Influence of tonic reflex activities on the slow auditory evoked potential.

Possible influences of tonic reflex activities on the slow auditory evoked potential were investigated. For this purpose, the slow auditory evoked potential was picked up in 18 volunteers with normal hearing in relaxed position and in postures corresponding to reflex-induced positions and in 15 patients with cerebral palsy (CP) in tonic reflex patterns. No influence on latency for N120 and P215 due to postures could be observed. Latency within CP patients was age-dependent. The effect of the reflex-inhibiting pattern (RIP) - the pattern inhibiting tonic reflex activity in CP patients as far as possible - on potential amplitude was different in the two groups. The effect of RIP in CP patients was comparable to the relaxed position in volunteers. This difference points toward an influence of tonic reflex activity on the slow auditory evoked potential.

Molecular modelling studies on G protein-coupled receptors: from sequence to structure?

In pharmaceutical research G protein-coupled receptors (GPCR) emerged as a superfamily of prominent drug targets. Extensive protein sequence analyses on GPCRs revealed a common protein topology consisting of a membrane-spanning seven-helix bundle, which is believed to accommodate the binding site for low-molecular weight ligands. Enormous efforts are undertaken to generate GPCR structural models by means of molecular modelling, since these have already been shown to aid the process of lead structure finding and optimisation in that they provide atomistic models for structure-based drug design approaches. One of the most critical steps in modelling the transmembrane domains of GPCRs is the assignment of the putative transmembrane sequence stretches from multiple protein sequence alignment analyses. This study focuses on the comparative evaluation of protein sequence analysis tools, such as periodicity analyses, multiple sequence analyses or directional helix descriptors, especially developed for modelling the 7TM domains of GPCRs. In this context we will demonstrate that from application of different methods contradictory results can be obtained for the identification of the putative transmembrane sequence stretches of peptide-binding GPCRs, as exemplified with a comprehensive protein sequence analysis study based on the most prominent members of that receptor class (angiotensin II, CCK/gastrin, interleukin 8, endothelin, etc.).

[Chronic diarrhea as a clinical expression of common variable hypogammaglobulinemia: apropos of 3 cases].

Sixty per cent of the patients with immuno-compromise of the humoral compartment may show chronic diarrhoea, specially seen in the common variable hypogammaglobulinemia (CVH). We presented 3 cases of late-onset CVH who referred chronic diarrhoea as the main symptom. The immunodiagnosis revealed undetectable or subnormal immunoglobulins levels (G, M, A), preserved total haemolytic complement and a negative autoimmune screening. The cell compartment analysis showed increased number of suppressor T lymphocytes (CD8) plus hyporesponsiveness of the total peripheral lymphocytes to polyclonal mitogens such PHA. The clinical approach of the CVH patients should be performed by a multispecialized team including the clinical immunologist and the gastrointestinal and infectious diseases specialists.

[Amoxicillin in the treatment of asymptomatic bacteriuria in pregnancy--3g single dose versus 3 times 750mg 4-day therapy].

A prospective, randomized, controlled comparative clinical trial was conducted to assess the efficacy and tolerability of 2 regimens of amoxicillin. In group A a single dose of 3 g amoxicillin and in group B a 4-day course of 3 doses of 750 mg amoxicillin tablets were administered 8-hourly. Significant bacteriuria (greater than or equal to 10(5) cfu/ml midstream urine) and of urine samples obtained by bladder catheterization (greater than or equal to 10(4) cfu/ml) was diagnosed with the dip-slide method (Uricult). 91 pregnant women with a mean gestational age of 25 weeks (14-38) were randomly allocated to both treatment groups. Group A consisted of 53 patients, group B of 38. The treatment groups were comparable in terms of age, duration of pregnancy and additional therapy. Control examinations of urine cultures were taken after 1 and 4 weeks following therapy. The predominant bacterial species was E. coli isolated in 60 to 65 percent. Bacteriological cure rates at 1 and 4 weeks were in group A 77% and 74% and in group B 62% each respectively. The differences in cure rates were not significant. Side effects occurred in group A in 4% and in group B in 13%. Our results suggest that for the treatment of asymptomatic bacteriuria in pregnancy a single dose of 3 g of amoxicillin is equally effective and acceptable as a 4-day course with the advantage of a lower total-dose, lower costs and a better compliance.

[Plasma volumetry with 131I in gestoses of different degrees of severity with dystrophy in children].

Plasma volume was measured with 131I-marked human albumin in 32 cases of gestosis at 35 to 38 weeks gestation. The same tests were made in 4 nonpregnant women. We examined 24 patients suffering from medium to severe gestosis (above 4 points according to Goecke) and 8 patients suffering from minimal gestosis (under 4 points). 4 nonpregnant women exhibited a mean plasma volume of 41.3 ml kg body weight, whereas patients suffering from minimal gestosis exhibited a mean plasma volume of 51.5 ml/kg body weight, in those suffering from medium to severe gestosis we found a volume of 48 ml/kg body weight. There was no significant correlation between the severity of the gestosis and plasma volume reduction, in contrast to birth weight of the infant with regard to gestation period and plasma volume. A low plasma volume in the mother was closely correlated to fetal dystrophy. The sensitivity of the method was 88.8%, specificity 80%.

Disturbed esophageal motility after total gastrectomy.

Esophageal manometry with a catheter microtransducer was performed as a functional diagnostic method on 30 patients after total gastrectomy because of gastric cancer (18 men, 12 women with a mean age of 64 +/- 3.7 years). Subsequently their symptoms were recorded. 21 of the patients (70%) complained of reflux discomfort and symptoms of disturbed peristalsis (dysphagia, odynophagia). 29 patients (93%) showed pathological patterns of contraction (repetitive, simultaneous, deformed, multipeak contractions) especially in the distal part of the esophagus. The contractile force was decreased on average by 10 mmHg in that area. The resting pressure of the upper sphincter was also decreased by about 10 mmHg. These results can be explained by an increased postoperative reflux (absence of the lower sphincter) and the changed biomechanics of the esophagus (decreased longitudinal tension) caused by the operation. The results of this study demonstrate the importance of postoperative manometry in total gastrectomized patients.

[Test battery for objective assessment and differential diagnosis in the early stage of suspected development of presenile dementia of the Alzheimer type].

The clinical diagnosis of organic brain syndromes (especially of primary degenerative dementia) in their early stages is uncertain or only tentative. We used a set of relatively simple tests to support the hypothesis that the disturbance of several operational brain functions precedes memory decline in primary degenerative dementia. Twenty-one patients with early stage, presenile onset of DAT, 14 patients with cardiovascular disease (CVD) and a questionable dementia, an age-matched control group (EC, n = 15), and a younger control group (YC, n = 16) were examined in the following tests: labyrinth learning test, tactile and visual pattern recognition, different reaction time tasks, eye-tracking, finger-tapping, alpha-EEG-blockade, photic driving, flicker fusion frequency, and interaction tasks. The test demonstrated that the patients performed more slowly for all tasks than did control subjects. The CVD patients performed better than the DAT group, but they were also significantly worse in the tests compared with healthy elder subjects. This test battery was found purposeful for early diagnosis of both primary degenerative and other forms of dementia, and may also be helpful to exclude other forms of organic brain syndromes.

Primary carcinoid tumor of the gallbladder.

We present the clinical course of a 77-year old woman who died of tumor cachexia 26.5 months after diagnosis of a primary carcinoid tumor of the gallbladder. To our knowledge, this is the 28th case reported in the world literature. The diagnosis was established coincidentally during histologic work-up of the gallbladder resected for cholecystolithiasis. The entity of this rare tumor type is discussed with regard to the literature.
