used to study the bulbo-cavernous reflex in 85 patients with true premature ejaculation (TPE) and in 52 subjects as a control group. In the perineal and perianal recordings the amplitudes of the evoked responses (means +/- SD) were 70 +/- 91.1 microV and 35.6 +/- 36 microV in the TPE patients, and 39.2 +/- 36.8 microV and 26 +/- 21.6 microV in the controls, respectively. Both these differences resulted significant (P less than 0.01 and P congruent to 0.05, respectively). Our results suggest a reflex hyperexcitability, or an impaired "modulation" of the motor neurons of the pudendal nucleus by the regulating upper centers in the TPE patients.

Penile arterial Doppler evaluation in basal condition and after intracavernous injection of papaverine.

In diagnostic screening, 73 impotent patients were submitted to a Doppler study of the deep penile arteries under basal conditions and after oral nitroglycerin (Basal-DOPPLER) and after intracavernosal injection of 30 mg of papaverine (ICIP-DOPPLER), as well as other tests. The results of the Basal-DOPPLER and the erectile response to intracavernosal administration of 30 mg papaverine (ICIP-30), indicative of the condition of the deep penile arteries, were in total agreement for 33 cases, partial agreement for 29 cases and totally in disagreement for 11 cases. The agreement between the results of the ICIP-DOPPLER and the ICIP-30 was total for 40 cases, partial for 24 and totally lacking for 9. In many of the cases for whom the response to ICIP-30 was less than would have been expected from the ICIP-DOPPLER, abnormal venous outflow was later shown. Our data support the hypothesis that papaverine acts on the venous component of erection. The ICIP-DOPPLER was found to be apparently more reliable than the Basal-DOPPLER for evaluating the arterial component of erection, but Basal-DOPPLER is still the basic screening test before any such invasive test as the ICIP are used, since these latter are not always accepted immediately by the patients.

Effectiveness mechanism of chlomipramine by neurophysiological tests in subjects with true premature ejaculation.

The effectiveness mechanism of the chlomipramine treatment was evaluated by sacral evoked response (SER) and dorsal nerve somatosensory cortical evoked potential (DN-SEP) testings in 15 patients with true premature ejaculation (TPE). We couldn't demonstrate any significant difference between the values of either latency times or amplitudes of the evoked responses determined just before and at the end of the treatment with chlomipramine in these patients. However, the sensory thresholds were 24.4 +/- 4.3 V in the pretreatment term and 30.2 +/- 7.3 V at the end of the treatment. This difference is statistically significant (P = 0.0031). Our results suggest that chlomipramine increases the sensory threshold for the stimuli in the genital area.

Inflammatory pathology of the genital tract and male infertility. A short review.

The relationship between non-specific infections of the male genital tract (i.e. infections other than gonorrhea and genital tuberculosis) and male or couple infertility/subfertility still reassess various problems related to this important subject and to complement it with some original observations from our clinical experience. The reader is also invited to refer to a previous paper by Gattuccio et al. (1988), which more specifically addresses the question of the inflammatory diseases of the prostate gland.

A direct action of kallikrein on spermatozoal motility?

Semen samples from infertile normozoospermic and slight to moderate asthenozoospermic subjects were tested in parallel with and without kallikrein (dry substance) by the Kremer's capillary tube test. Penetration in human midcycle cervical mucus and AB Rh + serum was evaluated. Kallikrein increased spermatozoal penetration into cervical mucus after 3 hrs incubation (p 0.01 in normozoospermic subjects).

Functional voiding disturbances of the ampullo-vesicular seminal tract: a cause of male infertility.

We present herein six cases of male sterility due to purely functional ampullo-vesicular tract voiding disturbances, out of a series of 94 vasovesiculographies performed on selected patients with suspected obstructive azoo-or oligozoospermia. This pathology was suspected from the particular seminal vesicle ultrasonographic pictures (anechoic areas) and the absence or marked reduction of the vesicular component in the ejaculate. The non-anatomic cause of the ampullo-vesicular voiding disturbance was demonstrated by means of the vesiculographic images and by the finding of large amounts of spermatozoa in the fluid obtained by micturition or bladder catheterism after seminal tract washout during vasovesiculography. Until now and to the best of our knowledge, only four other such cases were described in Literature, although documented in a different way (Ichijo et al., 1981). We may suppose that this new syndrome, possibly due to a local neurological defect, could be a not so rare cause of male infertility. Therapeutical attempts are also described.

Cortical evoked potentials in subjects with true premature ejaculation.

Cortical somato-sensory evoked potentials (SEPs) from pudendal stimulation were determined in 23 patients with true premature ejaculation (TPE) and in 25 subjects as a control group. SEPs give a cortical representation of the sensory stimuli arising from the genital area. The amplitudes of the evoked response in the cortical area (P1-N1-P2 wave) were 4.96 +/- 1.89 microV in subjects with TPE and 3.79 - 1.38 microV in the controls (means +/- SD). This difference is statistically significative (P less than 0.005). Our results show a greater cortical representation of the sensory stimuli from the genital area and suggest an organic basis for TPE.

Seminal tract inflammation and male infertility. Correlations between leukospermia and clinical history, prostatic cytology, conventional semen parameters, sperm viability and seminal plasma protein composition.

A retrospective clinical evaluation of various types of semen analyses from infertile couples attending our Infertility Clinic was undertaken with the scope of studying the possible correlation between seminal inflammation and infertility. The parameters considered were leukospermia (severe, slight, constant or non-constant), clinical history (anamnestic data possibly inferring inflammation), conventional semen parameters, sperm viability (as assessed by capillary tube in vitro penetration test) and seminal plasma proteins patterns (SDS-PAGE). History data such as dysuria, urinary infection, cystitis symptoms and hematospermia were found to be significantly more frequent in infertile men with than in those without leukospermia. Leukospermia in itself did not seem to affect the conventional semen parameters such as total sperm count, motility (at 45 and 180 min) and/or morphology. The seminal volume could represent an exception to this rule. Furthermore, leukospermia did significantly affect sperm viability as evaluated by the capillary tube penetration test. Leukospermia was also significantly coupled to alterations of the seminal plasma protein composition (increase of the albumin concentration, decrease of prostatic markers and other anomalies).

Fertilization of human oocytes by spermatozoa from an artificial spermatocele.

Spermatozoa from a patient with an artificial spermatocele (Dardik's prosthesis) implanted onto the vas deferens were found to have high levels of IgG/IgA antibodies absorbed on their surfaces (MAR-test). Pre-treatment of such sperm with Percoll/BMW (d = 1.05) apparently reduced their MAR positivity and improved their ability to penetrate into ZP-free hamster oocytes or into the oocytes of the wife, respectively.
