Computed tomography for the evaluation of knife impalement injuries: case report.

A patient with a stab wound to the back with suspected major vascular injury was evaluated by computed tomography instead of arteriography. The computed tomographic (CT) scan demonstrated no injury to the aorta, inferior vena cava, or esophagus, which were adjacent to the blade, and excluded a hematoma. The information from the CT scan made possible a less extensive surgical procedure than might have otherwise been necessary.

Paranasal sinuses: low-dose CT.

While computed tomography (CT) has become an important imaging modality in the evaluation of the paranasal sinuses, the radiation dose remains higher than is necessary. With use of a head phantom and constant kilovolt peak setting, axial and coronal CT scans of the paranasal sinuses were obtained at each of six successively lower milliampere second settings than are commonly used in clinical practice. Although noise, as measured by the standard deviation of the CT numbers, did increase, images were of diagnostic quality even when dose levels were reduced by a factor of 28. In the same incremental manner, the milliampere second settings used in scanning 90 patients were reduced, with no loss of diagnostic quality. The authors discuss the methods of analysis and the advantages of use of lower milliampere second settings at CT scanning of the sinuses.

Endoscopic placement of a Foley catheter across a stricture and rectovaginal fistula to perform a barium enema.

Evaluation of enteric fistulas is often best performed with barium contrast studies. Clinical situations that preclude the satisfactory installation of barium decrease the yield of the study. A case is presented of a 59-yr-old female with stage III-B cervical carcinoma and a known rectovaginal fistula with an adjacent sigmoid colon stricture. An additional more proximal, enteric fistula was suspected. Definitive preoperative knowledge of the existence or absence of this fistula would have shortened the duration of a planned palliative intervention. The rectovaginal fistula and stricture precluded adequate barium and air installation for contrast study. Colonoscopy was unsuccessful. We have developed a combined technique in which the endoscopic placement of a semirigid guidewire allowed placement of a Foley catheter across the fistula and stricture. Successful barium study was performed. Intraoperative time and hospital stay were shortened.

Unusual computed tomography findings in hepatic amyloidosis.

An unusual pattern of delayed enhancement in a patient with a focal hepatic defect due to amyloidosis is presented. Without the aid of region-of-interest density measurements, the lesion was not appreciated on unenhanced computed tomography scans.

Acalculus lymphoeosinophilic cholecystitis associated with interleukin-2 and lymphokine-activated killer cell therapy.

A case of unusual cholecystitis that developed on completion of interleukin-2 and lymphokine-activated killer cell therapy is described. A 62-year-old man was treated with interleukin-2 and lymphokine-activated killer cells for disseminated renal cell carcinoma. During the course of the immunotherapy, his serum alkaline phosphatase level increased, as did the peripheral eosinophil count (0.31). Subsequently, clinical and radiologic evidence of acute cholecystitis was noted. The removed gallbladder showed acalculus cholecystitis with extensive diffuse infiltrates of numerous eosinophils and T lymphocytes, but sparse polymorphonuclear leukocytes. The authors name this unusual cholecystitis acalculus lymphoeosinophilic cholecystitis and believe it to be associated with interleukin-2 and lymphokine-activated killer cell therapy. The pathogenic relationship is discussed.

Role of abdominal CT, when available in patients' records, in the evaluation of degenerative changes of the sacroiliac joints.

To determine the role of abdominal computed tomography (CT) that had been performed for other reasons, in the evaluation of degenerative changes of the sacroiliac joints, the authors performed a retrospective review of the sacroiliac joints of 100 patients, all of whom had an abdominal CT scan and a plain abdominal and/or pelvic roentgenogram in their files. The results indicate that, when available in the patient's record, abdominal CT scans provide substantial additional information in the evaluation of degenerative changes of the sacroiliac joints at no extra cost. The authors' results also suggest that after the age of 55, the patient's age does not seem to influence the extent of sacroiliac joint narrowing, which does appear to be affected by the presence of coexisting diffuse idiopathic skeletal hyperostosis.

Liposuction fat-fillant implant for breast augmentation and reconstruction.

The perfect breast implant fillant material would have higher viscosity than water and would be autologous and harmless. We describe the confinement of liposuction fat in implants using the Lipovacutainer during a routine liposuction procedure. This collected fat is prepared inside the Lipovacutainer and is reinjected through a Lipomedia filling cannula into a leaf valve implant as the fillant in place of saline. The implants are used for bilateral augmentation mammoplasty and breast reconstruction procedures. Our six clinical cases have been monitored closely using mammography and MRI. These cases showed slow liquefaction without interference with mammography studies. We obtained excellent overall body contours. All complications were correctable and non-life-threatening and there was no capsule formation.

CT of pseudohydronephrosis: hydroureter in a weight lifter.

A case of bilateral pseudohydronephrosis and hydroureter produced by markedly hypertrophied psoas muscles in a weight lifter is presented. Plain film and CT findings of the case are discussed together with the topographic anatomical explanation of the radiologic findings.
