Acute orbital cellulitis: a multidisciplinary emergency.

A case of acute orbital cellulitis in an adolescent girl is reported and compared with ten cases treated in the same hospital in recent years. The clinical features and stages of orbital cellulitis and its differentiation from cavernous sinus thrombophlebitis is discussed. Although these complications are nowadays rare with modern chemotherapy and antibiotics, they may still occasionally result in blindness and death. (Duke-Elder et al., 1974).

Clinical electromyography in ear, nose and throat practice.

A new method of surface 'integrated' electromyography is described for use in ENT practice. It is particularly suitable for the relatively delicate muscles of the mouth and throat. A special bipolar surface electrode has been designed to sample the interarytenoid, posterior crico-arytenoid and crico-pharyngeal sphincter. Very important EMG abnormalities have been found in a variety of neurolaryngological disorders, and it is expected that surface EMG will very soon become the method of choice in otolaryngology.

A new method for laryngeal electromyography.

A new method of surface electromyography (EMG) is presented, for sampling the posterior crico-arytenoid, interarytenoid and crico-pharyngeal sphincter. The results obtained in normal volunteers are discussed and the indications for laryngeal electromyography (LEMG) outlined. It has the advantage of being non-invasive and safe, and can be done as an out-patient procedure with minimal discomfort to the patient, and unassociated complications. This has provided a new and fascinating field of investigation in neuromuscular disorders of the larynx. The same principles of surface EMG may also be applied to the muscles of the tongue, palate, pharynx . . . and surface EMG in the upper respiratory tract may become a valuable method of clinical investigation.

Pathology of the intrinsic muscles of the larynx.

The intrinsic muscles of the larynx were exposed in each of 54 normal larynges derived from the autopsy series of a general hospital and submitted to microscopic examination. Muscle fibre alterations were found in all the posterior cricocarytenoid muscles in patients from 13 years of age on wards. In about half of the patients these changes included necrosis of segments of muscle fibres and secondary cellular reactions to this. In two cases of chronic autonomic failure with multiple system atrophy the intrinsic laryngeal muscles were similarly examined. Both cases showed a marked gross and microscopic atrophy of the posterior cricoarytenoid muscles; there were also microscopic changes similar to those found in the normal population. The significance of the changes in the routine post-mortem larynges, the changes in the cases of chronic autonomic failure with multiple system atrophy and the relationship between the two are discussed.

Laryngeal electromyography in multiple system atrophy with autonomic failure.

In five patients with multiple system atrophy, electromyographic studies using a new surface electrode technique, showed unequivocal evidence of denervation of the posterior crico-arytenoid muscle, and in three of them, of the interarytenoid muscle or crico-pharyngeal sphincter. By comparison, only minor electromyographic abnormalities were found in one-third of patients with other extra-pyramidal syndromes, though in two cases, the abnormalities were of a different kind from those found in multiple system atrophy.

Nasopharyngeal mechanoreceptors and their role in autoregulation of endotympanic pressure.

Gairn's method for nerve endings was used to study the mucous membrane of the nasopharynx. A simple type of end bulb was found in the fossa of Rosenmüller. The end bulbs were especially abundant near the tubal orifice. They are probably part of a pressure sensor mechanism having a direct reflex control on Eustachian tube musculature for the equalisation of middle ear pressure.

Lateralization of the Weber response after stapedectomy.

The lateralizing value of the Weber test has been studied in relation to otosclerosis and stapedectomy. Special attention is given to the occurrence of equivocal and paradoxical results whenever the conductive pathology is different on the two sides, a fact that has to be remembered by the clinician during his every day use of the Weber test.

Congenital labyrintho-tympanic fistula -- a recently recognized entity in children.

A case of congenital labyrintho-tympanic fistula is reported. The possible anatomical pathways and main clinical characteristics are discussed together with a review of the literature. Special emphasis is made for early recognition in children, of this entity without invasive forms of investigation, and the proper surgical approach via tympanotomy is described.

A reappraisal of the salpingo-pharyngeus muscle.

Closure of the eustachian tube has been studied as a function of the salpingo-pharyngeal muscle. The muscle was dissected in ten cadavers and examined histologically. Its elastic fibres and anatomic relations especially during swallowing, together with other data, all confirm that the muscle is active at rest and relaxes during deglutition.
