[The relationship between the aortic pulse wave velocity and osteoporosis in elderly women].

The relationship between arteriosclerosis and osteoporosis (OP) was assessed in 20 elderly women. The aortic pulse wave velocity (PWV) was used as the index of arteriosclerosis and bone mineral content (BM) of the vertebral body, which was measured by quantitative computed tomography, was used as an index of OP. PWV and BM showed a significant correlation with aging (PWV and age: r = 0.466, p less than 0.05, BM and age: r = -0.487, p less than 0.05). In the group in which the serum alkaline phosphatase (ALP) was higher than 180 IU/l (n = 11), there was a significant inverse correlation between PWV and BM (r = -0.728, p less than 0.02). On the other hand, there was no correlation between PWV and BM in the group in which ALP was lower than 180 IU/l (n = 9). These results suggest that in cases with OP and high levels of ALP, calcium (Ca) seems to be released from the bone system and transferred to the wall of the aorta. The mechanism and pathogenesis of this Ca transference is unknown and should be investigated.

[A case of aortitis syndrome complicated with amyloidosis, type AA].

We recently saw a patient who had aortitis syndrome associated with secondary amyloidosis. To our knowledge, she is the fourth report of this complication occurring in aortitis syndrome. In November 1985, the patient, a 18 year-old woman, was admitted to our hospital because of a high fever, back pain, abdominal pain and general fatigue. On physical examination, bruit was audible on the abdomen, bilateral radial artery was weakly palpable. Angiography showed the stenosis of bilateral carotid artery, subclavian artery, renal artery and superior mesenteric artery. From the above findings, she was diagnosed aortitis syndrome, and treatment was begun with prednisolone. However, she developed recurrently a high fever, chest pain, abdominal pain and exertional dyspnea. Laboratory findings at the active stage revealed the marked elevation of leukocytes, erythrocyte sedimentation rate and C-reactive protein. On her clinical course, the number of circulating thrombocytes was paralleled with the activity of the disease. On June 1988, she developed suddenly a high fever and severe pain of abdomen. Pathological findings of her stomach showed the deposition of amyloid protein A. Laboratory findings depicted the marked increment of thrombocytes, beta-thromboglobulin and platelet factor 4. These results suggest that circulating thrombocytes may play a role in product ion of amyloid protein.

[The relationship of osteoporosis to mitral annular and aortic valvular calcification in elderly women].

Mitral annular calcification (MAC) and aortic valve calcification (AVC) are thought to be attributable to the aging process, and their incidences are especially high in elderly women. It is known that the incidence of osteoporosis (OP) is also relatively high in elderly women. Therefore, we investigated the relationship of OP to MAC and AVC. The study subjects were 39 women aged 70-89 years. MAC and AVC were investigated by echocardiography. The amount of bone mineral (BM) in the lumbar vertebral bone was measured by computed tomography using a bone phantom and the standard amount of CaCO3. MAC and AVC were detected in 14 (35.9%) and 19 (48.7%) of the 39 subjects, respectively, and there was a tendency of coexistence of MAC and AVC. There was no significant age difference between patients with and without MAC. The amount of BM was significantly lower in patients with MAC than in patients without it (27.1 +/- 8.2 vs 53.5 +/- 27.3 mg/cm3: p less than 0.01). There was no correlation between the amounts of BM and AVC. In conclusion, the amount of BM is clearly lower in elderly women with MAC, suggesting that MAC may be related to OP etiologically. However, there was no clear relationship between AVC and OP. Thus, the etiologies of AVC and MAC may be different.

[A case of spongiform encephalopathy with ataxia and amyloid plaques].

CASE: A 63-year-old man was admitted to our hospital because of gait disturbance. His daughter died of the same disorder at age 29. In 1974, at age 59, the patient noticed mild recent memory disturbance and clumsiness in handwriting. In 1976, his gait was markedly unstable, but he could walk without assistance, and his speech became dysarthric. In spring, 1977, he was unable to walk without assistance. From Jan. 9, 1978, through Feb. 14, 1978, he was hospitalized in our hospital. Neurological examination revealed an ataxic gait and scanning speech. Deep tendon reflexes of the upper extremities and ankle jerks were normal, but knee jerks were absent. Pathological reflexes were not elicited. Both superficial and deep sensations were normal. He was alert and showed no overt dementia. Laboratory data including CSF, EEG and brain CT were normal except positive TPHA in serum. Thereafter, he was followed up with the diagnosis of spinocerebellar degeneration. In 1979, he developed a limb ataxia. In spring, 1980, he became very irritable and was easily excited. He gradually developed dementia and urinary incontinence. In 1981, his illness progressed to an akinetic mutism and died of pneumonia on July 19, 1981. Myoclonus and periodic synchronous discharges in EEG were not observed until his death. The brain was 1,075 g in weight after formalin fixation and the cerebrum was generally soft. The gyri showed no evidence of abnormalities or atrophy, while the cerebellum and brain stem, especially the pons, were atrophic. The arterial and venous systems showed no remarkable changes. The spinal cord was externally unremarkable. The ventricles were dilated.(ABSTRACT TRUNCATED AT 250 WORDS)

EEG correlation with intelligence test performance in senescence: a new pattern discriminative approach.

Two hundred and seventeen geriatric EEGs were quantitatively analyzed in relation to their intelligence test performances. The subjects were divided into three graded intellectual levels according to their test score in the three diagnostic categories classified with respect to hemiplegia. By means of a novel method using auto-regressive analysis and multivariate analysis, the mean spectrum patterns were numerically compared and classified in terms of general pattern similarities. As the results were in good accord with their intelligence test performances, it was suggested that there were proportional changes of EEG along with the intellectual deterioration. In that occasion, changes in frequency and dumping characters of alpha activity made the most significant contribution. These results suggest that an assessment of intellectual level might be possible through the EEG examination applied in this study and that a prediction of prognosis might also be expected.
