[Sclerodermic heart. Report of a case with malignant evolution (author's transl)].

In progressive systemic sclerosis, cardiac involvement, evidenced by histology, is rather unusual and even more rarely it gives clinical manifestations. In our patient cardiac symptoms, characterized by recurrent pulmonary edema as well as by serious conduction defects and arrhythmias, are prevailing and have a rapid and fatal course, whereas the involvement of the skin and of other internal organs is not important. The post-mortem examination gives an evident explanation of the clinical course of the disease, which we can consider as a typical manifestation of sclerodermic heart.

Creatine phosphokinase assay in the progress of delivery.

Serum CPKase activity in 30 women during pregnancy was detected. The maximum increase of activity appeared during the expulsive step of parturition. The CPKase activity after parturition, in the range between the eighth and the twentyfourth hours, drops at the normal level.

[Trials of the diagnostic potentials of TPA in tumorous and nontumorous lung pathologies in 303 cases].

Serum tissue polypeptide antigen (TPA) has been determined in 271 patients with pleural or pulmonary neoplastic diseases (131) or with benign lung diseases (140). Using a cut-off of 95 U/L, TPA has a sensitivity and specificity of 65%. Therefore, TPA serum determination can suggest a diagnosis of malignancy, but its evaluation, as a single test, is not useful to differentiate between malignant or benign disease. TPA can be useful as an ancillary test in the follow-up of lung cancer therapy.

Relationship between urine acidification and intracellular pH in respiratory acidosis.

The renal net acid excretion (NAE), the blood pH (pHe), the total body intracellular pH (pHi) and the urinary pH (pHu) were calculated in 10 patients with chronic obstructive lung disease and hypercapnia and in 5 normocapnic subjects. The mean value of NAE was significantly higher in hypercapnic subjects than in normocapnic ones. pHe was significantly lower in hypercapnic than in normocapnic subjects. The differences of pHi and pHu between normo and hypercapnic subjects were not significant. NAE is significantly correlated with PaCO2, pHe, pHu and pHi in all the subjects considered together. H+-secretion probably depends on the H+-availability and pHi of tubular cells, but from our results it is not possible to confirm this relationship, because the method used for pHi is fundamentally a measure of muscle-pHi.
