Time for action: a new system for training mental health practitioners.

Continuing the debate from previous issues of Mental Health Care, MICK McKEOWN and colleagues call for a radical rethink in mental health training. Institutionalised neglect continues on hospital wards and in the community. The answer is a systematic approach to mental health care. Training should focus on psychosocial interventions which are known to be effective. It should take place in the work setting, to ensure developments are integrated into the routine practice of all members of the mental health team.

Understanding the needs of relatives of patients within a special hospital for mentally disordered offenders: a basis for improved services.

This paper outlines one component of a major research study, which will form the basis for a new service for relatives of schizophrenia sufferers within a special hospital. The aim of this initial component has been twofold: firstly to identify and understand the needs of relatives in terms of their present knowledge and understanding of schizophrenia; and secondly to establish what stress the relatives are presently experiencing in maintaining contact with patients and whether this stress may have an impact on the management of schizophrenia at the hospital. A purposive sample of 17 relatives were interviewed using two semi-structured interview protocols. All interviews were audio-taped and later transcribed for analysis. The data have provided an illuminating insight into the thoughts and feelings of relatives and indicate many ways in which a service for relatives could be tailored. It is apparent that many relatives are currently not being supported, involved or allowed to contribute to the care of their family member at Ashworth Hospital. This present situation is stressful for relatives and is likely to have a detrimental effect on the course of schizophrenia in patients at Ashworth Hospital.

Clinical management: a special case*.

This paper briefly discusses the development of some of the principles of case management as they have been applied in the community. As a clinical management system we argue that the key elements of case management could and should be applied within institutional settings. We draw as an example Ashworth Special Hospital recently criticized for its lack of co-ordinated care by a Public Inquiry Committee. The resulting discussion focuses upon a rehabilitation framework which could be used to implement case management within such a setting; the implementation of such a framework and the benefits that would be gained.

Cardiorespiratory and symptomatic variables during maximal and submaximal exercise in men with stable effort angina: a comparison of atenolol and celiprolol.

Celiprolol is a novel beta 1 selective adrenoreceptor blocker with partial beta 2 agonism and direct vasodilator activity. These ancillary properties may reduce symptomatic breathlessness and fatigue and modify respiration during exercise. To test this hypothesis 20 men with stable effort angina were enrolled in a double-blind crossover study to investigate the effects of atenolol 100 mg once daily (A) and celiprolol 400 mg once daily (C) on cardiorespiratory and symptomatic variables during maximal and submaximal exercise. Total exercise time on a modified Bruce protocol was similar on both treatments: C12.5 min, A 13.1 min. During steady state submaximal exercise at 60-75% (mean 68%) of maximum work capacity, minute ventilation (C33.81 min-1, A 33.51 min-1), oxygen uptake (C14.6 ml.kg-1.min-1, A15.1 ml.kg-1.min-1), respiratory exchange ratio (C 0.89, A 0.87), ratio of VE/VCO2 (C 33.6, A 33.4), ratio of VE/VO2 (C 2.34, A 2.72), Borg perceived exertion score (C 11.2, A 10.9) and visual analogue scores for breathlessness (C 29.5, A 25.9) and muscle fatigue (C 28.9, A 26.0) were all similar on both treatments. At maximal exercise capacity on the modified Bruce protocol, minute ventilation (C 58.31 min-1, A 60.41 min-1), oxygen uptake (C 21.3 ml.kg-1.min-1, A 21.7 ml.kg-1.min-1), respiratory exchange ratio (C 1.02, A.1.05), ratio VE/VCO2 (C 34.8, A 35.9), and ratio VE/VO2 (C 2.80, A 2.83) were also similar on both drugs. Over a 10 day period anginal attacks (C 10.1 +/- 10.4, A 5.4 +/- 5.9) and sublingual GTN use (C 5.9 +/- 10.3, A 4.4 +/- 9.8) were both more frequent on celiprolol).(ABSTRACT TRUNCATED AT 250 WORDS)

Relatives' support groups in a special hospital: an evaluation study.

A support group for the relatives and friends of patients at a special hospital in Merseyside, England, has been in operation for over 1 year. A detailed evaluation of that group has been completed based on the minutes of each meeting and on a structured questionnaire sent out to each relative/friend involved. The evaluation provides an illuminating account of the particular needs of this group, highlighting many of the concerns expressed by the Committee of Inquiry during the recent Public Inquiry at Ashworth Hospital. The results of the evaluation are discussed in relation to work already completed on network support and mental health, as well as the recent work involving the relatives of patients in the community. It provides a direction for future work and research within the special hospitals.

A microstereotactic approach to small CNS lesions. Part I. Development of CT localization and 3-D reconstruction techniques.

The authors describe a newly designed and utilized stereotactic methodology for the removal of central nervous system lesions as small as a few millimeters in diameter. These small lesions are detected and localized by non-invasive computerized axial tomography (GE 8800 scanner) with additional computer processing of the digital data by means of a PDP-1145 computer. Multiple computer algorithms have been developed to enhance regions of interest on CT scans by three-dimensional reconstruction and magnification techniques. This same data can then be used to calculate a stereotactic approach to a small CNS lesion. The stereotactic approach coordinates are then mated to a head fixation system modified from the Reichert-Mundinger stereotactic apparatus enabling information transfer from CT scan to the stereotactic surgical system. These small CNS lesions, as small as 5 mm, can be removed with the apparatus described herein under direct binocular 3-D vision with minimal tissue damage, through a small trephine craniotomy or burr hole. Newly designed instruments and instruments in the process of being developed are mounted on a micromanipulator attached to the Riecher-Mundinger frame for guidance at surgery. These new instruments include stereoendoscopes with xenon arc illumination, multiple tissue expanders for exposing the operative site, a radiation tracer probe, a rotary-sucker extractor, and multiple other small instruments for operating and removal of blood and tissue from small CNS lesions. The stereotactic frame accurately defines all areas of the cranium in three-dimensional coordinates, and its combination with the micromanipulator-instrument assembly enables the site of any small CNS lesion, the three-dimensional coordinates of which have been located by additional computer processing of the CT digital data, to be accurately approached by the stereotactic guide micromanipulator assembly. The instrument described herein allows removal of very small tumor burdens, and opens the possibility for successful secondary application of adjuvant immunotherapy to a CNS tumor site as described in part II of this paper.

Identifying the needs of relatives of forensic patients.

This paper describes a research project recently undertaken in a special hospital. The focus is on evaluating the needs of relatives of forensic patients, in particular, the problems resulting from the offence and a diagnosis of schizophrenia. The developmental and implementation phases of the project are outlined, as are the interview schedules used. The discussion of the results that follows raises important implications for services and clinical practice.
