[The treatment of heel skin defects using a medial plantar skin flap].

The instep neurovascular flap, based on the medial plantar artery and innervated by the medial plantar nerve, is in our opinion the treatment of choice for resurfacing heel skin defects. The flap is usually raised as an island flap in the non weight-bearing surface of the foot. It provides heel cover with thick skin combined with a perfectly adapted texture. Conservation of sensibility offers a protective advantage. Four patients, treated with the flap, are discussed.

[Reconstruction of the retracted eye socket].

The various methods of treatment of the contracted eyesocket are discussed according to the severity of the disease and are illustrated by examples of treated patients. A method to keep the newly grafted cavity expanded by means of a conformer, stabilised by external fixation to the frontal bone, is described.

[Zygomatic fractures].

In this article, the authors relate their experience in the management of zygomatic fractures, based on 168 cases treated in the Plastic Surgery Unit of Bruges between January 1971 and August 1980. The symptomatology and treatment of those fractures are discussed according to the type of fracture. The importance of the early treatment and the necessity of osteosynthesis when the fronto-zygomatic suture is distracted are stressed. Multifragmented fractures are treated in the same way by means of osteosynthesis. The use of a silastic sheet for blow-out floor fractures is advocated.

[Functional value of the hand after digital replantation (author's transl)].

The absolute and relative indications and contraindications of fingerreplantation are exposed. The function of the replanted fingers depends on the level of amputation, the bone-joint and tendon lesions and the presence of protective sensibility. The range of joint movement of the replanted fingers is related to the amputation wound. The residual motion depends also on the level of amputation giving better results as the amputation is more distally situated. Limited range of movement in multiple amputations is not an obstacle to a useful grasp and pinch, which results in a more or less functional hand. The importance of sensory recovery in the use of replanted fingers is stressed. Assessment of the results is made upon the two-point discrimination rating, the range of motion of the fingers, the grip strength, and stereognosis, the absence of cold intolerance and return to employment.

[Microsurgery of the facial nerve (author's transl)].

The extent and importance of facial paralysis is established by means of the diagnosis of the etiologic factors, the level of the nerve lesions and the condition of the facial muscles. Treatment depends on the analysis of these different elements. A survey of the methods of treatment is given with special reference to the cross-face nerve grafting. The results of the varied methods are reviewed. The substitutional procedures are listed.

Cross-face nerve grafting in facial paralysis.

Facial paralysis causes a most disturbing deformity with functional, emotional and social consequences for the people afflicted. Since a few years most promising advances in the treatment of facial palsy have been developed and new ways of surgical management have been introduced. Cross-face nerve grafting connects the non-paralysed facial nerve branches to the paralyzed ones and by these means realizes a reanimation of the paralyzed side of the face. Fourteen cases have been operated according to this technique. The results of 10 cases with a sufficient follow-up are analysed. Good and satisfactory results were obtained in 8 cases. Two cases were considered bad with poor recovery of facial muscle activity. The overall results of cross-face nerve grafting are very gratifying. The technique should always be attempted in first instance. Other substitutional methods for further improvement should be reserved for later.

[Functional value of replanted fingers (author's transl)].

The indications of the replantation of amputated digits are listed. The functional value of 38 replanted fingers is analysed according to the level of amputation and also to the mobility of the digital joints and to tactile sensitivity. Analysis of the residual function shows that the results are related to the level of amputation; the more distal the amputations the better the results. Limited motion of the thumb and the fingers in cases of multiple amputation is not a contraindication to replantation as all patients regained a useful pinch. Almost always protective sensation has been achieved.
