Distribution of tumor necrosis factor alleles (NcoI RFLP) and their relationship to HLA haplotypes in an Italian population.

The NcoI RFLP of the tumor necrosis factor (TNF) beta gene was analyzed in a panel of 105 unrelated healthy Italian blood donors. The gene frequencies of the 10.5 kb and 5.5 kb allele were 0.73 and 0.27, respectively. The 5.5 kb band was significantly positively associated with HLA-A1, B8, DR17.1, and C4AQ0, and negatively associated with DR7.2, DQw9 and C4A6, all being specificities which belong to two well-known Caucasoid ancestral haplotypes. When the population was subdivided on the basis of TNF phenotypes, different linkage disequilibria between HLA alleles were detected in the three phenotypic classes. From this analysis it was possible to relate preferential HLA associations, most of which are characteristic of ancestral haplotypes, to TNF polymorphism.

Clinical and haemodynamic effects of ketanserin in lean and obese hypertensive patients.

Systemic and central haemodynamics were evaluated in 10 lean and 10 obese hypertensive patients (World Health Organization stage I-II) after treatment for 8 weeks with a serotoninergic antagonist, such as ketanserin. Blood pressure and heart rate were recorded and first-pass radionuclide angiocardiography was performed to determine cardiac output, cardiac index and ejection fraction of the left ventricle; total peripheral resistance was also calculated. In both obese and lean patients, ketanserin significantly reduced diastolic (P less than 0.05) and mean (P less than 0.005) blood pressure but no significant changes in systolic blood pressure, cardiac output, cardiac index and ejection fraction were observed in lean and obese hypertensive patients. Total peripheral resistance was significantly (P less than 0.05) reduced in lean patients but in obese hypertensives it was only moderately reduced. It is concluded that monotherapy with ketanserin is effective in treating mild to moderate hypertension in both lean and obese hypertensive patients, without interfering with left ventricular performance.

Thyroid function and release of thyroid-stimulating hormone and prolactin from the pituitary in human obesity.

Thyroid function, basal serum thyroid-stimulating hormone (TSH) and prolactin concentrations, and the effects of 200 micrograms TSH-releasing hormone (TRH) given intravenously on TSH (delta TSH) and prolactin (delta prolactin) were investigated in 25 euthyroid obese subjects and 20 lean controls. No significant differences in serum thyroid hormone concentrations, glucose metabolism parameters, or basal TSH and prolactin concentrations were detected between groups, but a significant (P less than 0.01) increase in delta TSH and a significant (P less than 0.01) decrease in delta prolactin were observed in obese subjects. No significant differences in basal TSH and prolactin were observed in obese men and women compared with corresponding lean controls, but delta prolactin decreased significantly (P less than 0.01) in obese men and women compared with lean controls and TSH increased significantly (P less than 0.01) in obese men and women. No correlation was found between delta TSH or delta prolactin and body mass index. The study suggests that hyper-responsiveness of TSH and hyporesponsiveness of prolactin to TRH in obese subjects can be related to changes in the central serotoninergic system.

Cardiac and pulmonary effects of alpha tumor necrosis factor administered by isolation perfusion.

AIMS: We studied cardiac and pulmonary function in 22 patients affected by in transit metastases from cutaneous melanoma and metastases from soft tissue sarcoma of the limbs and treated with isolation perfusion in extracorporeal circulation with rTNF alpha at doses ranging from 0.5 to 4 mg/m2 in mild hyperthermia. PATIENTS AND METHODS: All patients experienced a septic-like shock syndrome of variable severity: this feature lasted from 24 to 72 h and was controlled by the infusion of dopamine. Seventeen patients suffered from respiratory insufficiency, which required assisted ventilation (7 cases mechanical ventilation for 1 day, 8 cases for 2 days, and 2 cases synchronized intermittent mandatory ventilation). RESULTS: Spirometric parameters recorded 7-15 days after treatment did not change from baseline values. In contrast, lung transfer factor for carbon monoxide significantly declined in a dose dependent fashion. CONCLUSIONS: Our data confirm that rTNF alpha administered by isolation perfusion technique induces systemic cardiovascular and pulmonary side effects. Further studies are required to better define time course and reversibility of impaired pulmonary function.

Historical recombinant sites in extended HLA haplotypes.

Each ancestral or extended HLA haplotype contains a unique combination of alleles among which some may be entirely specific for that haplotype (haplospecific alleles). In the course of evolution many recombination events occurred which disrupted the original haplotypic combination. We analysed the sites of historical recombinations in four extended HLA haplotypes (B8-DR3; B18-DR3; B50-DR7 and B57-DR7) in 60 random Italian individuals selected through the presence of haplospecific alleles. In general the distribution of recombinations in each interval was similar for the four extended haplotypes and no haplospecific recombination "hot spot" could be detected. However some differences between the four haplotypes can be pointed out: a) only 48% fragmented B8-DR3 were found in contrast to 83% B18-DR3, 89% B50-DR7 and 88% B57-DR7; b) in the B8-DR3 haplotype recombinations fall preferentially in the B/TNF interval. In fact among 22 historical recombination events, 50% were mapped in this region; c) conversely, no recombination event was detected in the B/TNF interval among the 19 disrupted B18-DR3 haplotypes thus evidencing the presence of a putative recombination "cold spot".

HLA class II gene frequencies in Italy.

The frequency of HLA alleles at HLA-DR and DQ loci, and that of the related HLA-D specificities, were estimated in the Italian population. 109 healthy unrelated subjects, born in several Italian regions and living in the district of Torino, were studied. DNA typing was achieved by the restriction fragment length polymorphism (RFLP) analysis of HLA-DR beta, DQ alpha and beta genes, hybridizing specific probes with TaqI digested DNAs. The present study allowed to define in more detail the HLA class II polymorphisms in the Italian population.
