[Lung cancer: present concepts of treatment].

The role of the various treatment modalities of bronchial carcinoma in respect to clinical stage and histology is described. Criteria to select patients for surgical treatment and for radiotherapy with curative or palliative intent are discussed. The present state of cytotoxic therapy in respect to adjuvant treatment as well as its use in metastatic disease is summarized. In small cell carcinoma combination chemotherapy may result not only in palliation, but in an improved survival in a high percentage of patients. Recommended protocols, remission rates and the long-term prognosis of treated patients are summarized.

Antineoplastic Agents↗

[The radiological diagnosis of bronchial atresia (author's transl)].

Congenital atresia of the apico-dorsal segmental bronchus of the left upper lobe in a 10-year-old boy is described. The radiological features of bronchial atresia are described in detail and the view is expressed that the diagnosis can be made on plain films of the chest with tomography. According to present day concepts, treatment is not indicated in the absence of symptoms.

Bronchi↗

[The problem of endotoxin shock after antibiotic therapy in surgery (author's transl)].

2 fatal cases of endotoxin shock after antibiotic therapy due to massive Escherichia coli infection after surgery are described to emphasize this rare possibility of complication. In cases of major infections with Gram-negative bacterais antibiotics should therefore prophylactically be given initially in low doses and only after observation in increasing amounts.

Appendicitis↗

[Contribution to biliobronchial fistula (author's transl)].

A case report has been made on a bronchobiliary fistula which was caused from an unspecific liver abscess. The causal therapy was the drainage of the liver abscess. The fistula then healed spontaneously. A primary thoracic sanitation of the fistula is in such cases not indicated.

Biliary Fistula↗

[Functional examination findings after parietal pleurectomy in patients with recurrent of spontaneous pneumothorax (author's transl)].

Eight patients with recurrent of spontaneous pneumothorax were re-examined sixteen months or more after a total parietal pleurectomy had been carried out. This re-examination included: 1. Detailed case history with emphasis on physical performance; 2. clinical findings; 3. Thorax x-ray at two planes; 4. Lungscintigraphy; 5. Spirometry. Following parietal pleurectomy, none of the patients exhibited any functional disturbance worthy of mention. This operative technique, perhaps modified as a partial parietal pleurectomy, can be recommended at this time as the method of choice for treating recurrent spontaneous pneumothorax.

[An oesophagotracheal Fistula following prolonged intubation (author's transl)].

Attention is drawn to the case of an oesophagotracheal fistula following prolonged intubation. A combination of physical factors, the use of unsuitable cuffs and a nasogastric feeding tube, are considered to be the cause. Surgical repair of the fistula is advised as soon as possible. While primary closure of the oesophageal defect is always possible, suture of the membraneous trachea may be impossible and a graft may be necessary to cover the defect. For this is recommended oral mucosa, which achieved very good result in this reported case.
