[Intraoperative autologous blood transfusion. Value in major orthopedic surgery].

Intra-operative autologous blood transfusion (IAT) employing a cell separator has proved an effective and safe means of reducing the amount of homologous blood needed for major orthopedic operations. Homologous blood was required in only 15 or 26% of patients, who underwent cemented or uncemented total hip replacement. This is significantly less than in a control group without IAT, in which homologous blood was required in 65 and 71% respectively. In 33% of patients, who had an operation of the spine and in 35% of cases of revision total hip surgery homologous blood was necessary in addition to IAT. We feel, that preoperative blood donation is indicated in addition to IAT, in total hip revisions, major surgery of the spine, as well as in special situations when a total hip replacement is planned.

[Medical advice for golfers].

Although golf is an ideal form of exercise for all age groups, complete mastery of the specific movements involved is a prerequisite. The typical golfing injuries and and pathological sequelae are usually the results of faulty driving and hitting techniques. Apart from golfer's shoulder and golfer's elbow, complaints affecting the lumber spine predominate.

[Backache--causes, diagnosis and therapy].

Low back pain is considered to be a symptom of a variety of changes and disorders affecting the lumbar spine, the sacrococcyx and pelvis, but it may also be a symptom of disorders affecting the neighboring organs. If these points are taken into account and an accurate diagnostic evaluation including such modern procedures as scintigraphy, CT and MR imaging, low back pain can now--in contrast to 50 years ago--be analysed, and often treated with considerable success. In many cases, conservative treatment proves possible. In the presence of relevant morphological findings, however, various surgical procedures, developed in particular in recent years, offer new possibilities.

[Surgical therapy of backache].

Apart from disc prolapse, there is a range of other disorders associated with low back pain, for which, when all conservative efforts have failed, surgical treatment should be offered. It must be emphasized, however, that the indication for surgery needs to be rigorously applied, since the most common cause of failure of surgical therapy is an erroneous surgical indication. Here, the clarification of the cause of the pain and not the diagnosis per se is decisive. In particular in degenerative disorders of the lumbar spine in the elderly, the diagnoses established often have little pathological value. Unfortunately, in many cases the real cause of low back pain still cannot be clarified with certainty. Our experience to date, however, in particular with the use of modern implants, shows that, in cases of lumbar spinal stenosis especially, tumors and pronounced segmental instability, excellent results can be achieved with surgery.

[Capsular ligament injuries of the knee joint. Significance of anamnesis and findings].

Despite the enormous developments in imaging procedures, the patient's history and clinical examination remain the basis for the diagnosis of fresh injuries to the knee. The description of the mechanisms of the accident as revealed by the case history already provides important clues. The clinical examination must cover all the anatomical detection of lesions of individual parts of the capsular ligaments. With an appropriate knowledge of anatomy, and adequate experience in performing the examination, the general physician (with no specialized orthopedic knowledge) can obtain a good idea of the nature and severity of the injury.

Restriction enzymes MspI, HpaII, HaeIII, and HinfI applied to chicken mitotic chromosomes.

The restriction enzymes MspI, HpaII, HaeIII, and HinfI were applied in situ to chicken metaphase chromosomes to determine if they denatured specific regions to produce banding patterns. Human metaphase chromosomes were treated simultaneously to serve as controls. The MspI, HpaII, and HaeIII enzymes produced no visible banding patterns in chicken chromosomes. There was slight banding on a few chromosomes in HinfI preparations, which was similar to the occasional spontaneous banding observed in chicken chromosome preparations. The results indicate that these four enzymes do not denature C-band regions in sufficient magnitude to be detected in chicken metaphase chromosomes.

[Long-term follow-up of lung function and standing height following scoliosis surgery in children and adolescents].

Previous investigations have reported conflicting results on the development of lung function and body height after operative correction of scoliosis at young age. Partly the number of patients or the duration of observation were relatively small. 120 patients were followed for at least 10 years (10-14, average 12 years) after surgical correction of scoliosis at the age of 18.2 +/- 6.8 years. Standing height and vital capacity were measured 1, 2, 5 and 10 years after operation. Height was expressed as percentile for age and vital capacity as % expected for actual height. The primary diagnoses were idiopathic scoliosis (n = 84, average Cobb angle 90 degrees), congenital scoliosis (n = 20, 83 degrees), poliomyelitis (n = 10, 128 degrees), neurofibromatosis (n = 6, 101 degrees). We found an initial decrease in lung function after 1 year, which was probably due to post-operative immobilization and increased by the gain in body height achieved by the operation. During the rest of the observation period a significant (p less than 0.05) increase of vital capacity (% of expected for actual height) was observed. The gain was maximal for idiopathic scoliosis (+6%). In congenital scoliosis body height %ile and relative vital capacity appeared fixed at the preoperative level. Interestingly patients with an initial vital capacity below 50% expected (n = 29) or preoperative Cobb angle above 90 degrees showed the best results. They also lacked the initial deterioration of lung function in the first postoperative year and had a significant gain of vital capacity (+14%) over 10 years.(ABSTRACT TRUNCATED AT 250 WORDS)

[Self-healing cecal fistulas for securing anastomoses of the colon and rectum--is this still indicated?].

UNLABELLED: During a period of 2.5 years self-healing coecostomies according to Stelzner (1970) were applied in 48 elective procedures to the left colon. The following operations were done by 16 different surgeons: left side hemicolectomy 5, sigma resection 24, anterior resection of the rectum 19, 13 of which had anastomoses less than 8 cm. RESULTS: Mean postoperative hospital care 25.3 days, no clinical manifestation of anastomotic leaks, first passing of stool after a mean of 8.5 days. Lethality of 4.2% independent of anastomoses, spontaneous occlusion of fistulae after 42 +/- 26 days. 21% local not life-endangering complications occurred.
