The results of the analgesic block of the lower extremity by means of an anterior (150 patients) or a posterior (114 patients) approach to the sciatic nerve, associated to a "3 in 1 block" were compared. The anterior approach technique was associated with a higher incidence of failures, insufficient analgesia and hence a higher demand for intraoperative analgesic and sedative drugs. Also tolerance to a pneumatic tourniquet over the proximal thigh was less than with the posterior approach. However, the sciatic nerve block by anterior approach granted a more prolonged analgesia. This technique was suitable for trauma patients immobilized in the supine position, for patients with skeletal traction on Zupinger frame, both for surgery and for closed reduction of lower extremity fractures.
Thiopentone doses corresponding to 5.238 mg x kg-1, that is 2UD95 (UD95:unconsciousness dose 95) inhibit the intensity of succinylcholine-induced fasciculations compared with UD95. Alfathesin doses corresponding to 0.570 mg x kg-1, also 2UD95 (calculated as alphaxalone at a concentration of 9 mg/ml in alfathesin), do not have any greater effect on the degree of fasciculations compared to UD95. Furthermore, neither induction agent influences the incidence, distribution and duration of succinylcholine-induced myalgias. The inhibition of the degree of fasciculations caused by thiopentone is believed to be a consequence of a postsynaptic depressant effect of thiopentone at the neuromuscular junction. The evaluation of the degree of succinylcholine-induced fasciculations must take into consideration the dose of thiopentone administered at induction of anaesthesia.
The authors studied the effects of acupuncture and pentazocine on post-operative respiratory function and pain management on patients hysterectomized by means of a subumbilical midline incision. The acupunctural technique consisted of GB-26, St-36, Sp-6 and auricular Shen-Men bilateral electrostimulation for 40 minutes. The analgesic effect of acupuncture was equivalent to that of 30 mg pentazocine, yet the most important effect of acupuncture consisted in a net increase of vital capacity during the period of acupuncture analgesia that lasted for 3-4 hr after stimulation; contrariwise, pentazocine did not cause any vital capacity increment and vital capacity remained at the levels observed prior to narcotic administration.
The Authors have investigated the antihallucinogenic and sedative effects of auricular points R and shen menn during surgery under ketamine anesthesia. The results demonstrate that the insertion of the needle in point R is very efficient in reducing hallucinations of ketamine emergence, while the insertion of needle in auricular point shen menn causes only a brief period of sedation in the beginning of the emergence period. The acupunctural technique employed by the Authors has been shown to increase considerably approbation of ketamine anesthesia.

The Authors report their experience with the placebo, an essential step in the clinical study of reflexotherapy. They assert that also in this branch of medicine it is possible to carry out a "double blind" clinical experimentation by means of placebo, that is, an essential condition in order to develop a scientific assessment of both the limits and the potentialities of reflexotherapy.
The authors reported 5 cases of post-operative cervicalgia of spondylodiscitic nature. Authors deem that this severe complication is to be attributed to the three following factors: microtraumatisms of cervical rachis with consequent local microcirculatory troubles; bacteremia provoked by anesthesiological manipulation (intubation, tracheal or oropharyngeal aspiration) or by the surgical operation and decrease in the immune response caused by operation and by used anesthetic drugs. Severe radiological alterations of the structure of cervical vertebrae and of the intersomatic space appear around 2-3 months after the surgical operation. They recover 8-12 months after the operation. They are preceded and accompanied by clinical signs and symptoms that lead the patient to go for several specialists' visits and subsequent hospitalizations. The authors think that the identification of cervicalgia of spondylodiscitic nature must lead the anesthetist to take necessary measures i.e. to immobilize the neck and to administer antibiotics.
Interference on the part of premedication with diazepam and trazodone with althesin anaesthesia was investigated and compared by means of variance analysis according to Fisher, and the X2c test according to Yates, in 150 patients aged 35-45 yr in three groups of 50 during examination of the uterine cavity. No marked difference between the effects of the two drugs were noted, with two exceptions: 1) trazodone was better able to potentiate the hypnotic action of althesin, so that less was needed for the induction of anaesthesia; 2) patients pretreated wih diazepam displayed a marked increase in postoperative somnolence, this being an expression of the drug's hypnotic effect.
The effects of Etomidate, a new intravenous anaesthetic agent, were studied in 138 cases which underwent different procedures of variable duration under general anaesthesia. The effects on different organs and systems of this drug, alone or in association with other anaesthetics or adjuvants, were analyzed in this series. Clinical signs, plethysmography and electroencephalography were the main parameters studied. It was shown that Etomidate has a rapid, transient action and a high therapeutic index without any cumulative effects. In conclusion this drug is recommended as an excellent induction agent which could also be advantageously employed for short surgical procedures.
