[Diagnostic problems of extrapulmonary tuberculosis in a surgical infected patient sample].

Reported in this paper are 20 cases of extrapulmonary tuberculosis, following septic surgery, between 1983 and 1987. It seems to be as important as ever before to consider tuberculosis for differential diagnosis of abscesses and fistulae. Treatment was rendered to patients with osteoarticular, renal, and lymph node tuberculosis forms. Tuberculosis should be considered as a possibility particularly in the context of periarticular accumulation of pus and therapy-resistant fistulae. The use of pus cultures from tuberculotic abscesses is considered to be a promising approach to bacteriological verification. However, in cases of poly-infected fistulae, bacteriological verification was obtainable solely from animal experiments.

[Diagnostic value of various tuberculin preparations].

Despite declining incidence of tuberculosis in developed countries importance of tuberculin testing has not subsided. Previous investigations of the equivalence of 7 tuberculin preparations from 6 countries to RT 23 have shown differences in the diagnostic value of the tuberculin preparations. The smallest rates of negative skin-reactions in cases with bacteriologically confirmed tuberculosis were found with the hungarian tuberculin PPD-5 TU (HU 5 TU) and RT 23. The national tuberculin PPD Dessau 2 TU showed highest rates of non-reactors in tuberculous patients. In 400 cases of active bacteriologically confirmed tuberculosis further investigations were carried out to compare biologic equivalence of RT 23, HU 5 TU, tuberculin PPD Dessau 2 TU and the test-preparation PPD Dessau 5 TU with and without Tween addition and 7.5 TU without Tween. The results showed that with increase of antigen concentration to 5 TE or 7.5 TU the smaller reactions of 6 to 10 mm could be strengthened with a small, statistically not significant, increase of the mean value of reaction size but no increase of larger reactions of more than 20 mm. The rate of false negative reactions decreased using the test-preparation PPD 7.5 TU. The results allow to recommend the production of a national tuberculin preparation with higher antigen concentration biologically equivalent to RT 23 or HU 5 TU.

[Investigations on the equivalent of different tuberculin preparations].

Report on the comparison of the effectiveness of seven tuberculin preparations from 6 countries to RT 23. In 2 field trials performed among tuberculous patients with confirmed excretion of tubercle bacilli significant differences have been found among the preparations when their effectiveness and specificity was determined expressed in terms of the rate of negative (0-5 mm) reactions and the size of reaction and its range of variation. Only three of the seven preparations were equivalent to RT 23, one of them was even superior to it; another of the equivalent preparations originated from the stock substance of RT 23. However, one among the deviating preparations originated from RT 23, too, having a threefold higher rate of negative tuberculin reactions. Investigations in the USA with tuberculin preparations from this country done in time parallel to those reported here, had a similar result and the same conclusions. There are set limits to the comparison of testing results with tuberculins of different production. Thus, the comparison of incidence and prevalence of tuberculous infection obtained with different tuberculins must be met only with caution.

[Comparison of the efficiency of the Soviet culture medium no. 3 and the media used for cultivation of mycobacteria in routine in the GDR (author's transl)].

In cooperation of ten laboratories the Soviet culture medium Mo. 3 (FINN) was compared with the media used in the GDR for primary cultivation of mycobacteria in routine (German Pharmacopoeia (diagnostic laboratory methods) GDR). The efficiency of the media tested in the study was nearly the same, not only concerning the number of positive cultures but also in time of growth and the number of colonies.
