[Unilateral leukopathia unguis toxica and diffuse hair loss after cytostatic perfusion of an extremity].

We report on a patient with simultaneous manifestation of two different side-effects of a cytostatic therapy, which have to be ascribed to cytotoxic influences of different intensity. In this 37-year-old woman, "Mees-like" transverse white bands appeared on all the nails on the left hand after isolated cytostatic perfusion of the left arm. The nails on the right hand and the feet remained free of visible changes. However, the low doses of cytostatic drugs shunted into the systematic circulation induced diffuse toxic hair loss.

[Initial experiences with a new monoclonal antibody in diagnosis of malignant melanomas].

In a study of 17 patients with malignant melanomas in the extremities the sensitivity and specificity of a new monoclonal antibody directed at melanoma cells (BW 575, Behring) were investigated. The specificity was 100%, but the sensitivity 70%. In 3 cases known foci could not be detected. All nodular melanomas and their metastases were detected (9 patients). Repeated examination during the first 24 h after injection of the Tc-99m-marked antibody, and two-plane investigations made it possible to detect even small tumors less than 1 cm in diameter and subcutaneous lesions where the melanomas absorbed the antibodies. Due to its high specificity, the antibody seems to be a promising aid in deciding the operative strategy.

[Super-selective embolization--an alternative to surgery in acute diverticular hemorrhage].

The embolisation (microspirales) of the diverticle-bleeding of the colon is an alternative treatment to the operative colon resection. Advantages for the most older patients are fewer complications and no risk of anaesthesia. An intensive surgical care after embolisatio is necessary to prevent peritonitis in the case of a colon perforation. The risk of an elective colon resection after embolisation if necessary is less than that of resection in cases of emergency.

[Stereoselective differences of central nervous system-depressive action of a homologous series of 3-alkyl-thalidomide analysis].

The racemates and the enantiomers of the 3-alkyl-thalidomide analogues 2a-c were synthesized via the lactams 1a-c. The absolute configuration of 1a-c and 2a-c was elucidated, their enantiomeric purity established. From racem. 2a 1,3-dimethylthalidomide (3) was synthesized. By means of the hexobarbital sleeping time prolongating effect in rats it was shown that racem. 2a and 3 were less CNS-depressant active than thalidomide; 3 caused a more prolonged sleeping time than 2a. Both enantiomers of 2a (i.p. 200 mg/kg) exceeded in their sleeping time prolongation action racem. 2a (same dose), whereby the (S)-enantiomer was to a factor of 1.8 more effective than the (R). With both enantiomers of 2a time and dose dependency of the effect were investigated. The enantiomers of 2b + c and 1a-c had also a prolongating effect on the hexobarbital sleeping time. Independently of the alkyl group the (S)-enantiomers were always more active than the respective (R)-enantiomers. (S)-1b (200 mg/kg) caused a long lasting loss of the righting reflexes as "own effect". Stereoselective differences between the enantiomers of 2a and 2b were observed only when a dose of 200 mg/kg was applied, whereas the higher activity of the (S)-enantiomers of 2c, 1b + c was obtained already with a lower dosage range.

Optimization of isolated hyperthermic limb perfusion.

During isolated limb perfusion, we studied the impact of limb temperature on the concentration of cytostatic drugs in the tissue to identify a possible selective absorption of cytostatic agents by the various tissues. Ten consecutive patients with malignant melanoma were randomly divided into two different groups and perfused with 1 mg cisplatin per kg body weight. In one group the cytostatic agents were injected under hyperthermic conditions (39.5 degrees C) and in the other group under normothermic conditions (37 degrees C). The platinum concentration in the melanoma was twice as high in the hyperthermic group as in the group under normothermic conditions. In the tumor-free tissue the platinum concentration decreased with temperature while it remained constant in the musculature. This selective concentration of cisplatin in the tumor under hyperthermic conditions is accompanied by fewer side effects. Follow-up will show whether the oncological results will be improved in the hyperthermic conditions as one might expect.

[Controlled distraction as a therapeutic option in moderate degeneration of the intervertebral disc -- an in vivo study in the rabbit-spine model].

AIM: The aim of this study was to investigate the effects of temporary distraction on a degenerated intervertebral disc to characterize regenerative changes associated with disc distraction. METHOD: New Zealand white rabbits (n = 32) were used for this experimental animal study. The rabbits were randomly assigned to one of five groups. 6 animals were loaded for 28 days using a custom-made external loading device to stimulate disc degeneration (G2). In 6 animals the discs were first loaded for 28 days and after 28 days loading time the discs in six animals were treated as dynamic distraction with an external distraction device (G1). In six animals the discs were distracted for 28 days without previous loading (G5) and in six animals the discs were loaded for 28 days and afterwards the loading device was removed for 28 days for recovery without distraction (G3). Six animals were sham operated (G4) without application of axial load. After 28 to 56 days loading and distraction time, the animals were sacrificed and the lumbar spine was harvested for histological and radiographic analysis. Histology was performed according to a degeneration score and disc height was calculated radiographically. For the cell viability examination, the number of apoptotic cells was determined. RESULTS: After 28 days of loading (G2), the discs showed a significant decrease in disc space of the treated segment. Histologically, a disorganization of the architecture of the annulus occurred. The number of dead cells increased significantly in the annulus and cartilage endplate. These changes were reversible after 28 days of distraction (G1). The disc thickness increased significantly to physiological levels as compared to the specimens from the 28 days loading group without distraction. Histologically, the discs showed signs of tissue regeneration after 28 days of distraction (G1). The number of apoptotic cells decreased significantly in comparison to the loaded discs without distraction (G2). CONCLUSION: The results of this study suggest that disc regeneration can be induced by axial dynamic distraction in the moderately degenerated rabbit intervertebral disc. The decompressed rabbit intervertebral discs showed signs of tissue recovery at the cellular and histological levels after temporary disc distraction.
