Correlation between steroid hormone receptors and prognostic factors in human breast cancer.

Knowledge of the tumor content of estrogen (ER) and progesterone (PgR) receptors has proved to be of significant value in human breast cancer. Relative determinations were performed in 589 specimens in our laboratory. The positivity of ER and PgR is correlated with the patients' age at diagnosis, tumor size and relative grade. In particular, the significance of PgR versus ER status and the possible prognostic role of these receptors are investigated.

Primary tumor site, size, patients age and axillary lymph nodes in breast cancers.

This study considers the correlations between some characteristics of the primary tumor and level of lymph node involvement in 185 primary breast cancers. The average number of lymph nodes was higher in N+ women than in N- women. Primary tumors with a diameter of more than 4 cm yielded the highest mean number of lymph nodes (17.5). The risk of developing lymph node metastases was fourfold in tumors with a diameter greater than 2 cm when compared to those with a diameter less than or equal to 2 cm. The most commonly metastasized lymph node level, in both large and small tumors, was the first; however, one-fifth of the patients had simultaneous lymph node metastasis in all three axillary levels. Although the left breast was the most affected (58.9%), there was no evidence of a different risk of metastasis between the two breasts; 34.1% of the tumors were multifocal. Lymph node involvement was higher in women under 50 years of age with a primary tumor larger than 2 cm.

Antitumoral activity of human fibroblast interferon administered intranodularly.

Human fibroblast interferon was given intranodularly to 14 patients with cutaneous metastases from breast cancer and malignant melanoma; 1,000,000 units/cm3 of tumor tissue was administered daily for 8-10 days. 13 patients were evaluated. Complete response was achieved in 1 of 7 breast carcinoma nodules and in 2 of 6 melanoma nodules; partial response was achieved in 1 of 7 breast carcinoma and in 3 of 6 melanoma nodules. The overall objective response was therefore 7 of 13 (53.8%) metastatic nodules. Pathological complete response was confirmed in 2 of 3 complete clinical responses. Necrosis, lymphocytic infiltration and fibrosis were observed in all the specimens pathologically examined. In spite of a clear antitumoral activity, this approach does not appear to have clinical significance due to its extremely localized effect.

Kaposi's sarcoma and HTLV-III infection. Virus-like particles in skin lesions: experimental observations.

Skin biopsies from a subject affected by KS and AIDS were examined by means of EM. Samples were obtained both in correspondence of a typical nodule, and from an apparently normal area. The disease process had started at least 8 mo. before, and the patient showed a reduced number of OKT4 cells, with an inversion of the normal T4/T8 ratio. Besides that, high titre antibodies versus HTLV-III were present, whereas antibodies versus CMV and EBV were not demonstrable. Light and electron microscopy demonstrated the typical picture of KS, with marked proliferation of undifferentiated endothelial and spindle-like cells within a network of collagen fibers. Neoformed capillaries very rarely showed an organization comparable to small vessel of normal dermis. Retro-virus-like particles recalling the ones described in lymph-nodes of subjects affected by AIDS by Armstrong et al. (1984) were observed in the skin tissue obtained in correspondence of the Kaposi lesion; the same were either isolated or gathered in small groups within cytoplasmic vescicles. Similar particles were not evidenced in the tissue obtained from normal skin. The interpretation of morphological aspects requests of course a great caution. However the observation seems to be noteworthy, specially if one considers the demonstrated association between retro-virus of HTLV-III group and AIDS, and that its unusual frequency as well as the malignancy in an immunodepressed host are as yet poorly understood.

Cytophotometric analysis of the nuclear uptake of adriamycin in in vivo normal mouse liver and kidney cells.

Adriamycin levels are usually determined in biologic samples (tissue homogenates) by means of spectrophotofluorimetric procedures. After intravenous administration of a pharmacologic dose of Adriamycin to 21 female Swiss mice, the fluorescence released from the nuclei of single liver and kidney cells was determined by cytofluorimetric measurements on 3,916 in-vivo-treated liver cells and 1,472 in-vivo-treated kidney cells. Similar measurements were made on 287 untreated (control) liver cells and 288 untreated kidney cells as well as on 541 liver cells and 773 kidney cells stained in vitro with acriflavine. The results indicate that (1) it is possible to quantitatively evaluate the nuclear uptake of Adriamycin in in vivo single cells by cytofluorimetry, (2) the pharmacokinetics of Adriamycin can be evaluated on the cellular level and (3) Adriamycin can be considered a fluorochrome suitable for in vivo staining of cell nuclei as can acriflavine for the in vitro staining of cell nuclei.

Value of multiple forceps biopsies in assessing the malignant potential of colonic polyps.

Fifty-nine colo-rectal polyps were detected at endoscopy and repeatedly biopsied before removal by endoscopic snare polypectomy. The aim of the present paper was to evaluate the reliability of multiple forceps biopsies in assessing both the malignant potential and the presence or absence of invasive cancer (IC) in colo-rectal adenomas (CRA). In order to achieve the first objective, the histologic types and the degree of dysplasia have been defined. The data obtained by means of multiple biopsies examination, compared with those of polyp in toto study, show that fractional biopsies were of value in the histologic classification of only the smallest 41 polyps (agreement 88.09%), whilst no reliability of biopsies was demonstrated in the 18 largest polyps (agreement 27.68%). In the field of dysplasia grading, the agreement was 55% and 61% for the smallest and the largest CRA respectively. These last figures are hardly acceptable. Biopsies examination gave also under- and overestimation of the histologic severity and of dysplasia as well as a significant incidence of false negative results in IC detection. It is concluded that polypectomy is the only method which provides adequate material for precise diagnosis, no matter how large a polyp. Therefore it should be performed whenever possible. Finally the authors discuss the management of small sessile adenomas.

Endoscopy in asymptomatics previously submitted to anterior resection for colorectal cancer.

Eighty-two colonoscopies were performed in 58 asymptomatic subjects previously submitted to anterior resection for colorectal cancer. In 6 patients endoscopy disclosed recurrent cancer at the anastomosis; in 1, a cancer synchronous to the primary neoplasia was detected 6 months from resection. Twenty-four polyps were detected in 19 patients. Severe epithelial dysplasia was present in two adenomatous polyps, and invasive cancer in another. From these findings, it appears that an endoscopic periodic surveillance of this high risk group is strongly advisable.

Gastric carcinoma: preliminary data of a cooperative program for early diagnosis.

The results of a program of early gastric cancer detection, carried out in the Oncologic Institutes of Genoa, Forlì and Florence, are evaluated. General practitioners screened subjects, according to possible anamnestic and clinical risk indicators (age, family history, previous gastric lesions, digestive complaints, etc.). The diagnostic consisted of medical examinations, endoscopy with cytology and/or directed biopsy, and possibly air contrast barium X-ray. Of 3,180 subjects examined, 224 had gastric cancers (7 %), 29 of which were early cancers, and 688 had high risk lesions; 1,478 cases of other pathology were detected. The authors stress that the proportion of early cancer is clearly higher than that found in routine diagnosis, yet still lower compared to the results of screening programs carried on the general population. Moreover, the high diagnostic sensitivity of each method in early gastric cancer detection and, on the other hand, the difficulties in the detection of high risk lesions, which require systematic multiple biopsies, are emphasized.
