Dreams, images and emotions associated with propofol anaesthesia.

One hundred and twelve patients scheduled for day case varicose vein surgery were randomly allocated to one of three groups: total intravenous anaesthesia with propofol, propofol induction followed by inhalational anaesthesia with nitrous oxide and isoflurane or thiopentone induction followed by inhalational anaesthesia with nitrous oxide and isoflurane. Assessments were made in the recovery room of the incidence of dreaming, the content of the dreams and the emotional status of the patients. The groups differed significantly in reporting that they had been dreaming: patients who underwent total intravenous anaesthesia reported the most dreaming and patients who received thiopentone the least. However, despite the large number of case reports of sexual imagery following propofol anaesthesia and despite the two groups who had received propofol experiencing significantly greater happiness upon recovery than the thiopentone group, there were no appreciable differences in the sexual content of the dreams. Each group had only a small number of dreams even remotely related to sex.

Use of the Family Adaptability and Cohesion Evaluation Scales in child clinical research.

Although the FACES has become a widely used instrument for assessing children's family relations, the possible linearity vs. curvilinearity of its cohesion and adaptability scales has been treated inconsistently by investigators. This study evaluated whether samples of adolescent repeat offenders, young adult prisoners, and adolescent nonoffenders were discriminated better by a linear or curvilinear treatment of the FACES scores. Between-groups comparisons showed that significant effects were observed for each of the three curvilinear measures (cohesion-curvilinear, adaptability-curvilinear, distance-from-center) and only one of the two linear measures (cohesion-linear). These findings support the superiority of a curvilinear treatment of the FACES. In light of these findings, it is suggested that investigators who use FACES evaluate the linearity of the scales and determine whether a linear or curvilinear treatment of the data produces more meaningful results.

Mortality associated with mode of presentation in the acquired immune deficiency syndrome.

A population-based survival study was done for all cases of the acquired immune deficiency syndrome diagnosed in the city of San Francisco through May 1983. Follow-up was obtained for 165 of 173 diagnosed cases. Median survival among 75 patients presenting with Kaposi's sarcoma (KS) alone was 21 months. Median survival among 90 patients presenting with opportunistic infections, primarily Pneumocystis carinii pneumonia, was 9 months; survival at 21 months was zero. Survival among patients presenting with both KS and opportunistic infections was not statistically different from survival among patients presenting with opportunistic infections only. When cases were divided into those diagnosed before and after May 1982, there was no significant improvement in survival from diagnosis in the more recently diagnosed cohort.

A trial of bran and bran biscuits for constipation in mentally handicapped and psychogeriatric patients.

Six elderly psychiatric and 17 mentally handicapped individuals suffering from severe constipation received 3 week periods of treatment with 10 g unrefined bran, three bran biscuits, and Senokot syrup daily or three times weekly. There was no significant difference in the number of bowel movements or enemas or in the consistency of the motions. The results suggest that bran may be substituted for laxatives.
